Contract No. 676-16-103 Tomah VA Medical Center – Remove Asbestos From Crawl Space

SCOPE OF WORK

Using the drawings, specifications, and supplementary information, remove asbestos insulation from existing piping and asbestos fibers from the soil, and miscellaneous debris in the crawl spaces of Buildings 400, 401, 403 and 424. Where asbestos-containing insulation is removed, new asbestos-free insulation is to be provided and labeled for type of piping, and direction of flow, and that the material is Non-ACM. After abatement work is complete, it shall result in crawl spaces that are free of detectable asbestos.

Drawings, specifications and a scope of work for each building are provided to assist the abatement contractor in asbestos removal, insulation replacement and identity labeling. The building summary in this scope of work identifies materials that were sampled, tested for ACM, and the results. If the contractor should question the presence of asbestos, then testing should be accomplished.

Some ACM but not all has been marked with red paint. There is also debris on the floor of the crawlspaces. These items were also sampled. All debris shall be removed and disposed of properly. The disaster preparedness barrels in building 424 shall be removed and disposed by the contractor.

The abatement contractor shall prepare a written plan for asbestos removal and include methods for protecting patients, staff, volunteers, visitors and contractor employees from exposure. Describe the decontamination process, disposal method and location. Describe procedures in the event of an emergency.

Describe ingress/egress, protective clothing, and rest periods for contractors employees. How will negative pressure be maintained and exhaust be filtered. All crawlspaces are considered "Confined Spaces" and will require Contractor Provided safety monitoring and employee emergency extraction plans and equipment. This plan must be submitted to the COR for approval at least two weeks prior to work beginning.

Additional Information:

Space Utilization Issues – The areas to be abated are located in crawl spaces below the building footprints. In Buildings 400, 401 and 403, the height of the space is limited to approximately 4 feet. The crawl spaces are crowded with piping and equipment, making it very difficult to maneuver through the areas.

Constructability issues – Abatement of the asbestos and replacement of piping insulation shall be designed to minimize disruption and allow for the flow of employees and patients at the Tomah VA site.

Energy Goals – As part of the removal of asbestos insulation, the replaced insulation will be installed to reduce thermal loss in the piping.

Maintenance objectives – To create a crawl space environment where staff and contractors can safely inspect, service and maintain utilities.

Environmental Safety Issues - Environmental safety issues not only include the presence of asbestos in the crawl spaces, but also bacteria, mold, and dust. The contractor will be required to wear

PPE appropriate for the conditions encountered in each crawl space.

Hazardous Materials:

General/Existing Conditions:

The original asbestos-containing insulation on the piping within the existing crawl spaces at Buildings

400 and 401 has generally been removed. There are a few areas in these buildings where suspect mud-type insulation remains on the pipe fittings.

The insulation throughout Building 403 is free from asbestos and is typically fiberglass insulation. There is a foam type insulation on the pumped condensate line that penetrates the west wall of the crawl space.

This was tested and found not to contain asbestos. All walls, columns, and floor will be plastic lined.

In Building 424, much of the steam pipe insulation is original and contains asbestos. At the straight runs, magnesia type insulation is present, with asbestos-containing mud-packed asbestos at the fittings. The remainder of the piping has asbestos-free fiberglass insulation on straight runs, and asbestos-containing mud-packed insulation on the fittings.

All of the buildings have much debris on the dirt floors of the crawl spaces, including old piping, wire, fiberglass insulation, and cardboard. There is material in the debris that contains asbestos, located in various areas throughout the crawl spaces. Some of this debris was observed to be ground into the dirt floor of the spaces.

Asbestos Abatement Requirements:

Proposed Asbestos Abatement Description:

Based on the results of laboratory testing of the samples collected, asbestos-containing insulation and fittings have been physically marked in red within each crawl space for removal and replacement.

Abatement contractor shall also remove all debris and foreign objects not attached to the building structure. Where visible debris is mixed in with loose soil, the soil shall be removed in 3" lifts until deemed to be be clean of ACM. All loose soils to be smoothed and graded, inspected, and COR validated prior to poly installation.

