Lisa Bailey Contracting Officer Questions Received from Vendors
1. Is it acceptable to submit BLS and ACLS after award instead of with the proposal (page 139, point Bii)? We’re going to revise Subfactor B to remove this. An amendment to update this is to follow.
2. Is it acceptable to submit transcripts after award instead of with the proposal (page 139, point Biii)? We note that transcript requests can take some time to process by the university and that these time lines are outside the control of the contractor and the candidate. We’re going to revise Subfactor B to remove this. An amendment to update this is to follow.
3. Various parts of the Solicitation refer to a “Clinic Manager” or “Clinic Nurse Manager”. Is the Contractor required to provide an on-site clinic manager? If so, is this person required to be an RN? Or is this position provided by the VA? The contractor is to provide the Clinic Manager and does not have to be a RN
4. Can the Government indicate the reason(s) that the patient population is predicted to decline over the contract period? The declining population is anticipated due to the largest group of veterans in the area being <65
5. Paragraph 4.9.11 refers to an emergency generator. Is the contractor required to provide an emergency generator at the Goshen CBOC? Yes, the contractor is required to provide a generator.
6. Paragraph 2.1 states, “The Clinical Pharmacy Specialist (CPS) shall be provided the same support staffing given to other providers on the team when they are working in the capacity of a mid-level provider.” Is the contractor required to provide any additional support staff for the CPS other than what will already be provided in the context of primary care teamlet staffing under section 2.3? No additional staff will be required.
7. Paragraph 6.1.2 states, “The Contractor shall have ninety (90) days from contract award to commencement of the provision of medical care to local veterans.” In our experience in opening VA CBOCs, we find that this time frame is very difficult to meet and meeting it may be associated with significant costs. We have also noticed that recent Solicitations for CBOC services are providing a 120- or 150-day time frame for clinic start up. Would the Government be willing to adjust this time frame to 120- or 150-days? The government is willing to adjust no more than 120 days. An amendment to update this is to follow.
8. Will the Contractor be required to provide a dedicated enrollment clerk for the completion of the requirements of paragraph 4.2.1? No, the enrollment clerk is provided by the VA and will be located at the NIHCS main campus.
9. Please clarify if the Contractor is responsible for providing breast cancer screening (section 4.6.2.1) or if the VA will provide this. The contractor is to provide breast cancer screening.
10. Paragraph 4.6.3.1 states that the contractor shall be responsible for emergent 10-day prescriptions, but then states that “authorization shall occur BEFORE sending the patient to the local pharmacy to ensure the prescription will be filled”. Please clarify if the contractor is responsible for providing emergent prescriptions for up to 10 days. The contractor is responsible for providing emergent prescriptions for up to 10 days. *Medications not on the emergent formulary list must be approved by the NIHCS Pharmacy BEFORE sending to the local pharmacy*
11. Paragraph 4.11.1.6 states that the contractor is responsible for “The procurement, installation and maintenance of all printers, copiers, scanners, fax machines*, shredders, or other peripheral office equipment and all related and ongoing supplies (paper, toner, ink cartridges) required to operate the equipment in support of the facility under the specifications of this contract.” Please confirm this responsibility will be the Contractor’s under the resulting contract, as it is our understanding the VA currently provides these items to the Goshen CBOC. Yes, this is the contractor’s responsibility.
12. Paragraph 4.6.2.4.8 states, “Women Veterans must have women-only or unisex toilet rooms and bathing/shower facilities.” We have had previous experience where bathing/shower facilities have been deemed to be not desirable by the VA. Could the Government please confirm if these bathing/shower facilities are a requirement for the Goshen CBOC. Bathing/showering facilities are not required. An amendment to remove this is to follow.
13. Can the Government confirm that a vending machine area is required as listed on page 80? If so, who is responsible for providing the vending machines. The vending machine is not required. An amendment to update this is to follow.
14. Are there any set asides for this solicitation? we read couple in this solicitation for small business and also VOSB. No, this is not a set aside but Small Business and/or VOSB are more than welcome to submit a proposal.
15. Do you need VA contracting past performance to bid for this solicitation? No, you don’t need VA contracting past performance. If you have other past performance for different agencies of similar work (size and scope) please submit that information.
