I. THE FOLLOWING CHANGES ARE TO BE MADE TO THE SOLICITATION:

1. The proposal due date is changed on the SF 1442 page 1 as follows; Block 13. A. of the SF 1442 is hereby changed to read 14 July 2021; proposal receipt time remains unchanged at 1:00 PM (CST).

II. THE FOLLOWING CHANGES ARE TO BE MADE ON THE SEED PROJECT:

1. Drawing A200 - Room Finish Schedule is hereby replaced in its entirety with the revised attached Revised Room Finish Schedule.

2. Drawing HA101 Detail 2 is revised to include Note 6.

III. THE FOLLOWING QUESTIONS AND ANSWERS / CLARIFICATIONS ARE THE RESULT OF BIDDERS REQUEST(S) FOR INFORMATION:

Q1. - Volume 1 part 1 Tab 2 - Can the Superintendent and PM share the roles of QC and Safety?…i.e Can the Superintendent share the role of Safety and PM share the roll of QC Manager? (no exceptions) A1. - The Prime Contractors Superintendent or Project Manager can also be the designated SSHO. Quality Control Manager must be separate from the SSHO. Contractor to identify and verify all roles at the Pre-Construction Conference.

Q2. - Volume 1 part 2 Tab 1 – Are the PPQ’s counted toward the 25-page count? I know they are sent directly to you from our clients but wanted to make sure they wouldn’t be inserted into our binders and counted as the 25 pages as each one I believe is 5 pages.

A2. – No, they are not part of the page count.

Q3. - Volume 1 part 1 Tab 4 – The Pre-Award Contractor Safety and EMR Form (Attachment 1) states to include OSHA 300 & 300A forms for 4 years. With attachment 1 this would take up 12 pages of the 25-limit total for technical. Are the OSHA 300 & 300A counted towards the 25-page limit?

A3. – The 25-page limit is to be applied for the entire Volume 1 Part 1 Technical Index which is to include the following tabs: 1 – Company Experience, 2 – Project Management, 3 – Safety Plan, 4, Pre-Award Contractor Safety and EMR form, 5 – Contractor General Information, 6 – Financial Disclosure Information, and 7 – Contractor Surety Limitations. NOTE: Under tab #4, the filled out 1-page (solicitation attachment 1) form is to be included in the 25-page limit, however, the supporting documentation (OSHA 300 & 300A and Insurance Carrier’s EMR letter) may be included as an attachment at the end of part 1 and WILL NOT count towards the 25-page limit.

Q4. - What is the minimum number of projects required for the technical proposal?

A4. – There is not a minimum

Q5. - What is the maximum number of projects allowed for the technical proposal?

A5. – There is not a maximum, you just need to ensure that you do not exceed the 25 page limit.

Q6. - If a past performance project has a completed CPARS will a PPQ be required or can the contractor provide the necessary information for access to the CPARS?

A6. - The submission instructions on page 61 of the solicitation are clear, and further clarified on page 63 paragraph 5.3 for past performance. The Past Performance Information Retrieval System (PPIRS) is reviewed for all respondents which contains all of your past CPARS evaluations.

Q7. - Currently one area has 6” rubber base while the other area has 6” coved sheet vinyl. The spec’s call out 4” rubber base. Is 4” correct for both areas? The walls will need to be repaired if using 4” base.

A7. - All Vinyl Base (WB1 & WB2) on this project to be 4”. General Contractor to facilitate all prep and repairing of walls in all project rooms where existing 6” base is removed.

Q8. - The spec’s call out the cap metal and cove backer strip for coving the SV-1 material. Room Finish Schedule only calls for WB-1 in this area. Please confirm that there is only WB-1 in this area.

A8. See revised Room Finish Schedule.

Q9. - Room A26A is scheduled for the flooring to be demo’d. A26A is not on the room finish schedule. Does A26A get the same carpet as A26?

A9. Room A26A to receive C-1. See revised Room Finish Schedule.

Q10. - Forbo calls for a “net fit” seam as the preferred installation method for the SV-1. Please confirm that the VA does not require a welded seam.