In Buildings 400, 401, and 403, the abatement contractor shall install poly sheeting at the floor throughout the crawl space. The poly sheeting shall be a minimum of 10 mil thick, reinforced, and fire retardant laminate. It will be secured to the concrete foundation walls and columns with continuous poly furring strips and tapcon fasteners. Contractor to provide 20 year warranty for poly sheeting, furring strips, and fasteners.

In Building 424, the abatement contractor shall remove asbestos-containing insulation at steam piping, and at fitting insulation throughout the space. The locations have been physically marked in red. Following the insulation abatement, the contractor shall remove a minimum of 3 inches of asbestos contaminated soil throughout the crawl space. Where suspect debris is found below 3 inches of removed soil, contractor shall remove additional soil until no further debris is visible.

After completion of the soil and debris removal at Building 424, the abatement contractor shall generously apply a penetrating encapsulant to the soil, which must penetrate a minimum of one inch.

Contractor to HEPA Vacume and wet-wipe all surfaces that have accumulated dust and debris prior to poly installation and inspection.

General/Existing Conditions and Recommendations:

The existing insulation at plumbing piping in the crawl spaces of Buildings 400, 401, 403 and 424 has generally been abated and replaced with non-asbestos containing insulation. However, some fittings have been found to contain asbestos and must be replaced after abatement.

Mechanical:

General/Existing Conditions:

The existing insulation at steam and steam condensate piping in the crawl spaces of Buildings 400, 401, and 403 has generally been abated and replaced with non-asbestos containing insulation. However, some fittings have been found to contain asbestos and must be replaced after abatement.

In Building 424, much of the existing steam piping is insulated with magnesia (asbestos-containing) insulation, as well as many of the fittings.

Proposed Mechanical System Descriptions:

Replace insulation at steam piping and pipe fittings where it has been removed due to asbestos abatement. See specifications for type and size of replacement insulation.

Period Of Performance:

The period for construction activities from the date of the Notice to Proceed is 120 days, unless bid alternates are exercised.

T1= Notice to Proceed T1 + 10 days = Pre-construction Kick-Off Meeting scheduled including all sub-contractors, T1 + 40 days = B 424 complete T1 + 100 days = All buildings complete (100%) T1 + 120 days = Project Closeout

Bid Preparation:

Bids shall be prepared to include all items of work as a base bid. Bid alternates shall be provided such that :

Bid Alternate 1: includes the base bid but excludes work to complete building 403. Period of performance to be 120 days.

Bid Alternate 2: includes the base bid but excludes work to complete building 403 and 401. Period of performance to be 90 days.

Bid Alternate 3: includes the base bid but excludes work to complete buildings 403, 401, and 400. Period of performance to be 60 days.

Summary of Findings – Building 400 Crawlspace

The asbestos containing insulation on the piping in the building 400 crawlspace has generally been removed, however, a few areas have been identified where suspect insulation remains on the piping. There are approximately 160-180 pipe fittings with insulation that is suspect to contain asbestos. A total of 8 bulk samples were collected of the remaining suspect insulation. Of these 8 samples collected, 2 of the samples were found to contain asbestos. Based on these results, all the remaining suspect insulation, which is mud type insulation on pipe fittings, is considered asbestos containing insulation.

All other pipe insulation is non-ACM, and is typically fiberglass insulation.

The piping leaving the crawlspace in the tunnel at the North end, is insulated with asbestos containing insulation.

There is much debris on the dirt floor of the building 400 crawlspace. This debris contains many different materials including old piping, wire, fiberglass insulation, cardboard among other items. There is material included in this debris that is suspect to contain asbestos. A total of 7 samples were collected of debris that appeared suspect to contain asbestos. Of these 7 samples, 3 of them were found to contain asbestos. The suspect debris identified to contain asbestos was found in various areas, and not considered localized to any particular area of the crawlspace. Also, the suspect debris observed is not only on the surface of the dirt floor, it was observed to be ground into the dirt floor of the space.

