1.0 INTRODUCTION & EXECUTIVE SUMMARY
Mabbett and Associates, Inc. (Mabbett®), with ECS Midwest, LLC as a sub-contractor, performed surveys for suspect asbestos containing building materials (ACM) and screenings of suspect lead-based paint on building surfaces utilizing an X-Ray Fluorescence (XRF) analyzer. Surveys were performed of Building 1 at the Fort Wayne VA Medical Center (Fort Wayne VAMC) located at 2121 Lake Avenue, Fort Wayne, Indiana, under Contract GS-10F-0120T. Site survey work was performed on November 8, 2019, by appropriately credentialed personnel as required. Collected suspect ACM samples were submitted to an analytical laboratory certified as an environmental testing laboratory pursuant to the provisions of the Health and Safety Code for bulk asbestos analysis by polarized light microscopy (PLM) using positive stop methodology. The survey effort involved the collection or screening of the following samples, resulting in the following conclusions:
One hundred sixty-five (165) bulk samples for suspect ACM were collected in this building.
Based on laboratory analysis of suspect ACM, ten (10) of the collected samples contained asbestos greater than 1%.
Six (6) additional materials were previously sampled and identified as ACM or Presumed ACM (PACM).
Eight hundred and twenty-four (824) lead screening measurements were taken in this building.
One hundred fifty (150) lead screening measurements were greater than 1.0 mg/cm2.
This building report consists of a summary of findings, floor plans that indicate positive sample locations, detailed analytical findings for the specific surveyed building materials, relevant photos and laboratory analytical reports.
2.0 BUILDING DESCRIPTION
According to information provided by The Department of Veterans Affairs VA Northern Indiana Health Care System (VANIHCS) and observations made by the Mabbett team at the time of the survey, Building 1 is an occupied six-story masonry and steel structure on a concrete foundation with a flat roof of varying levels, and encompasses approximately 203,056 square feet. The building is the Main Hospital and is used for patient care and administrative purposes and was reportedly built in 1949.
3.0 ASBESTOS SURVEY
3.1 Sampling Methodology
The Mabbett team performed a physical U.S. Environmental Protection Agency (EPA) Asbestos Hazard and Emergency Response Act (AHERA) type inspection and sampling of suspect materials as required by EPA AHERA regulations (i.e. 40 CFR 763), Occupational Safety and Health Administration (OSHA) Asbestos Standards (i.e. 29 CFR
1910.1001 & 29 CFR 1926.1101) and the VHA Asbestos Management Program (VHA Directive 7714).
The VANIHCS provided Mabbett with historical asbestos inspection reports for asbestos surveys that were previously conducted at the Fort Wayne VAMC. The surveys were conducted by USFIN Development, LLC in August 2016. These reports were reviewed by the Mabbett team prior to performing inspection activities. In some instances, ACM identified by the previous surveys was confirmed by Mabbett, reassessed and included in this Asbestos & Lead Survey Report.
A visual screening inspection was conducted to identify locations of suspect ACM throughout identified areas of the building by EPA Asbestos Hazard Emergency Response Act (AHERA) accredited and Indiana certified asbestos inspectors. Copies of inspector accreditation certificates are provided in Appendix A. Only areas that were accessible during the field work phase were inspected. Every effort was made during the initial field survey work to access areas as necessary to complete the survey.

General areas that were inaccessible or where the survey was limited to visual observation only are identified below:

