INSTRUCTIONS, CONDITIONS AND OTHER STATEMENTS TO BIDDERS/OFFERORS

2.1 PROPOSAL PREPARATION

Instructions, Conditions, and Notices to Offerors

1. OVERVIEW

1.1 This request for proposal (RFP) seeks proposals from Offerors interested in being selected to renovate a series of restrooms on the Bay Pines VAMC Campus. Offerors responding to this RFP are required to submit their proposals in accordance with the instructions contained in this section.

Bay Pines VAMC has a requirement for the renovation of a series of restrooms in buildings 1, 12,13, 19, 20,24,100, 102. This work involves a complete renovation of areas established since the original construction of these buildings. This renovation will modernize the restroom infrastructure and provide a more pleasant experience for all 100,000 patients serviced by PBVAMC.

1.2 The purpose of these instructions is to establish uniform evaluation procedures for the proposals and the development of the Best Value Decision by the Source Selection Authority using the Trade-Off Process (See Federal Acquisition Regulation 15.101-1). In as much as the proposal shall describe the capability of the Offeror to perform the resulting contract, the proposal shall be specific and complete in every detail as well as be prepared simply and economically, providing a straightforward and concise description of capabilities to satisfactorily perform the contract.

The Government reserves the right to evaluate offers and award a contract without discussions with offerors, however, the offeror’s initial proposal should contain the offeror’s best terms from a price and technical standpoint. The Government reserves the right to conduct discussions if the Contracting Officer later determines them to be necessary. If the Contracting Officer determines that the number of proposals that would otherwise be in the competitive range exceeds the number at which an efficient competition can be conducted, the Contracting Officer may limit the number of proposals in the competitive range to the greatest number that will permit an efficient competition among the most highly rated proposals.

1.3 For this solicitation, the Government will award a single firm-fixed price contract.

2. PROPOSAL PREPARATION INSTRUCTIONS

2.1 Proposals must be complete, self-sufficient, and respond directly to the requirements of the solicitation. Any significant inconsistency, if unexplained, raises a fundamental issue of the offeror’s understanding and/or ability to perform the contract. This may be a basis for rejection of the proposal.

3. SUBMISSION INSTRUCTIONS

3.1 One electronic copy of all Volumes of the proposal (General Information, Technical, Past Performance and Price) shall be submitted to the Contract Specialist Iris.thomas1@va.gov on or before the due date.

3.2 Faxed proposals are not permitted under any circumstances.

The Past Performance Questionnaire included in the solicitation is provided for the offeror or its team members to submit to the client for each project the offeror includes in its proposal for Factor 2, Past Performance. Ensure correct phone numbers and email addresses are provided for the client point of contact. Completed Past Performance Questionnaires should be submitted with your proposal. If the offeror is unable to obtain a completed PPQ from a client for a project(s) before proposal closing date, the offeror should complete and submit with the proposal the first page of the PPQ (Attachment B), which will provide contract and client information for the respective project(s).

Offerors should follow-up with clients/references to ensure timely submittal of questionnaires. If the client requests, questionnaires may be submitted directly to the Government’s point of contact, Iris Thomas via email at Iris.thomas1@va.gov prior to proposal closing date. Offerors shall not incorporate by reference into their proposal PPQs previously submitted for other RFPs. However, this does not preclude the Government from utilizing previously submitted PPQ information in the past performance evaluation. Also include performance recognition documents received within the last seven years such as awards, award fee determinations, customer letters of commendation, and any other forms of performance recognition.

In addition to the above, the Government may review any other sources of information for evaluating past performance. Price proposals shall be submitted on SF 1442, completed, and signed by authorized individual(s) of the Offeror. Offers submitted in the name of a Joint Venture must be signed in accordance with the terms and conditions specified in the joint venture agreement as evidenced in the proposal. The price proposal shall encompass all costs associated with the requirements of the solicitation. The total proposed price for CLIN 0001 shall be rounded to the nearest dollar. Price will not be rated or scored but will be evaluated for fairness and reasonableness through the use of price analysis. In accordance with FAR 15.404-1(b)(2) the government may use various price analysis techniques and procedures to ensure a fair and reasonable price.