16. Do we have to have an existing clinic in Goshen? or Can we setup a new clinic? No, you do not have to have a pre-existing clinic, a new one can be proposed.
17. Will the new contractor inherit the patient’s last Primary Care vesting visit and be allowed to bill for one year from that date upon contract award? Yes, the new contractor will inherit the patient’s last “vesting visit” (also called qualifying encounter) and will be allowed to bill for one year from that date, upon award.
18. How many Mission Act referrals occurred for primary care in the past 12 months for enrolled veterans of the Goshen CBOC? The Mission Act has only been in place since June 2019. Since then 1,799 CIC (care in the community requests) have been issued.
19. In paragraph 4.6.3 it states that the Contractor will provide all medication, including any necessary vaccines that are to be administered to patients in the clinic. Will the Medications and Vaccines be provided to the clinic at the cost of the Contractor or the VA? Since the contractor will provide, it would be the cost of the contractor.
20. Is the “shall not exceed” number of $40,000,000 for the contract an absolute “no greater than” proposal value the VA will accept? Yes, that is the absolute “no greater than” amount. The contract total cannot exceed that value for the life of the contract.
21. If the cost of vaccines is the responsibility of the contractor, please confirm that the following list of Vaccines are the responsibility of the contractor? Please see below:

22. If the cost of vaccines is the responsibility of the contractor…. Are there any additional Vaccines required to be supplied by the contractor other than those listed in question 21 (above)? There could be additional requests for medications in the future that we are not aware of at this time.
23. If the cost of vaccines is the responsibility of the contractor…. What is the number of Vaccines, by type as listed in question 21 (above) and any additional Vaccines added by the government pursuant to question 22 (above), that was provided to the Goshen CBOC Enrolled Veterans in the last 12 months? Please see question #21
24. Pursuant to paragraph 4.6.3, If the cost of Medications is the responsibility of the contractor…, please provide a list of Medications the government expects the contractor to dispense (at the cost of the Contractor) from the clinic that would not be a routine medication dispensed through the VA or an urgent/emergent medication dispensed through the contracted local pharmacy. The attachments are the list of approved urgent/emergent medications (Palisade) and medications are on the VA National Formulary.
25. If the cost of Medications is the responsibility of the contractor…. What is the number of Medications, by type as provided pursuant to question 24 (above), that was provided to the Goshen CBOC Enrolled Veterans in the last 12 months? The current contractor is responsible for this and we do not have access to this information as the current vendor is providing this requirement.
26. In paragraph 4.6.4.1 it states the Contractor shall provide two (2) GE Healthcare Discovery XR656 Plus Base System. Is the VA asking for 2 (two) x-ray machines for the Goshen CBOC? If yes, please provide clarification on the VA’s strategy for the use of 2 machines to support a clinic with 3300 Veterans. The contractor shall provide one (1) GE Healthcare Discovery XR656 Plus Base System. An amendment to update this is to follow.
27. Is the current incumbent contractor under the current Goshen contract required to provide all medications, including any necessary vaccines that are administered to patients in the clinic. This is a new requirement for CBOCs, all medications and vaccines are to be provided by the contractor.
28. Does the current Goshen CBOC facility meet the new contract PACT Space Module Design Guide requirements as stated in paragraph 4.9 of the solicitation? No, the PACT model is a new requirement for CBOCs.
29. Will the Contractors proposed space be required to meet the on-stage/off-stage design with the PACT Zone? Yes.
30. Is the VA supplying the phones and phone system for the CBOC or is the contractor to provide? The VA will provide the following phones for VA use: mental health, social services, telehealth, and pharmacy services, clinic manager, and provider offices. The contractor shall supply all other phones to be used for primary care (clinical and administrative), lab, x-ray, and scheduling.
31. How many VA staffed personnel will be providing services at the CBOC? Please list by job title and number of days per month. Two (1) full time and one (1) part time Social Workers, one (1) full time MH provider, one (1) full time CPS, one (1) full time LPN.
32. How many offices will be needed at the Goshen CBOC for the VA provided staff (by position/function) identified in question 31 (above)?
