About this file

This document is an amendment to Request for Lease Proposals (RLP) No. 36C24W24R0018 issued by the Department of Veterans Affairs (VA) for a Community Based Outpatient Clinic (CBOC) in Norman, OK. The amendment provides clarification on the requirement in Section 4.07(C)(c) for the Offeror to provide completed GSA Form 527, "Contractor's Qualifications and Financial Information" for the Offeror, General Contractor, Mechanical Contractor, and Electrical Contractor. The VA will not eliminate this requirement, as it is one of the evaluation factors to verify the contractors are not debarred from government service. For General Contractors, since the State of Oklahoma does not issue licenses, the VA is requesting copies of the trade licenses and/or registrations of the proposed contractors that may be performing work requiring a license and/or registration in Oklahoma.

AMENDMENT NO. 4

Department of Veterans Affairs (DVA)

Norman, OK – Community Based Outpatient Clinic (CBOC) VA Request for Lease Proposals

No. 36C24W24R0018

This amendment is to provide answers of clarification to questions concerning RLP No.

36C24W24R0018. These questions specifically reference Section 4.07(C)(c) which states:

a. Construction Team Qualifications: The Offeror shall provide a completed GSA Form 527 (10/15), "Contractor’s Qualifications and Financial Information" for the Offeror, the General Contractor, Mechanical Contractor, and Electrical Contractor;

Section V needs to be completed for the Offeror but not any of the aforementioned parties. In Section VII of Form 527, the Offeror must provide a description of outstanding commitments, names, and qualifications of key personnel, and any other information related to experience, competency, and performance capabilities with construction projects similar in scope to a 25,000 ABOA SF medical clinical project. The Offeror shall provide a copy of the license in the State of Oklahoma where the facility is located for the individual(s) and/or firm(s) proposed as contractors. If the Lessor is also the contractor, information provided in response to paragraphs Past Performance and Financial Resources above need not be duplicated. Installers shall have OEM credentials for the Special Systems listed in Exhibit B: Agency Special Requirements of this RLP.

Question 1: Will the VA consider eliminating this requirement until a later time?

Answer 1: No, this requirement is one of our evaluation factors and can not be eliminated.

The purpose of this form is to get a good understanding of the subcontractors that we have the potential of working with and to verify they are not debarred from government service. Providing this information does not preclude you from changing your contractors as needed.

Question 2: For General Contractors, The State of Oklahoma does not issue licenses nor is there any vehicle for the licensing of General Contractors. Could you take a moment and advise us on what would be the response you are requesting?

Answer 2: The VA is requesting copies of the trade licenses and/or registrations of proposed contractors that may be performing work requiring a license and/or registration by the State of Oklahoma.

All other requirements remain unchanged.
