About this file
This is an Amendment to a Solicitation/Modification of Contract (Standard Form 30) issued by the Department of Veterans Affairs Network Contracting Office in Kettering, Ohio for solicitation 36C25026Q0356, effective March 26, 2026, with an amendment date of April 30, 2026. The amendment provides responses to a Request for Information (RFI) regarding the flooring replacement project at the Richard L. Roudebush VAMC campus.
The RFI responses address fourteen contractor questions covering critical project specifications and requirements. Key clarifications include: (1) the VA will accept a dual-role Construction Supervisor and Safety Officer if properly certified, rather than requiring a dedicated full-time SSHO; (2) grinding existing cove base in Operating Room A-4076 is acceptable instead of removal, provided the finish meets specifications; (3) floor plans and as-builts have been provided as attachments; (4) the overall resinous flooring system thickness shall be 3/16" to 1/4"; (5) ICRA Precaution Level IV with class C construction standards are required; (6) contractors must allow for substrate repairs per specifications, as the VA cannot guarantee existing substrate condition; (7) padded flooring in the Mental Health ward shall be installed during normal business hours while operating room epoxy flooring is installed nights and weekends; (8) bed fasteners must penetrate both existing and new padded flooring; and (9) alternative payment protection methods are acceptable per FAR 52.228-13. The amendment also references attached as-builts, a VHA ICRA form, and a pre-bid site visit sign-in sheet. All other terms and conditions of the original solicitation remain unchanged.

RFI Q&A
Q1: Can we get floor plans of the areas of work?
A1: Please see the as-builts attached to this amendment.
Q2: Paragraph 1.9 A & C, 01 35 26-16 states:
The Prime Contractor shall provide a minimum of one “Full-Time” SSHO at each project site, for each shift (with no other duties). A dedicated SSHO for this size of project would add considerable cost. Is this what the VA wants?
A2: The VA will accept a dual-role of Construction Supervisor and Safety Officer as long as the individual(s) proposed obtain and submit all required certifications for each position.
Q3: In the OR, removing the cove base would cause significant wall damage. It has been recommended that the material on the base be ground to accept the new material instead of removing. Is that acceptable to the VA?
A3: The VA will accept grinding of the existing cove base to install the new material over top, as long as the resulting finish meets relevant specifications.
Q4: I have requested floor plans earlier. After the walkthrough, the flooring contractors are requesting floor plans to get accurate measurement, especially linear dimensions for the cove base. Can we get a floor plan?
A4: Please see the floor plans attached to this amendment.
Q5: We have reviewed the solicitation documents and specifications and were unable to identify any insurance requirements, including applicable FAR clauses or minimum coverage limits. Please confirm the required insurance types and limits for this project.
A5: IAW FAR 28.310 this solicitation does not meet the threshold requirements for proof of additional insurance coverage.
Q6: Please provide a dimensioned floor plan for Operating Room A-4076.
A6: Please see the as-builts attached to this amendment.
Q7: Would it be acceptable to retain the existing wall cove base in Operating Room A-4076 and sand it down for preparation prior to recoating with new epoxy flooring? Full removal of the existing wall base may damage the walls, and we would like to avoid that additional work if possible.
A7: The VA will accept grinding of the existing cove base to install the new material over top, as long as the resulting finish meets relevant specifications.
Q8: Do you know if a bed will be bolted down in the room where the padded floor will be installed?
A8: The beds currently in use by the Mental Health ward will require fasteners in the flooring. Current penetrations in the epoxy flooring may be used, but the fasteners must also penetrate the padded flooring for bed attachment.
Q9: Can the padded floor be installed during normal business hours? I know the operating room epoxy is nights and weekends but want to confirm if the same is true for the area with the padded flooring.
A9: The padded flooring in the Mental Health ward shall be installed during normal business hours. These hours are specified in the SOW.
Q10: Resinous Flooring System: Please confirm the required system thickness and performance criteria for the specified resinous flooring system in Operating Room 2.
A10: The overall system thickness shall be between 3/16" and 1/4" as per specifications.
Q11:Infection Control Requirements: Please confirm if a specific infection control classification (ICRA level) is required, including requirements for containment, negative air, and particulate monitoring.
A11: ICRA Precaution Level IV will be required as the area is of the highest risk category, and the construction will be at least class C. Please see sample ICRA form attached.
Q12: Substrate Condition: Please confirm if the existing substrate should be assumed suitable for installation or if contractors should include allowances for repairs such as leveling or moisture mitigation.
A12: The VA can not guarantee the condition of the substrate. The contractor should allow for repairs as detailed in the specifications.
Q13: Payment Protection: Please confirm if alternative payment protection (e.g., escrow account or letter of credit) will be accepted in lieu of a payment bond for contracts exceeding $35,000.
A13: Please see the list of acceptable alternative payment protection IAW FAR 52.228-13, found on page 34 of the solicitation.
Q14: Drawings / Existing Conditions: Please confirm if there are any available drawings, layouts, or photographs of Operating Room 2 (A-4076) and Room C-5018 that can be provided to better understand the existing conditions and limits of work.
A14: Please see the as-builts attached to this amendment.