Overall, the crawlspace is a very difficult space to work in. Generally, the height of the space is approximately 4’. The piping and equipment in the space makes it very crowded and difficult to maneuver through the space. There are areas of the dirt floor, that appeared to be discolored from growth of fungus or bacteria. There are areas of the space where the floor was wet at the time of the inspection.

Summary of Findings – Building 401

The asbestos containing insulation on the piping in the building 401 crawlspace has generally been removed, however, a few areas have been identified where suspect insulation remains on the piping. There are approximately 110-120 pipe fittings with insulation that is suspect to contain asbestos. One of the three samples collected of the remaining pipe fitting insulation was found to contain asbestos. Based on these results, all the remaining suspect insulation, which is mud type insulation on pipe fittings, is considered asbestos containing insulation. All other pipe insulation is non-ACM, and is typically fiberglass insulation.

Overall, the crawlspace is a very difficult space to work in. Generally, the height of the space is approximately 4’. The piping and equipment in the space makes it very crowded and difficult to maneuver through the space.

Summary of Findings – Building 403 Crawlspace

The asbestos containing insulation on the piping in the building 403 crawlspace has been removed. All pipe insulation found throughout the crawlspace is non-ACM, and is typically fiberglass insulation. There is a foam type insulation on the pumped condensate line that comes through the center of the West wall of the crawlspace.. This piping is insulated with a foam type insulation with a tar paper-like covering. This insulation was tested and found not to contain asbestos.

The piping leaving the crawlspace in the tunnel at the South end, and the Northeast end is insulated with asbestos containing insulation. These areas go into the tunnels below the pedestrian tramway.

There is much debris on the dirt floor of the building 403 crawlspace. This debris contains many different materials including old piping, wire, fiberglass insulation, cardboard among other items. There is material included in this debris that is suspect to contain asbestos. A total of 5 samples were collected of debris that appeared suspect to contain asbestos. Of these 5 samples, 4 of them were found to contain asbestos. The suspect debris identified to contain asbestos was found in various areas, and not considered localized to any particular area of the crawlspace. Also, the suspect debris observed is not only on the surface of the dirt floor, it was observed to be ground into the dirt floor of the space.

Summary of Findings – Building 424 Crawlspace

Within the building 424 crawlspace, most of the insulation is original, asbestos containing insulation. Some of the steam piping is insulated with asbestos containing, magnesia type insulation on the straight runs, and mud packed asbestos containing insulation on the fittings. The rest of the piping has fiberglass insulation (Non-ACM) on the straight runs, and asbestos containing mud packed insulation on the fittings. The following are approximate quantities of asbestos containing pipe insulation within the crawlspace:

Magnesia Type Pipe Insulation & Fittings – 700 LF Mud-Packed Fittings on Fiberglass Pipe Insulation – 460 Fittings

There are pipes leaving the crawlspace to the East and West. The piping in these areas appeared to be insulated with fiberglass insulation on the straight runs and asbestos containing mud-packed insulation on the fittings. These areas were not assessed as part of this inspection.

There is much debris on the dirt floor of the building 424 crawlspace. This debris contains many different materials including old piping, wire, fiberglass insulation, cardboard among other items. There is material included in this debris that is suspect to contain asbestos. A total of 3 samples were collected of debris that appeared suspect to contain asbestos. Of these 3 samples, 2 of them were found to contain asbestos. The suspect debris identified to contain asbestos was found in various areas, and not considered localized to any particular area of the crawlspace. Also, the suspect debris observed is not only on the surface of the dirt floor, it was observed to be ground into the dirt floor of the space.

There is a spray-applied insulation in the Northwest corner of the crawlspace. This material is applied to the walls and the ceiling of this area of the crawlspace. This material was sampled and found not to contain asbestos.

There is an area with disaster supplies located in the Southwest area of the crawlspace (See attached photographs). These items appeared to be there for many years.
