About this file

This document is an amendment to a Request for Lease Proposals (RLP) for a new Community Based Outpatient Clinic (CBOC) to be located in Norman, OK, issued by the Department of Veterans Affairs (VA) under solicitation number 36C24W24R0018.

The amendment provides clarification on several questions regarding the RLP, including pricing terms for tenant improvements, the submission of past performance questionnaires, proposal formatting requirements, sustainability and environmental standards, on-site management, and janitorial services. Key details include: the VA will provide a Tenant Improvement Cost worksheet after award for negotiating shell and TI costs; previous PPQs are acceptable but the VA prefers direct submission from references; there are no specific PDF formatting requirements; there are no additional sustainability requirements beyond those in the RLP; an on-site manager is not required but local site management is needed; and the lessor is responsible for ensuring sufficient staff to maintain the clinic during operating hours, but will not be reimbursed for pest control treatments. The amendment also states the GSA Form 527 may be submitted separately with the proposal.

AMENDMENT NO. 5

Department of Veterans Affairs (DVA)

Norman, OK – Community Based Outpatient Clinic (CBOC) VA Request for Lease Proposals

No. 36C24W24R0018

This amendment is to provide answers of clarification to the following questions concerning RLP No. 36C24W24R0018:

Question 1: RLP section 3.03 PRICING TERMS says to provide "D. TI Unit Price List", however other areas in the RLP say this is for the successful offeror. Should any TI pricing be included at this time? If so, will a form be provided?

Answer 1: We ask for your Tenant Improvements (TI) estimate on the GSA Form 1364.

To be completed after award and during design, we provide a Tenant Improvement Cost (TIC) worksheet to be filled out for mutual negotiated Shell & TI costs.

Question 2: Would previous PPQs be acceptable to submit or are new PPQs required?

If the 11-Exhibit K PPQ's are required, do these need to be submitted directly to the VA (Mr. Wagstaff & Ms. Kloeppel?) from our references or should they be included in our response?

Answer 2: Preferred submission would be directly from references, directly to MIRANDA.KLOEPPEL@VA.GOV or CRAIG.WAGSTAFF@VA.GOV but we will accept the submission as part of the response to the RLP.

Question 3: Does the VA require the response to be submitted in a certain PDF arrangement? (Example – as one PDF, or Pricing submitted separately, or all forms submitted as individual documents, or multiple PDF's as needed under a certain MB size, etc.)

Answer 3: Other than RLP Section 3 - How to Offer, there are no requirements as to how the Proposal is to be arranged or number of attachments that should be submitted.

Question 4: Is there a sustainability firm requirement for this RLP response? (LEED, Green Globes, etc.)

Answer 4: There are no additional sustainability or environmental requirements besides those outlined in the RLP.

Question 5: Is an on-site building manager required? (Exhibit A, 6.14 Onsite Lessor

Management) Answer 5: An on-site manager is not required. However, local site management is required to assist with on-site concerns and ensure there are no delays or coordination issues to be resolved.

Question 6: As janitorial tasks are minimum twice a day, can we assume a day porter type position is required? If so, what would be the minimum hours required?

(Exhibit B, 9.0 Janitorial Services)

Answer 6: The Lessor shall ensure sufficient employees are available to prepare and maintain the clinic to see patients from 7:00 am to 4:30pm, to be available to clean up spills, keep the restrooms, clinical and public spaces clean and keep areas stocked with sufficient paper products, sanitizer, and soap.

Question 7: As it would be outside the Lessors control and impossible to estimate the size and frequency of non-routine pest infestations that the clinic will experience, will the Lessor be reimbursed for each treatment of other non-routine rodents and pests? (Exhibit B, 8.1, 6)

Answer 7: Lessor will not be reimbursed for any pest infestation treatments.

Question 8: Is it possible to submit the GSA Form 527 (10/15), “Contractor’s

Qualifications and Financial Information” as a separate document (at the same time as the rest of the proposal)?

Answer 8: Yes

All other requirements remain unchanged.