A10. - Provide to install the “net fit” seams instead of the specified welded seams at SV-1. Flooring contractor to follow manufacturers’ recommendations for SV-1 installation and ‘net fit’ seam installation.

Q11. Ref drawing AD100-- Keynote 8B & 8G- Will the VA decontaminate both hoods? Who is responsible for hood certification for the relocated hood? Will the hood filters, which are very delicate and could be damaged during relocation, be replaced by the VA after relocation?

A11. - Part 1: The VA will decontaminate and decommission each hood prior to Contractor disposal and/or relocation.

Part 2: The VA will provide for hood certification after relocation.

Part 3: Fume Hood located in Room A26 to be disconnected, relocated, and reconnected by the Contractor. See additional information in Q93, A93. The contractor is responsible for any damage incurred to the hood during the relocation process.

Q12. - Ref specification 01 00 00 1.6 Phasing- How many days are allowed onsite for each phase of the seed project? How many total contract days will be allowed from NTP? What is the anticipated date for the seed project to be awarded?

A12. - Part 1: Per Project Spec 01 00 00 1.6, G; Contractor shall furnish the COR with a schedule of approximate phasing dates on which the Contractor intends to accomplish work in each specific area of site, building or portion thereof. Contractor shall complete each Phase within the allotted amount of contract completion days.

Part 2: As noted on the Bid Schedule - Contract Completion Days: 270 days from NTP.

Part 3: Anticipated award date is 9/30/2021 Q13. - Ref drawing A100 detail 2- Who provides and installs the 5H Lateral file in room DB1-229?

A13. – VA will supply and install the 5H lateral File as shown in Room DB1-229.

Q14. - Ref drawing A100 detail 1 & 2- Who is responsible to provide and install office furniture in room DB1-232? Who is responsible for tables and chairs in classroom A24?

A14. - Part 1: VA will supply and install the office furniture in Room DB1-232.

Part 2: VA will supply and install the office furniture in Room A24.

Q15. - Ref drawing A10G detail 2 keynote 5- What type of furniture needs to be adjusted and will it require cutting and or patching? Can photos be provided of the area?

A15. - Disregard Keynote 5. VA will move and /or relocate any furniture in Room A1035 as needed to accommodate the chase wall. Contractor may take photos during a prearranged site visit. Contact the Project CO/ COR.

Q16. - Ref spec 08 71 00- The hardware spec is an older spec still using the hardware codes F##, ROY###, LO###, J## - Would it be possible to get further details brands, model numbers ETC. to help match up with what is on site?

A16. The hardware codes are ANSI codes and are still standard in the industry. We cannot specify specific brand products and model numbers. Examples of commonly used hardware are: Latch sets: Sargent/Medeco, Wall Stops: Ives, Auto and other door Seals: Pemko, Kick plates: Rockwood and Closers: LCN.

Q17. - Ref spec 08 14 00- The wood veneered doors appear to require 2 finishes, one stained and one primed for field painting. For the primed, do you want a wood veneer like a paint grade birch or a smoother surface finish like a MDO/MDF?

A17. Primed wood doors to be paint grade birch.

Q18. - The SF1413 form states an Original and 3 Copies, which would be (4) Hard Copies, but the preparation instructions just state (3) hard copies and (1) Electronic.

A20. No, Quality Control Manager must be separate from the SSHO.

Q21. Can the superintendent be both the SSHO and the QCM?

Q22. Is project tax exempt?

Q23. Due to the existing COVID-19 restrictions, can this proposal be emailed to a designated person in lieu of hand deliver?

Economic Price Adjustment for Labor and Material Due to the fact that this solicitation requires us to “hold” our pricing for 180 calendar days for acceptance and given the recent uncertainty in materials and labor costs; would the government consider including the FAR Clause 52.216-4, Economic Price Adjustment for Labor and Material in this solicitation?

Q28. What is existing roof type for work required? Is existing roofing under any warranty?

A28. Roof of 2nd floor D-wing is a Firestone Ultraply TPO Roof System, installed in 2010 with a 15 Year Warranty.

Q29. A Wing Safety Office A26: Cubicles are furnished & installed by VA?