Scope of Work 516-19-104 Renovate Station Restrooms

Goals and Needs:

This project includes the renovation of a series of restrooms on the Bay Pines VAMC campus per the provided design documents and specifications.

Site Conditions:

The buildings where the included restrooms are located will be occupied during performance of the work. Construction work must be coordinated not to interfere with patient activities occurring adjacent, above and below the project areas.

In areas where the exterior wall of the rooms are to be renovated, a one-hour fire barrier from deck to deck is required around the project area and is to be constructed with drywall on both sides of metal studs. Doors in the fire barrier will be rated ¾ hour doors and the construction area will be negative to the adjacent space. All specifications and regulations of the VA Safety & Emergency Management Service are to be adhered to at all times on site.

Infection control procedures will follow the VA Infection Control Risk Assessment Matrix of Precautions for Construction and Renovation, including but not limited to the following:

1. Remove or Isolate HVAC system in area where work is being done to prevent contamination of duct system.

2. Complete all critical barriers i.e. sheetrock, plywood, plastic, to seal area from non-work area or implement control cube method (cart with plastic covering and sealed connection to work site with HEPA vacuum for vacuuming prior to exit) before construction begins.

3. Maintain negative air pressure within work site utilizing HEPA equipped air filtration units.

4. Contain construction waste before transport in tightly covered containers.

5. Cover transport receptacles or carts. Tape covering unless solid lid.

6. Do not remove barriers from work area until completed project is inspected by the owner’s Safety Department and Infection Control Department and thoroughly cleaned by the owner’s Environmental Services Department.

7. Remove barrier materials carefully to minimize spreading of dirt and debris associated with construction.

8. Vacuum work area with HEPA filtered vacuums.

9. Wet mop area with disinfectant.

10. Remove isolation of HVAC system in areas where work is being performed.

11. Additional containment procedures, including the construction of an anteroom and additional PPE and HEPA vacuuming will be required during ACM abatement processes.

12. Consider all ceramic tiling to be lead-containing and follow all required procedures for removal and disposal.

Scope of Demolition:

Demolition is to be performed in accordance with the design drawings and specifications. Abatement will occur at for any materials determined to contain hazardous materials/ACM. Any materials found to be impacted by mold or moisture damage within the project area are to be removed.

The Industrial hygiene consultant (GLE) performed the ACM and lead survey for the project scope areas. Below is a summary of the results by building:

Bldg. 100 IF 107 & IF 108 – No ACM 146 & 139 – No ACM 141, 142, 144, 145 – Floor Tile & Black Mastic positive for asbestos Bldg. 102 134, 135, 110, 111 – No ACM Bldg. 24 109 & 124 – No ACM Bldg. 20 108 & 109 – No ACM Bldg. 1 A222 & A223 – No ACM Bldg. 12 11A, 7, 8 – No ACM Bldg. 19 7 – Ceiling Texture – 5% Chrysotile asbestos, and Joint Compound – 5% Chrysotile asbestos (Drywall Composite = <1% ACM) Bldg. 13 Not surveyed

Construction Schedule:

All work will be completed within 210 days from Notice to Proceed.

Contractor shall be required to work with the VA COR to develop a schedule of phasing dates on which the contractor will accomplish work in each specific area of site, building or portion thereof, in order to coordinate phasing. Only one restroom in each building will be able to be closed at any given time, but it may be possible in some cases to have some restrooms closed concurrently provided that they are in different buildings, so phasing and scheduling will be vital to this project. A three-week lookahead schedule is to be maintained and updated at least weekly with the upcoming work plans for the project. A minimum of 3 weeks’ notice is required before starting work in each location to ensure that proper notices can be sent out regarding the closures.

Utility shutdowns are to be requested via email a minimum of 3 weeks prior to the planned work and requests are to include a detailed work plan, mitigation and contingency options, and lock out / tag out (LOTO) plans.

2.2 52.216-1 TYPE OF CONTRACT (APR 1984)
