About this file

This is a Questions and Answers document for RFQ 36C24926Q0140 addressing vendor inquiries regarding Board Certified Anesthesiology Physician Services at the James H. Quillen VA Medical Center in Mountain Home, Tennessee. The document clarifies that the requirement is for 4,160 hours annually (2 full-time equivalents), with anesthesiologists working 10-hour shifts Monday through Friday from 7:00 AM to 5:30 PM in a department currently operating with 3 full-time anesthesiologists and 12 CRNAs (with a fully staffed target of 6 anesthesiologists and 12 CRNAs). The facility has 7 operating rooms plus GI/Endoscopy procedure coverage and performs approximately 537.4 cases monthly, including vascular procedures (carotid endarterectomy, transcarotid artery revascularization, arteriovenous fistula creation, endovascular abdominal aortic aneurysm repair), thoracic procedures (thoracotomy, lobectomy, video-assisted thoracoscopy surgery), and cardiac procedures (TEE, cardioversions, EP ablation).

Key requirement clarifications include that anesthesiologists must be American Board of Anesthesiology board-certified (not board-eligible), proficient in ultrasound-guided regional anesthesia, and willing to supervise CRNAs at a 1:4 ratio without required presence during induction or emergence. The contract allows billing of overtime hours exceeding 40 hours per week at the Call-In Activation Rate (CLIN 0002), and on-call services will be shared with current full-time anesthesiologists rather than solely contractor-provided. Vacation requests require a minimum 45-day notice period. The government confirmed removal of hospital facility accreditation verification requirements and clarified that applicable attachments include Quality Assurance Surveillance Plan, Contractor Certification (Immigration and Nationality Act of 1952), Conflict of Interest Certification Statement, Past Performance Survey Questionnaire, and Onboarding Flowchart.