SW and MH – 6 offices Telehealth – 2 dedicated rooms Pharmacy – 1 office These offices must be available 5 days a week during clinic hours.
33. How many veterans using the Goshen CBOC are currently on the Primary Care billable roster? 2,584
34. How many times per year does the VA anticipate that the Contractor staff will be required to travel to and attend VA mandated meetings on-site at the NIHCS in Ft. Wayne, IN or other VA Facility? Less than 3 times per year, of those times, travel may be required for consecutive days at a time, i.e. some new staff may require specific face to face training.
35. Reference E.2 Addendum to FAR 52.212-1 Instructions to Offerors - Commercial Items, Paragraph Heading 'Submission of Proposals' page 141 of 159, indicates a hard copy submission is required. Previous to that, on page 138 of 159, indicates a two-part submission (price and non-price).
Questions:
a. should each part be submitted in separate binders? Yes, they should be submitted separately
b. how many copies of each part are required? 1 copy of each
c. does the price part need to be sealed? No, but needs to be separate from the non-price portion
36. Reference E.2 Addendum to FAR 52.212-1 Instructions to Offerors - Commercial Items, Paragraph Heading Sub-Factor B: Staffing and Experience, page 139 of 159 - bullets ii and iii:
Questions:
a. is it acceptable to submit the CV and License for the physicians with the proposal and then submit the ACLS and BLS certificates after award?
b. is it acceptable to submit the CV and License for the NP and then submit the transcripts after award?
We’re going to revise Subfactor B to remove this. An amendment to update this is to follow.
37. Reference E.2 Addendum to FAR 52.212-1 Instructions to Offerors - Commercial Items, Paragraph Heading PART 2: Non-Price Response, Specific Instructions, Page 138-159, bullet b indicates this is to be a blind submission (no identifying information).
Question:
a. As Factor 1 (Technical Ability), Factor 2 (Past Performance) and Factor 3 (Veterans Preference) will presumably be included in the same binder, does the Government wish the Factor 2 and Factor 3 sections be redacted also or does the redaction apply only to Factor 1? Yes, please redact or remove any marks that would indicate your company’s name/logo.
38. Section B.2 – Schedule of Supplies/Services (6 of 159): Can the Government please explain why they anticipate a decrease in enrollment over the course of the 10-year contract period? Declining of the aging population. The largest group of veterans in the area are 65 or over.
39. Section B.3 Performance Work Statement – General: Is the VA or Contractor responsible for Telehealth? Can the VA please list out all of the VA provided positions? The VA will provide Telehealth services. The vendor is to provide space for telehealth services to be conducted.
40. How many veterans are currently on the billable roster? Please see question #33
41. What is the most recent three (3) months of invoiced enrollment at the facility? Invoices are based on users, not enrollees, the most recent 3 months are from Sep-Nov. 2613, 2633, 2584.
42. The VA projects a decline over the next 10 years, please explain why the base period is higher than current enrollment? As stated in question #38, declining of the aging population. The largest group of veterans in the area are 65 or over.
43. Since implementation, how many Mission Act referrals have occurred for enrolled veterans at the CBOC? As stated in question #18, The Mission Act has only been in place since June 2019. Since then 1,799 CIC (care in the community requests) have been issued.
44. Please specify how many of the Mission Act referrals were for primary care patients. Please specify the types of specialty services by number of patients. See question 18 for the number of referrals. The type of services to provide are all stated in the Performance Work Statement and would vary based on the patient’s need.
45. How many VA staffed personnel will be providing services at the CBOC? Please list job title and number of days per month. Please see question #31.
46. The RFP calls for 2.0 FTE Phlebotomist/lab staffing. Is this correct? If correct, please explain rationale for 2.0 FTE in clinic of this size. The rationale for 2.0 FTE of Lab personnel is to have enough coverage for vacation, sick leave, coverage while making specimens ready for transport to the FTW facility, coverage for lunches and 15 min breaks and coverage while performing POC testing (urine dipstick, Rapid HIV and Pregnancy testing). Please confirm Telederm staff will be provided by the VA. Yes, the VA will provide.
a. Please confirm Telederm staff will be provided by the VA. Yes, the VA will provide.