A29. Cubicle furniture to be installed under separate contract with the VA.

Q30. A Wing Conf Rm A1035 millwork for modification. Provide photographs for accurate pricing.

A39. No, work is not occurring in the elevator shaft. Work does occur in the vertical duct chase. See note on Drawing HA100, Detail 1 – “Duct to 8th Floor Room D8201 to be removed”. Contractor to remove/abate the 16” round duct from Ground Floor up to 8th Floor fan room D8201 in its entirety.

Q40. Sheet HA101: Note 4: Is floor to be patched where ductwork is removed to 9th floor level? If closure is required provide specification and construction required for closure of floor.

A40. The floor in Room D8201 does not have to patched.

Q41. Sheet HA101: Note 4: Is wall louver to be removed at 9th floor level or can louver remain and opening be sealed on building interior.

If louver is to be removed, what is exterior wall construction and what is to be used in closing of wall construction on 9th floor.

A48. It is not clear what temp opening you are referring too. Each construction area requires temporary construction barriers as described on AD 100. The contractor can use their discretion to locate the rated access doors in these barriers after review and approval of the VA/COR. We do not anticipate the need for any additional temporary openings – if after a site inspection you think they are required review this finding with the VA/ COR.

Q49. Sheet AD100, Gen Demo Notes Are existing elevators able to accommodate weights of materials to be relocated and will materials fit through elevator doors?

A49. Elevators are rated at 4000 Lbs. The existing elevators are currently in use moving materials for similar interior renovation projects.

Q50. Sheet AD100, Gen Demo Notes Will relocation occur during normal business hours?

A50. The moving of equipment and other will take place during normal business hours. See Specification Section 01 00 00, Part 1.6, G, Construction Sequence.

A56. We believe a utilities tunnel runs parallel to column line C- this tunnel is 7’-0” wide. The trenching for the recessed power and data work is south of this tunnel. We assume the slab south of the tunnel in a 6” slab on grade with a 1 ¾” concrete topping.

The means and methods for the installation will need to be reviewed by the COR and the Engineers of Record.

Q57. A Wing Demolition Plan - Provide details of existing handrailing to be removed and salvaged.

A57. Existing corridor handrail is IPC brand model 1200. Existing wall protection is IPC rigid vinyl sheet product. Existing wall guard bump rail is IPC brand Model 700.

Q58. Sheet A100 A. Casework: What is existing casework basis of design for matching of existing.

A58. The new cabinets will not match the relocated. Basis of Design for new cabinets is Mott Manufacturing, see Section 12 31 00 Part 2 – Products for more information.

Q59. A Wing New - What is slab thickness at new floor trenches?

A59. See response to Q56 above.

Q60. A Wing New - What is concrete detail for infill.

A60. Trench infill to be lightweight concrete, 3000 psi, to meet ASTM C33/C33M aggregate. Submit concrete mix design to COR for approval. Fill bottom of trench with 1”-2” of sand. Final 4”-5” with concrete fill. Finish concrete smooth & prep for carpet layover. Floor shall be prepped and level. See Spec Section 09 65 16 for leveling requirements.

Q61. D Wing New, Equipment Closet DB1-229A. Does wall A need to be rated? What is rating to be?

A61. Closet/corridor wall which is part of a smoke barrier be constructed as a 1-hour wall - smoke tight. The other walls do not need to be 1 hour rated.

Q62. Question #: 32, Sheet: MH102, Spec: N/A, Topic: Wall Penetration Details Question: Please provide wall penetration details for duct penetration detailed on MH102 New Plan Note.

Q63. Provide base specification for patch and repair.

A73. No specific LEED certification is required for this project.

Q74. Can all work be done during normal daytime work shifts?

A75. Yes, an offer guarantee is required for the SEED project, see block 13. b. of the SF 1442. In addition Payment and Performance bonds will also be required for the SEED project.

A83. See Specification 01 35 26, Para 1.4. f for OSHA card submission requirements. OSHA cards do not have to submitted with each AHA. APP & AHA should identify the Competent Persons for the submitted task. All requirements should be submitted prior to starting the task.

Q84. IC walls to deck? How do we know there is room to build walls to deck?

