The Contracting Officer will also utilize all available records (e.g. PPIPRS and FAPIIS) as well. (see “Evaluation” information below).

EVALUATION

Basis of Award This is an SDVOSB set-aside, to be eligible for award, an Offeror shall be SDVOSB CVE certified via www.vip.vetbiz.gov. The Government intends to award contract to the lowest price technically acceptable (LPTA) contractor that represents the best value. Trade-offs between price and non-price criteria will not be permitted. The following criteria will be evaluated on complete and responsive offers:

Technical Capability (acceptable/not acceptable) To be considered technically acceptable, offeror shall submit a capability statement demonstrating their technical capability and ability to meet the full scope of the statement of work (all elements) and shall include evidence of compliance with VAAR clause 852.219-10 (VA Notice of total service-disabled veteran-owned small business set-aside). Additional information that shall also be included in offer as part of what will be used in determining “technical capability” are the following (each item being clearly labeled):

· Geographic Coverage -- Offeror must present documentation that they can accommodate a geographic office location within Eastern Time Zone to align work hours with the Philadelphia VAMC staff.

· Identify methods for Bereaved Family Survey delivery, data collection, data entry and data delivery, while showing a clear understanding of the requirement.

· Identifies key personnel who will provide the requested service, in specific detail that includes work experience, qualifications, and years in the industry.

· Show ability to work within, create products, and analyze data in a bilingual environment, specifically in both English and Spanish.

· Provide a copy of www.vip.vetbiz.gov SDVOSB certification

Past Performance (acceptable/not acceptable) To be considered acceptable, the Government will evaluate past performance to determine how the offeror performed on recent and relevant work (within the past 3 years). Past Performance will be based on an evaluation of past performance information provided by the offeror, and/or by utilizing information obtained from all available records (e.g. PPIPRS and FAPIIS) as well.

Price The Contracting Officer will analyze the Offeror’s proposed pricing and pricing information per “(c)" above to ensure the quoted prices are fair & reasonable.

Questions and Answers (Answers in Red):

1. Is the magnet a color image that is printed on magnet paper? Does the magnet only get inserted in the first mailing? Can you provide the weight and thickness of the magnet? The magnet is in color on what I assume to be magnet paper. It is rectangular, very thin, and not a heavy magnet. If there is a second survey going to a different person from the first mailing then a magnet would be included in there. Also, refer to the 2 attachments to this amendment for example magnet photos

2. Is the annual survey quantity anticipated to remain static at 20,000 for Inpatient but increase by 500-750 annually for Home based (i.e. 4172 for 2019, 4672 for 2020, etc.)? The “B.4 Price/Cost Schedule” quantities are an estimation, the quantity could decrease or increase depending on need, which would result in a contract/task order action to address it ahead of time, but for the purposes of this solicitation the estimated quantities are to be used.
