This document is an Asbestos Containing Materials (ACM) Inspection Summary Report for the Perry Point VA Medical Center prepared by Aerosol Monitoring & Analysis, Inc. on June 28, 2016. The report details an comprehensive asbestos survey conducted between December 14, 2015 and February 19, 2016, covering 45 buildings and 79 houses across the Perry Point campus, with a total area of 1,233,000 square feet. The inspection involved compiling existing asbestos survey reports, collecting bulk samples, identifying and assessing asbestos-containing materials, and recommending response actions in accordance with AHERA regulations.

Key findings include widespread asbestos-containing materials across various buildings and houses, such as floor tiles, mastics, pipe insulation, window glazing, and roofing materials. The report specifically highlights damaged materials in multiple locations, including mudded pipe fitting insulations, floor tiles, and window glazings. The inspection team, which included six certified inspectors, used polarized light microscopy (PLM) to analyze samples and evaluated materials following Asbestos Hazard Emergency Response Act (AHERA) guidelines. The report recommends removing or repairing damaged asbestos-containing materials and notes that some houses had significant vermiculite debris and damaged boiler breechings.

General Summary

AMA collected bulk samples of suspect materials throughout each building of the Perry Point VAMC Campus.

EPA Inspector(s) – Andrew Washington (I/MP 15013511. Exp. 7/10/2016) Robert Schoennagel (I/MP 15002091, exp. 2/28/2017) Patrick Dincher (I/MP 15020151, exp. 8/12/2016) Patrick LaShier (I 15024311, exp. 10/30/16) Ron Stallard (I/MP 15021561, exp. 9/28/16) Caleb Jones (I 16001771, exp. 1/20/2017)

AMA performed the bulk sampling for ACMs as stipulated in the AHERA regulation, 40 CFR 763 Subpart E. Samples were collected with a core bore, metal spatula, or χ-acto knife which was driven through the suspect material to the substrate so as to obtain a sample containing each discrete layer. The samples were then placed in sterilized “whirl-pak” bags and assigned unique identifiers, which were recorded on the bags and the bulk sampling survey sheets. A chain of custody form was filled out for the bulk samples. The samples were logged in and assigned unique lab numbers. Upon completion of analytical services, AMA Analytical Services, Inc. retained the remaining sample materials.

Samples of bulk material were analyzed using PLM following the EPA, “Method for the Determination of Asbestos in Bulk Building Materials" (EPA/600/R-93- 116). PLM is an optical microscopic technique used to distinguish the different types of asbestos fibers by their shape and unique optical properties. The technique is based on the refraction of light from the various crystalline asbestos structures and observing the corresponding color changes through the microscope.

AMA Analytical Services, Inc. is accredited by the U.S. Department of Commerce, National Institute of Standards and Technology (NIST) through the National Voluntary Laboratory Accreditation Program (NVLAP) for Bulk Asbestos Analysis, (NVLAP No. 101143).

ACMs were evaluated following the guidelines established by the Asbestos Hazard Emergency Response Act (AHERA).

General

These materials and quantities damaged are noted in the excel spreadsheet as indicated by the scope of work. A condensed table indicating the damaged ACM follows this report.

The construction dates of buildings are approximate and are based on information obtained from Veterans Administration personnel.

Several houses (1091, 1092, 1120, 1124, 1137, and 1151) were found to be demolished at the time of the inspection.

Roofing was not sampled and is therefore assumed asbestos containing.

Drywall associated with asbestos containing joint compound is considered contaminated as it cannot be separated from the joint compound.

Pipe and fitting insulation was assumed present and asbestos containing within wet walls throughout most buildings.