47. The RFP (Factor 1: Sub-Factor B) requires the Contractor to submit candidate certifications and transcripts with the proposal.
a. Can documentation requirements for the following positions be submitted at the time of hire?
b. For Physicians: ACLS and BLS Certifications
c. For Nurse Practitioners: Transcripts from an Accredited Nurse Practitioner Program. Requiring these documents with the proposal is unrealistic and provides a significant advantage to the incumbent. To collect these required documents would demand that we permanently hire all the required staff in less than 30 days. Asking candidates to provide such documentation before there is an available position is not a common practice in recruiting/staffing.
We’re going to revise Subfactor B to remove this. An amendment to update this is to follow.
48. Will the contractor or the VA be responsible for paying for POC equipment, validation and supplies? All required supplies are provided by the VA to perform required Quality Control, Validation, Patient Testing and Training.
49. 4.6.4.4 MANDATED POC TESTING INCLUDES: Urine Dipstic, iSTAT (Chem 8+ only), pregnancy test, accuchek, and HIV. Outpatient Site of Care shall have point of care or stat pregnancy testing at the same site of care. Please list the number of each type of test performed at the CBOC in the past 12 months. Urine Dipsticks- 40; Glucometer- 38; Pregnancy Test- 3; iSTAT panels- 28; Rapid HIV- 0
50. The RFP calls for the contractor to provide Screening and diagnostic mammograms is this correct? Diagnostic mammograms are typically covered by the VA. The contractor will be responsible for the screenings and the VA will be responsible for Diagnostic mammograms
51. The RFP calls for reading of EKG’s to be done by the Contractor, is this correct? Yes, the EKGs shall be confirmed and/or read by the licensed contracted provider.
52. Will Homeless programs be needed, if so, who will be responsible for staffing? Yes, the VA will provide these services
53. Clinic hours are listed as 0800-1700 Monday through Friday, later in the RFP it states phones will be answered 0800-1630 Monday through Friday, please confirm times are correct. The hours will be 0800-1700. An amendment to update this is to follow.
54. Please list all vaccines provided by the CBOC in the past 12 months. Please see question #21
55. Please list all medications provided by the CBOC in the past 12 months. Please see question #25
56. Will the VA or the Contractor be financially responsible for the cost of 10-day bridge medications? The contractor is responsible
57. What urgent/emergent medications will be needed for up to 10 days until the prescription can be processed and mailed by the NIHCS Pharmacy or CMOP (please list). Please see question #24
58. Thin-Prep PAP testing supplies are not available for purchase by Contractor, will the VA supply? Yes, the VA will supply.
59. Will the VA-provided staff require an office at the CBOC? If yes, what type and size? Please see question #32
60. Does the VA prefer sliding doors? If yes, on both sides or patient side only? Does the VA require sliding doors on the patient side and provider side of each exam room or will swing doors on the provider side be adequate? Swing doors are adequate
61. Section 4.9.16.1.2 states “Setback distance from the face of the Building's exterior to the protected/defended perimeter (i.e., any potential point of explosion). This means the distance from the Building to the curb or other boundary protected by bollards, planters or other street furniture. Such potential points of explosion may be, but are not limited to, such areas that could be accessible by any motorized vehicle (i.e., street, alley, sidewalk, driveway, parking lot).”
a. Question: What is the setback distance of the site to the bollards required for physical security? Typically, it is 25 feet, but most clinics in shopping center etc. … do not have spaces that conform to this requirement. 25 feet is the requirement.
62. Why does the RFP require so many exam rooms? The CLINs is only 3,281 enrollees in the Base Year and declines to 3,044 over the nine option years. However, the RFP space requirement of a total of 22 exam rooms suggests more PACT teams that the enrollment requires. For example, if there are three rooms per PACT team and 16 exam rooms as shown in the RFP, this would accommodate 5.1 PACT teams. Or four exam rooms per team would accommodate four PACT teams. Is this correct? After reviewing the current number of enrollees and the anticipated number of enrollees for the new CBOC’s delineated area, the VA anticipates that 4 PACT teams are more appropriate. An amendment to update this is to follow.