A84. We are not clear on the meaning of “IC Walls”. Partitions called out to extend to deck above are either required to do so because of the need for a fire rating, or to contain sound. Notify the COR to review existing conditions at all areas where there are impediments to the construction of partitions required to be “up to the deck above”. See Specification Section 01 35 26 para 1.12. and Drawing AD 100 Construction Barrier Notes.

Q85. Is full time SSHO required?

A85. Project requires a full-time site Superintendent/SSHO? The Contractor shall assume full responsibility and liability for compliance with all applicable regulations pertaining to the health and safety of personnel during the execution of work. The Government shall not be held liable for any action on the part of the Contractor, his employees or Sub-contractor, which result in illness, injury or death. The contractor shall also comply with VAAR 852.223-71 SAFETY AND HEALTH.

Q86. Can paint be sprayed on? Spec says spray with approval of COR

A86. Provided proper procedure are used to ensure OSHA mandated hygiene and protection of adjoining areas. All paint application shall follow Specification 09 91 00, Para 3.6.

Q87. Detail #2 on Sheet A500 calls for the Jambs of the HM frames to be grouted, But detail #1 does NOT call for the heads of the frames to be grouted. Please confirm this is correct.

A87. The head of the door frames do not require to be grouted, only the vertical frames.

Q88. Question #: 29, Sheet: NA, Spec: 08 14, Topic: Wood Door Veneer Question: For the face veneer on the wood flush doors, would wood veneer like paint grade birch or a smoother surface finish like MDO/MDF be preferred?

Q92. HA100, Demolition Plan note call for Fume Hoods to be removed / Salvaged. Have the hoods been de-contaminated, and certified as clean for removal / relocation?

A92. VA will decontaminate the hoods prior to relocation.

Q93. Do the fume hoods have transite panels located inside the cabinet? Can information on the hoods be provide for review? (Make, Model, Dimensions, weight, etc)

A93. Unknown if the fume hood contains transite. Hood to be relocated is manufactured by Kewaunee Mfg Co., Model number unknown. Weight unknown. Approx dimensions: base cabinet 72”w x 24”d x 32”t. Upper hood cabinet: 72” w x 39” t x 37” d. Upper Cabinet, some disassembly required. The sheet metal side & front panels require removal prior to moving.

Q94. HA 101, Keynote 4. Shows demolition of ductwork up to an existing louver. Please advise if louver is to be cleaned, decontaminated, and certified clean by the abatement contractor.

A94. Exterior wall louver to remain in place as is.

Q95. HA 101 Keynote #4, Existing louver appears to remain. Should the louver be capped from inside and sealed?

A95. Cap existing remaining exterior duct / louver from inside. Cap with 16 gauge sheet metal and seal all seams.

Q96. AD100 – Key note 8D calls for equipment to be “Prepared for relocation by Hitachi...” Is this cost to covered by the VA? If NOT, please provide Hitachi Representative contact Information for Pricing by the general contractor.

Q97. AD100 – Keynote 8E calls for equipment to be relocated, coordinate with Hitachi. Its this equipment being moved by Hitachi, and is the VA covering this cost? A97. Intent of Notes 8D & 8E: Contractor provide to move the electron microscope, pump and chiller. Move from A26A to new room DB1-228.

Anticipated Move Sequence:

A98. Existing Flammable Waste Cabinet is not to be refinished. Cabinet will be reused as is.

Q99. AQ100 -In the equipment schedule GR1 is listed as a glass drying rack and is to be relocated. Please advise what type of drying rack this is and if it contains ACM. As relocation may not be able to be done by General Contractor if this contains ACM.

A99. Glass drying rack to be relocated from A26 to DB1-231 and remounted by contractor. Rack is a stainless-steel rack and contains no ACM.

Q100. AQ100 – In the equipment Schedule the MICR1 it is indicated that this item is relocated and Installed by the Owner, but also Contractor installed. Please clarify who is to relocate and install this item.

A100. MICR1 refers to the smaller tabletop microscopes. VA will relocate and reinstall these microscopes.

