CONTINUATION PAGE

I. Offerors have submitted 5 follow-up questions to the Government in response to Solicitation Amendment 36C24423R0132 0003. The Government has included those questions submitted by Offerors below, verbatim, and provided responses. Offeror questions (Q#:) with numbering from Amendment 0003 and initial Government responses appear in regular font; Government responses to follow-up questions (A#:) follow in bold font.

Initial Government Response Q.23: Contractor is responsible for the generator. VA is unaware of the cost for maintenance.

What is the VA expecting to be on generator power? The entire facility, a portion of the facility

ie. LAB, medication supply?

A.23: At a minimum, the Generator should power the medication room, IT closet, lighting for 2 to 3 exam rooms, Laboratory and emergency outlets in the reception area. This is not an inclusive list and can be expanded to other areas.

Q.24: Will the VA provide a large copier, scanner, and fax machine?

Initial Government Response.24: No, the Contractor is responsible for Copier, Scanner, and Fax machine, which must be compatible with VA’s systems.

With the VA OIT policy of not allowing non-VA owned equipment how can the contractor use a scanner on a separate non-VA network?

A.24: Approval for the equipment to be connected to the VA network can be obtained through the ISSO and IT departments.

Q.18: Are Contractors responsible for any procedures? If so, please provide a list of expected onsite procedures.

Initial Government Response with combined contractor follow-up:18: These procedures are required for providers’ credentialing: Skin procedures such as cryotherapy/superficial excision or destruction – what is the estimated number of skin procedures that will be done monthly at the CBOC? Women’s health Pap smears and pelvic exams, placement of foley catheters- will catheters be ordered for the veteran in CPRS? ear flush, laboratory testing, vaccinations, basic wound care, joint injections, incision and drainage (I&D), Burn Care – will burn dressings be ordered through CPRS for individual patients?, local anesthetic techniques, management of uncomplicated closed fractures or dislocations, nose packing, peripheral nerve block, management of foreign object non penetration corneal, management of nasal FB, repair simple lacerations, Battle Field acupuncture – who will be responsible for the Battle Filed acupuncture training and supplies? If VAMC personnel are to do training, will that be done on site at the CBOC or will it require travel to the VAMC. How many days will training take if done by VAMC? This is the minimum required for credentialing; however, this is not a comprehensive list of procedures the contractor may be responsible for.

A.18: 1. VA is unable to determine the number of procedures performed. 2. Yes, veterans would have orders for the foley catheters (placement or replacement). 3. Unsure of the question for Burn Care and dressings. 4. The contractor would be responsible for supplies for Battlefield acupuncture. Training is conducted at Lebanon VA Medical Center and varies depending on the prior training of the credentialled individual.

Q.43: Who is responsible for courier services – VA or contractor?

36C24423R0132 0004

Initial Government Response.43: VA is responsible for courier services.

We would like to confirm that the VA (not the contractor) will be providing twice daily courier services to the VA Core Laboratory? Below is what is in the solicitation.

4.6.5.3. LABORATORY SERVICES: Contractor is responsible for: 1) Entering orders for laboratory tests into the VA EHR. Information concerning the laboratory tests are provided as an attachment in Section D.2) Sending specimens to the VA Core Laboratory twice daily, prior to the Contractor’s mid-day break period and after the close of business of the workday, except for those specified in this PWS

A.43: Contractor is responsible for gathering all necessary specimens for the Lebanon VAMC Courier to pick up twice daily. The Contractor is not responsible for transporting the specimens.

Q.31: Regarding paragraph 4.9.1, “consultation rooms for MH/SW 140-160SF each” – how many consultation rooms are required?

Initial Government Response.31: 7 consultation rooms.

For the 7 consultation rooms that are required, how does the VA prefer they are furnished? As offices or PACT design guide consultation rooms? Does the VA require handwashing sinks in all consultation rooms?

A.31: PACT design guide consultation rooms is the preference for the VA. Handwashing sinks are preferred but not required.

The Government has provided responses to all follow-up questions submitted in response to Solicitation Amendment 36C24423R0132 0003.
