INFORMATION RECEIVED IN RESPONSE TO THE SOLICITATION.

CONTINUATION SHEET – AMENDMENT 0003

1) The purpose of Amendment 0003 is to:

a) Revise the language of Section M.1 Evaluation Criteria, paragraph 7.3.1, Technical Subfactor 2 – Corporate Experience, as follows:

“7.3. Sub-Factor 2 – Corporate Experience

7.3.1. CRITERIA: For the purpose of this solicitation, Corporate Experience means project experience of the prime offeror entity, completed by the prime offeror entity as named in Block 14. of the SF1442, or under its previous entity names, if changed. Offeror must provide a minimum of two (2) and a maximum of five (5) examples of projects completed within the last five (5) years, with work completed not earlier than January 31, 2015, that are similar (“relevant”) in size and scope to the requirement being solicited. Experience of subcontractors cannot be used to meet this requirement. A project is considered similar in size and scope (“relevant”) if it meets the following minimum standard: Projects primarily involving (1.) the same construction trades required for the solicited work, and (2.) major renovations to, and/or construction of, medical facility laboratories, with a (3.) total contract value exceeding $1,000,000. Projects specifically for construction of new medical laboratories within an existing facility space, which also meet the above criteria, may be more highly rated.”

2) Terms and conditions not changed above remain unchanged by this Amendment.