Q101. AQ100 - In the equipment Schedule the TEM1 it is indicated that this item is relocated by the responsible party to Install is not indicated in the schedule – Please advise.

Q102. MH101 shows the fume hood being used for the general exhaust of the lab. Please advise if the sequence of operation is to have the Fume hood to run continuously, such that the space will remain negative at all times? It appears the design has 1052 CFM being supplied to the lab and 1100 CFM being exhausted thru the fume hood and 713 CFM being drawn thru the existing EA duct serving the room. Does this provide enough air changes per hour to meet the criteria of a laboratory?

A102. Hood exhaust flow is governed by operational conditions including stage of processing and sash height. During all operating conditions, the hood flow is to ensure a negative pressure around the hood

Q103. MH101 – It appears as the design is that 713 CFM is being drawn from the existing Exhaust Air duct serving room DB1-231. Can the general exhaust be drawn from a lab as designed? Please advise.

A103. Yes, hood exhaust is a dedicated exhaust with its own fan and ductwork. The general exhaust is drawn from general area only.

Q104. MH101 – There is an exhaust grill located in room DB1-229A that is connected to the existing EA duct. Please confirm that 80 CFM is to be drawn from this and is there a need for a damper to control / balance the CFM for this room?

A104. Mechanical dampers are to be provided at each location air flow is indicated. Dampers need to be set by TAB/general contractor to ensure stable operating conditions during the various stages of processing.

Q105. Note G4 on sheet A100 calls for solid Surface panels to be installed over Cement backer board. Who is to supply and install the solid surface panels? The Room Finish Schedule calls for all walls to be painted. No information on the drawings or in the specifications. There is no specification for Solid Surface panels and the only place anything can be found regarding “Solid Surface Materials” is in specification section 09 60 00-3 (page 396), under section “06 20 00, Joint Sealants” Please advise as to what is required.

A105. Eliminate the solid surface panel and backer board requirement as shown on Drawing A200, Detail 10. Install backer board and solid surface material as shown on Drawing A200 Detail 1.

The demolition plans do not call for existing drywall to be removed to allow for cement backer board to be installed. Is the cement backer board to be installed over the existing wall surface / finish? If so is there a detail for the finishing of the edge of the cement board / Solid Surface wall panels?

A107. Exhaust duct and new fan: Point of suction thru the point of discharge to the ambient. Existing exhaust to existing duct connection. Clean existing duct within 2’ of new points of connections.

Q108. HVAC drawings currently show drains connecting to plumbing sanitary waste piping. Unless this project has a variance, the Wisconsin plumbing code indicated these drain lines need to go to the clear water system. Please clarify.

A108. Plumbing drain discharges are based on existing practices at a Federal Government hospital that also predates the latest State codes. Facility does not have a two-drain system.

Q109. Please confirm that the Electrical contractor is providing and mounting the variable speed drives.

A109. General, Electrical and Mechanical Contractors to coordinate providing and mounting of the variable speed drive.

Q110. Please advise what the Plumbing and medical gas requirements are for the relocated fume hood as there appears to be nothing shown on the drawings.

A110. Relocated fume hood does not require plumbing or medical gas.

Q111. Please confirm personnel requirements for this project. Can (1) superintendent function as the QC person and the SSHO concurrently, or do we need separate people?

Q116. The plan page MD100 says to remove and turn over the hood to the VA on note 2; however nowhere on the new work pages does it say who is to re install the hood with controls. Is the VA staff doing that ?

A116. Fume hood in Room A23 will be decontaminated by the VA. Contractor to disconnect duct work per Drawing HA100. VA will relocate this hood and dispose of this hood. No controls work is associated with the A23 Fume Hood. Fume hood in Room A25 will be decontaminated by the VA. Contractor to disconnect duct work per Drawing HA100. Contractor to relocate hood to DB1-231 and reconnect per plans. Intend of Note 8G on AD100 is for the contractor to move & relocate hood and base to DB1-231. Also see Item FH1 on Equipment Schedule on Drawing AQ100.

According to other plan pages, it does not appear that a sink exists in Room A36B. Please confirm that there is no existing sink in this room and that KN 3 is placed here by mistake.
