About this file

This is a Medical Center Policy (MCP) document establishing standards for communicating critical test results at Charles George VA Medical Center in Asheville, North Carolina, effective January 2022 and scheduled for recertification in January 2027.

The policy defines critical results as test outcomes requiring timely, specific treatment where delay may result in significant morbidity, poorer outcomes, or death, and classifies them as either Critical Life Threatening (requiring immediate treatment within minutes to one hour) or Urgent Not Immediately Life Threatening (requiring treatment initiation within hours to days). Critical tests are defined as those for which timely results communication is standard for both normal and abnormal results. The policy mandates synchronous communication (in-person, telephone, instant message, or clinical video telehealth) between reporting and receiving staff within specified timeframes, with recipients responsible for documenting actions in the Computerized Patient Record System (CPRS). The Chief of Staff must ensure annual Medical Executive Council review of critical results lists, establish communication standards and compliance measures, and implement performance improvement processes. Clinical Service Chiefs must maintain current contact information in the "Asheville On Call" electronic folder and ensure provider compliance with communication standards. Ordering providers must accept critical results, initiate appropriate clinical action, and comply with "Write Down/Read Back" requirements, while reporting clinical staff must communicate results within established timeframes or document in CPRS that results were recognized and/or treated.

The policy includes detailed attachment tables specifying benchmark turnaround times (TAT) by diagnostic service, including Pathology and Laboratory Medicine Service (20-minute TAT for life-threatening chemistry, hematology, and arterial blood gas results), Cardiology (10-15 minute TAT for EKG and echocardiogram critical findings), Imaging Service (60-minute TAT for critical life-threatening results such as tension pneumothorax, pulmonary embolism, and intracranial hemorrhage, and 2-7 day TAT for urgent non-life-threatening results), Microbiology (immediate notification for positive blood cultures; 24-hour notification for resistant organisms), and Transfusion Medicine (24-hour notification for positive antibody screens and transfusion reactions).

(2) Document in CPRS the date, time, method of communication, and name of the provider/surrogate contacted.

f. Diagnostic Service Responsibilities:

(1) Establish a list of critical results for the diagnostic services provided.

(Attachment 1)

(2) Establish turnaround time (TAT) for communication of critical results.

4. DEFINITIONS

a. Definition 1. Critical Result: A test result that requires timely and specific treatment, where delay or lapse in care may result in significant morbidity, poorer outcome or death. The critical nature is an index of clinical impact, regardless of ordering priority. The time sensitivity is determined by the clinical course of an unaddressed result. Critical results are subclassified as Critical Life Threatening or Urgent Not Immediately Life Threatening:

(1) Critical Life Threatening: A test result that requires immediate treatment or action in a time frame of minutes to an hour.

(2) Urgent Not Immediately Life Threatening: A test result that has a significant impact on clinical outcome that requires initiation of treatment in hours to days to be considered effective care.

b. Definition 2. Critical Test: A test for which timely results communication is the standard for both normal and abnormal results.

c. Definition 3. Chain of Responsibility: A contingency plan for results notification that identifies a cascade of alternate contacts from the ordering provider to designated surrogates to receive critical results and initiate appropriate care when the ordering provider is not available.

d. Definition 4. Synchronous Communication: Real-time communication such as, in person, telephone conversation, IM or Clinical Video Telehealth (CVT).

Synchronous communication denotes immediate, bidirectional communication that confirms delivery and receipt of results.

e. Definition 5. Turnaround Time (TAT): The time from results known to the time communicated to provider/surrogate.