63. The RFP calls for four consult rooms and two women’s health rooms. Are these six rooms separate or can they be included in the PACT team exam room counts? These are included in the PACT room counts.
64. The RFP suggests Warsaw, IN as an alternative location for the CBOC. Does the VA see Warsaw as a potential growth area versus Goshen? If so, why? No, it’s not that the VA necessarily sees this as an area for growth but that having the CBOC in Warsaw (or the stated area of Goshen) will be closer for the veteran versus traveling to the FTW or Marion VA and may bring new patients to the clinic.
65. What advantages does the VA see to locating the clinic in Warsaw in terms of enrollment, accessibility, economic development, etc.? The location can either be in the Southeast area of Goshen/Elkhart County or the Northeast area of Warsaw/Kosciusko county. The rationale is that the current location is too close to the St. Joseph CBOC and we want to be sure that the new CBOC is more proportionate in distance from the St. Joe CBOC and the Ft. Wayne VAMC. When the current CBOC location was opened some patients located in Warsaw no longer sought treatment. We believe that locating the new CBOC in either the Southeast area of Goshen or the Northeast area of Warsaw could potentially bring these patients back.
66. What is the veteran population in the catchment area by zip code? The veteran population is 12,675.
67. The RFP states “the Contractor shall have ninety (90) days from contract award to commencement of the provision of medical care to local veterans. However, the Contractor shall have all start-up requirements in place and ready to commence operation NLT eighty-three (83) calendar days from contract award.”
a. Question: Will the VA permit one hundred and twenty days (120) from contract award to commencement of provision of medical care and all start-up requirements in place and ready to commence operations NLT one hundred and thirteen (113) day? Please see question #7
68. Will the government be doing site visits prior to award? If so, how much advance notice will the contractor be given? No, we will not be doing a site visit prior to award.
69. The table under section 4.9.1 on pages 76-80 lists a higher than expected number of required rooms (based on our projected numbers of PACT teamlets), including a total of 16 PACT exam rooms and 2 women’s health rooms, for example.
a. Could the VA please confirm the rooms that are required for the Goshen CBOC? The number/quantity of rooms are being modified to better reflect 4 PACT teams.
b. Specifically, could the Government indicate how many exam rooms and how many consultation rooms are required per PACT Teamlet as well as how many are required total for the clinic? An amendment to update this is to follow.
70. We are requesting clarification on the exact panel sizes that will be allowed for primary care providers at the Goshen CBOC. We present the full details of our questions below as it is crucial for all offerors to have the same understanding of allowable panel sizes, as these panel size quantities drive both staffing and facility space planning.
0. Paragraph 2.1 of the Solicitation states “Current standards are 1200 active patients per full time physician and 900 active patients per full time midlevel provider. Actual panel sizes can be determined by the VA in accordance with VHA Directive 1406 Patient Centered Management Module (PCMM) for Primary Care.” We note that our experience has indicated that the VA-assigned panel sizes at VA NIHCS clinic locations are significantly lower than 1200 active patients per full time physician and 900 active patients per full time midlevel provider. Additionally, we note that VHA Directive 1406, Appendix E, provides various factors that affect panel sizes, including teamlet support staff ratio, number of exam rooms per provider, primary care intensity score, and women veteran population. Regarding support staff ratio, the Solicitation requirements of three support staff members per provider would equate to no adjustment to panel sizes, per Table 1 in Appendix E. Regarding exam rooms, the provision of three exam rooms per provider would equate to a +5% adjustment to panel sizes, per Table 2 in Appendix E. Regarding primary care intensity score, we are not sure what, if any, adjustment from Table 3 in Appendix E would apply, as the Solicitation does not indicate the primary care intensity scores for the clinics. Regarding women veteran population, assuming an even distribution of women veterans among the providers at a single location, we are not sure what, if any, adjustment from Table 4 in Appendix E would apply, as the Solicitation does not provide the number of women veterans for the clinics. This is explained in question #62. If you question is in regard to the number of women veterans that are currently enrolled at the current CBOC, that number is 299 as of Fiscal Year 19.
