Upon completion, the contractor must submit a Lockout Summary Report to the VA Contracting Officer's Representative (COR) serving as official certification of the annual inspection. For any equipment without procedural coverage, the contractor is responsible for developing machine-specific energy control procedures including equipment description, location, hazardous energies present, photographs of equipment and isolation points, sequential shutdown methods, required lockout hardware, verification methods, and release protocols. All procedures must be formatted approximately 8.5" x 11" (with modifications only where necessary), color printed, laminated to 10-mil thickness, and installed at the respective machines' main electrical panels or control panels. The contractor must develop a VA-Specific/OSHA template for COR review and approval prior to finalizing all procedures. Additionally, energy identification tags require COR approval before final production and placement.

The contractor must assign at least one project team member as a Certified Safety Professional (CSP) with a minimum of ten years' experience in Lockout/Tagout compliance to attend the survey at Battle Creek VAMC. The contractor must demonstrate OSHA compliance experience, familiarity with VA guidelines, and provide a minimum of five references for previous LOTO audits, workplace safety consulting, and OSHA compliance work, with prior VA experience preferred.