Due to these access restrictions, a comprehensive pre-demolition ACM survey, in accordance with Asbestos National Emission Standards for Hazardous Air Pollutants (NESHAP) regulations, should be performed prior to any proposed renovations or maintenance involving inaccessible areas. Roofing materials were not sampled in order to maintain applicable warranties.
Bulk samples were collected of suspect ACM in accordance with EPA AHERA regulations. Bulk suspect ACM sampling was conducted according to the following sampling plan:
a) Surfacing material:
a. At least three bulk samples were collected from each homogeneous area that is 1,000 ft2 or less.
b. At least five bulk samples were collected from each homogeneous area that is greater than 1,000 ft2 but less than or equal to 5,000 ft2.
c. At least seven bulk samples were collected from each homogeneous area that is greater than
5,000 ft2.
b) Thermal system insulation:
a. At least three bulk samples were collected from each homogeneous area of thermal system insulation.
b. At least one bulk sample was collected from each homogeneous area of patched thermal system insulation.
c. Sufficient samples were collected from elbows and fittings to determine if they contain ACM.
d. Bulk samples were not collected from any homogeneous area where the state licensed asbestos inspector determined that the thermal system insulation is fiberglass, foam glass, rubber, or other non-ACM.
c) Miscellaneous material:
a. At least one bulk sample was collected from each homogeneous area that is less than 100 ft2.
b. At least two bulk samples were collected from each homogeneous area that is greater than 100 ft2.
3.2 Analytical Methodology
The collected bulk samples were submitted under chain of custody procedures to AmeriSci of Midlothian, Virginia.
AmeriSci is accredited by the American Industrial Hygiene Association (AIHA) and participates in the National Voluntary Laboratory Accreditation Program (NVLAP Lab Code 101904-0). Bulk samples were analyzed by PLM using EPA Method 600/R-93/116.
Bulk sample results are summarized in Table 3 – Summary of ACM Sampling Results and included as Appendix B. The EPA and OSHA defines any material that contains greater than one percent (>1%) asbestos as being an ACM. As such, the analytical laboratory identified bulk samples as asbestos-containing materials that met the regulatory

Laboratory reports and the chain of custodies are provided in Appendix C of this report.
3.3 Summary of Asbestos Containing Materials (ACM) Findings
3.3.1 Data Tables, Laboratory Results, and Field Notes
Mabbett sampled sixty (60) types of homogenous materials and collected one hundred and sixty-five (165) representative bulk ACM samples. One hundred and fifty-six (156) samples were analyzed by PLM and thirteen (13) samples were either positive stops or insufficient amounts of material and not analyzed. The current and historical laboratory analytical data indicated that sixteen (16) homogenous materials had greater than 1% asbestos and were ACM or presumed ACM (PACM). Estimated quantities of materials identified as ACM or PACM are presented in Table 1 below:

3.3.2 Photographs
Representative photographs of ACM Homogeneous Areas are provided in Appendix D of this report.
3.3.3 CADD Drawings
The location of each collected bulk sample and its abridged sample ID is shown on the CADD drawings in the Figures Section of this report. Each identified ACM sample location is colored red and marked with an asterisk (*), while positive stops are marked with two asterisks (**). Collected bulk samples that did not contain asbestos are identified solely by the sample ID. Building areas containing ACM are identified with hatching or symbols to show the approximate location of the identified ACM.
4.0 LEAD SCREENING SURVEY
4.1 Screening Survey Methodology
Mabbett completed a limited lead-based paint screening survey of Building 1 components which included painted building surfaces. The lead-based paint screening survey was performed by a certified lead inspector. The screening survey measured lead concentrations in accessible building surfaces by using a Heuresis Corp. Pb200i Handheld XRF Paint Analyzer (serial number 1752). The XRF instrument was calibrated at the frequency specified by the

Every effort was made during the initial field survey work to access areas as necessary to complete the survey.
4.2 Summary of Lead Screening Survey Findings
4.2.1 Data Tables
A concentration of lead greater than or equal to 1.0 mg/cm2 exceeds HUD residential standards and is an indicator of risk. OSHA does not specify a safe concentration of lead. Painted interior building components screened using an XRF found to contain lead at concentrations greater than 1.0 mg/cm2 have been included in Table 2 below:

A complete summary of XRF measurements collected during the screening survey of interior and exterior building components are provided in Table 4 – Summary of XRF Measurements and included as Appendix E. Table 4 includes a description of each screened surface and resulting XRF-measured lead-based paint concentration.
4.2.2 Photographs
Representative photographs of components identified with > 1.0 mg/cm2 lead content are provided in Appendix F of this report.
4.2.3 CADD Drawings
Based on the results of the lead-based paint screening survey, Mabbett created figures representing all materials with lead content > 1.0 mg/cm2. The information presented on the CADD drawings is for use by the VAMC for exposure assessments and preliminary renovation planning. Based on the results of the survey, these components identified in the CADD table and on the plan should be assumed to be lead-based paint containing unless otherwise determined.