In this context of the adjustments indicated by Appendix E and the assumptions that solicitation requirements for support staff and exam rooms will be followed and that women veterans will be divided evenly among all providers as a single location, we ask the following questions:
0. Can the Government indicate the panel size that will be allowed for a full-time (non-medical director) physician at the Goshen CBOC? 1200 is correct
0. Can the Government indicate the panel size that will be allowed for a full-time mid-level provider at the Goshen CBOC? 900 is correct
71. Is it possible to extend the solicitation due date? Yes, an amendment has been issued to extend the deadline is to follow.
72. RFP-Page 140, Factor 3, Item iii: “Past Performance in meeting VA’s department-wide SDVOSB subcontracting goals for 2014 and 2015.” The VA requires the contractor to “submit information evidencing the offeror’s past performance” in meeting these goals. Please clarify what the 2014 or 2015 goals were. Or please provide us with a copy of the goals or direct us to a resource with this information. We’re going to revise Factor 3- Veteran’s Involvement, to remove subparagraph iii. An amendment to update this is to follow.
73. We have noticed an ambiguity with respect to the requirement for performance location for RFP 36C25019R0093. Based upon what appears to be the current ambiguity in Solicitation language, we are not able to determine if this is requirement is for the south east area of Goshen or for the southeast area of Elkhart County, or, similarly, for the northeast area of Warsaw or the northeast area of Kosciusko County.
The applicable Solicitation provisions leading to the current ambiguity are as follows:
Paragraph 1.1 states, “Contractor shall provide community-based outpatient clinic (CBOC) services in the southeast area of Goshen, Indiana (Elkhart County) or in the northeast area of Warsaw, Indiana (Kosciusko County)”.
Paragraph 1.2 states “The contractor shall establish one location in the Goshen, Indiana area (Elkhart or Kosciusko Counties),” which seems to allow a performance location in any part of Elkhart County or Kosciusko County.
Our current belief of the most logical interpretation of the Solicitation language in the context of the above identified ambiguities is as follows: The VA intends that the clinic can be located in Elkhart County anywhere southeast of the central point of Goshen or in Kosciusko County anywhere northeast of the central point of Warsaw (including the Lake Wawasee area).
Please confirm if this is the correct interpretation. If it is the correct interpretation, can the VA provide further clarification of:
a. What constitutes the boundary within Goshen/Elkhart County that the clinic must be southeast of?
There are no boundaries per se. The VA just requires the location to be in the delineated areas stated so that the new CBOC is not so close to the St. Joseph CBOC and not too close to the Ft. Wayne VAMC.
b. What constitutes the boundary within Warsaw/Kosciusko County that the clinic must be northeast of? Same as stated in the above (73a).
74. please note the number of copies of the Technical Proposal and Price Proposal that the VA requires contractors to provide in their response to the RFP. One binder, two, three, etc. Please provide one binder for the price factor and 3 binders for the non-price factors portion.

VHA NATIONAL FORMULARY
Dec-19
The VANF is dosage form specific. If a specific dosage form is not listed, the specific product in question should be considered non-formulary (although other dosage forms of that molecular entity may be included as formulary). Dosage forms such as TAB and CAP, ORAL are considered immediate release. Sustained release products are listed individually. Formulary products may also be available in many dosage forms and strengths. In the case of products with multiple dosage forms and strengths, not all forms and strengths need to be stocked. Facilities should make those dosages available that are necessary to meet their patient needs.”
PA- There are three levels of Prior Authorization
• Prior Authorization-National (PA-N) refers to medications that are formulary, but require prior approval at the national level before dispensing.
• Prior Authorization-VISN (PA-V) refers to medications that are formulary, but require prior approval at the VISN level before dispensing.
• Prior Authorization-Facility (PA-F) refers to medications that are formulary, but require prior approval at the facility level before dispensing.
Prior Authorization is used to insure that the medication is appropriate for each individual Veteran.
R The national restriction for antibiotics is that all decisions regarding which agents to carry in these classes will be made at the local or VISN level. These decisions should be based on local culture and sensitivity patterns. The restriction for all other products is as otherwise noted.
