About this file

The document is a Past Performance Questionnaire for a lease opportunity for a Veterans Center in Sussex County, Delaware, issued by the NCO4 Network Contracting Office in Pittsburgh, Pennsylvania. The five-page questionnaire is designed to collect detailed performance feedback from previous assessors about a contractor's past performance across seven key evaluation areas: quality of service, schedule/timeliness, customer satisfaction, management/personnel, cost/financial management, safety/security, and general performance.

The questionnaire uses a rating scale from Exceptional (E) to Unsatisfactory (U), with sections for both the contractor to provide basic contract information and for assessors to provide comprehensive evaluations. The ratings cover aspects such as building design, construction quality, timeliness of completion, management effectiveness, cost control, safety compliance, and overall customer satisfaction. The document is part of a solicitation (RLP NO. 36C24425R0051) for leased space for a Veteran's Center, with the primary goal of gathering historical performance data to help evaluate potential contractors for the lease opportunity.