5.0 LIMITATIONS
This inspection report is the result of a diligent search of identified areas of Building 1 for ACM and LBP. Only accessible and authorized areas/materials were included in this survey. Although a comprehensive inspection was performed, Mabbett does not claim to have identified all ACM and LBP that could be present in the facility (for example, materials potentially located within aforementioned inaccessible areas in Section 3.1). Mabbett’s survey was performed with limitations inherent to visual inspections. Mabbett has conducted this assessment with reasonable care and has performed this project within generally accepted industry standards. There can be no assurances, and Mabbett makes no assurances, that the information, research, and technology used to prepare this report may not change in the future, thus affecting the results reported herein.
6.0 CLOSING REMARKS
6.1 Asbestos
Prior to initiating plans for maintenance activities, the Fort Wayne VAMC should review the asbestos survey results to determine whether any of the materials identified as ACM will be disturbed by proposed work activities. This report may not be used as a renovation/demolition NESHAP type inspection report. Prior to any renovation/demolition activities, a full destructive asbestos inspection of the work area including any previously inaccessible or unsampled materials that might be impacted must be conducted.
The purpose of this ACM survey was to create an Operations and Maintenance (O&M) Plan and to identify ACM in Building 1 within the limitations of the survey for worker protection purposes only and not for future renovation or demolition purposes. In regard to asbestos, any suspect material encountered during maintenance activities that is not identified in this report as being non-ACM should be assumed to be ACM unless laboratory analytical data for bulk samples prove otherwise. Suspect ACM that may be present within the walls, above inaccessible hard ceilings, or in other inaccessible locations, and that accordingly was not inspected, should be assumed to contain asbestos if discovered until otherwise verified. If ACM will be disturbed, the ACM must be removed by a licensed asbestos abatement contractor in accordance with applicable regulations. If proposed work activities will not disturb intact ACM, continuous monitoring of ACM should be conducted throughout work activities to ensure the ACM remains in an intact condition. Additionally, prior to commencing work activities, personnel involved with the work activities should be made aware of the location of ACM within the building in which they will be working. Additional information regarding asbestos management and the Operations & Maintenance (O&M) program is outlined in the VAMC campus-wide Asbestos O&M Management Plan available in Volume I of the Fort Wayne VAMC Survey Report.
6.2 Lead-Based Paint
The purpose of the lead-based paint screening survey was to identify LBP components. For the purpose of this lead-based paint screening survey, representative interior and exterior building components i.e. painted building components were tested. The regulations addressing lead in non-residential buildings are focused on protecting workers involved with lead-based paint disturbing activities and related waste disposal activities.
Worker protection is regulated by OSHA regulations as well as applicable state regulations. These regulations involve air monitoring of workers to determine exposure levels when disturbing lead-based paint materials with measurable lead. A lead-based paint investigation cannot determine a safe level of lead, but is intended to provide guidance as to the locations of where lead containing paint is present. VA employees and contractors may use this information to better determine exposures of workers to airborne lead by understanding the different concentrations of lead paint on representative components and surfaces. Worker exposure controls can then be implemented, and air monitoring can then be performed during activities that disturb lead-based paint on representative surfaces.

A concentration of lead greater than or equal to 1.0 mg/cm2 exceeds HUD residential standards and is an indicator of risk. OSHA does not specify a safe concentration of lead. Prior to demolition of lead containing areas, a Toxicity Characteristic Leaching Procedure (TCLP) should be conducted to ensure that waste material does not exceed
5.0 ppm for lead and complies with the EPA waste disposal requirements for landfills.
