1 (one) The purpose of this amendment is to revise or remove some of the requirements identified in the Performance Work Statement and to revise or remove some of the Evaluation Criteria. These changes were made to address contractor concerns which were identified through the Q&A Phase. Please see the attached (beginning on page 2) for changes.

This amendment is not being extended because amendment A00002 has provided the extension to February 28, 2020 at 3pm ET.

Lisa Bailey Contracting Officer

The following items are amended:

1) Evaluation Subfactor B- Removing the requirement for BLS and ACLS submission through the revision of Factor 1, Subfactor B’s evaluation criteria (located on page 139 & 148). “Subfactor B: Staffing and Experience” is removed in its entirety and replaced with the following:

“Factor 1, Subfactor B: Experience and Staffing- The offeror shall submit a response that includes:

i. A detailed description of the offeror’s length and breadth of experience providing primary care services using the Patient Centered Medical Home Model (known as PACT in VA) for the adult Veteran population age 18 and older within the past five years. Experience should include healthcare personnel management and set-up/operation of an outpatient clinic similar in size and scope to this clinic.

ii. Description of the ability to recruit adequate staffing and retain quality personnel to meet the needs of this contract. Describe your process of completing background checks on employees providing services. Identify the average personnel turnover rate and how it might affect contract performance. Explain how continuity of care will be maintained during periods of vacancy and provide a detailed description of the offeror’s contingency plan for staffing clinic(s) in the event of absence (planned and unplanned) of regularly scheduled staff.

iii. Provide a list of the names and qualifications of physicians, physician assistants, nurse practitioners, RNs, LPNs, and other clinical staff which are intended to be utilized in the performance of this contract, taking into consideration the enrollment numbers for the clinic and the minimum staffing requirements per the PWS. Also provide the name, title and qualifications of the Medical Director to oversee the clinic.

iv. Provide a list of the names and qualifications of administrative support staff and describe the level of training and experience that will be utilized to meet the administrative support functions of this contract, including (but not limited to) such functions as patient scheduling, medical record administration, record processing and reporting, grievance system, and quality assurance and performance improvement.”

2) Evaluation Factor 3, Veteran’s Preference, subparagraph iii – “Past Performance in meeting VA’s department-wide SDVOSB subcontracting goals for 2014 and 2015” (located on page 139). Factor 3’s subparagraph iii is removed in its entirety and will not be evaluated; subparagraphs i, ii, and iv remain in effect.

3) Paragraph 6.1.2. (page 99) currently states: “The Contractor shall have ninety (90) days from contract award to commencement of the provision of medical care to local Veterans. However, the Contractor shall have all start-up requirements in place and ready to commence operation NLT eighty-three (83) calendar days from contract award. The final seven (7) days will be used for training and resolution of any last minute or unexpected technical or personnel related challenges.” This paragraph is revised through this amendment to now state: “The Contractor shall have one hundred twenty (120) days from contract award to commencement of the provision of medical care to local Veterans. However, the Contractor shall have all start-up requirements in place and ready to commence operation NLT one hundred thirteen (113) calendar days from contract award. The final seven (7) days will be used for training and resolution of any last minute or unexpected technical or personnel related challenges.”

4) Paragraph 4.6.2.4.8. (page 37) currently states: “Women Veterans must have women-only or unisex toilet rooms and bathing/shower facilities. Appropriately locking door hardware with locking mechanisms providing for privacy, safety, security, and utility (allowing staff members to have key or code access in case of emergency) are required for all toilets, baths and showers.” The requirement for bathing/showering facilities is removed in its entirety through this amendment and now states: “Women Veterans must have women-only or unisex toilet rooms. Appropriately locking door hardware with locking mechanisms providing for privacy, safety, security, and utility (allowing staff members to have key or code access in case of emergency) are required for all toilets.” There is no requirement for a bathing/showering facility. The remainder of the paragraph language remains in effect.

5) Vending Machine Area: PACT Table Requirements (page 80) currently lists a vending machine area. This is not a requirement and is removed from the PWS through this amendment.

6) Paragraph 4.6.4.1. (page 43) states “the Contractor shall provide two (2) GE Healthcare Discovery XR656 Plus Base System”. Only one (1) is required. This paragraph is revised through this amendment to now states: “the Contractor shall provide one (1) GE Healthcare Discovery XR656 Plus Base System.”

7) Paragraph 3.1. (page 27), 4.6.4.3. (page 43), 4.6.6.1. (page 63), 4.9.7. (page 82), 4.9.13. (page 84) have conflicting Clinic hours listed. The clinic hours are revised through this amendment to now state: “Clinic hours are 8:00am to 5:00pm Monday through Friday.”

8) Why does the RFP require so many exam rooms? The CLINs is only 3,281 enrollees in the Base Year and declines to 3,044 over the nine option years. However, the RFP space requirement of a total of 22 exam rooms suggests more PACT teams that the enrollment requires. For example, if there are three rooms per PACT team and 16 exam rooms as shown in the RFP, this would accommodate 5.1 PACT teams. Or four exam rooms per team would accommodate four PACT teams. Is this correct? After reviewing the current number of enrollees and the anticipated number of enrollees for the new CBOC’s delineated area, the VA anticipates that 4 PACT teams are more appropriate. The PACT model table found under section 4.9.1, beginning on page 76 is removed in its entirety and replaced with the following table:
