About this file
This is an Amendment to Solicitation/Modification of Contract (SF 30, Form 0005) issued by the Department of Veterans Affairs Network Contracting Office for Hudson Valley Healthcare System. The amendment extends the deadline for receipt of quotes to September 18, 2026 at 09:45 EDT, with submissions directed to Dany Humphreys at Dany.Humphreys2@va.gov. The requirement is to replace physical therapy flooring at Castle Point VA facility with 5,500 square feet of flooring material and 600 linear feet of wall base, net of overage.
The governing product specification has been clarified as Mohawk Creative Terrain II, 36" x 36" with matching cove base, with field color to be priced and specified based on Glacial Lake 535 and walking track accent color of Tidepool Blue 565 (colors to be finalized based on manufacturer lead time, with VA reserving the right to waive samples for approval). The installation window is firm for January 2027 based on end-user scheduling constraints, with work occurring during standard hours of 7:00 am to 4:30 pm ET Monday-Friday unless otherwise authorized. Key clarifications include: VA will move all PT equipment and furniture on an approved schedule at no cost to contractor if scheduling delays occur; floor-mounted parallel bars in two locations will remain in place with flooring installed around them; moisture barrier underlay is to be included in bids with no testing authorized; and a wage determination amendment (NY20260007) has been issued with pending Department of Labor guidance on floor coverer/soft floor layer classification. Drawing 015H-PT-101 Revision 1 operates at a scale of 3/32" = 1'-0".

Dany Humphreys Contracting Officer Governing product The Statement of Work specifies 5,500 SF (plus overage) equal/equivalent to Mohawk Original Hero Rubber, 24x24, 4mm, Made in the USA, field 738 Dream and inset track 565 Navigate, with 600 LF (plus overage) of Mohawk Elemental Edges wall base.
Amendments 0002 and 0003 incorporate FAR 52.211-6 and attach Equal Product Salient Characteristics calling for Mohawk Creative Terrain II, 36x36, with cove base to match.
Which product and size govern for award: Original Hero Rubber 24x24, or Creative Terrain II 36x36 with matching cove? Are 738 Dream and 565 Navigate the required colors on the governing product? Is wall base Elemental Edges, or cove base to match Creative Terrain II?
Creative Terrain 36x36 with matching cove. Colors TBD – should be priced and specified based on field color of 535 Glacial Lake and walking track accent color of 565 Tidepool Blue.
Colors will be TBD based on final manufacturer lead time and VA reserves right to waive samples for approval.
Is moisture testing (RH, MVER, and/or pH) in scope, and who performs it? If results exceed the manufacturer’s limits for the governing product, is a topical moisture barrier / mitigation system in this contract, or would that be a change after award?
Include moisture barrier underlay in bid. No testing is authorized.
Please confirm that VA (not the contractor) will move all PT equipment, furniture, and related items out of and back into each phase area on a schedule that supports the approved 2-phase install. If VA staff are unavailable on a scheduled work day, does work stop at no cost to the contractor, or is the contractor required to move equipment?
Also confirm what stays in place (for example wall bars, mirrors, floor-mounted equipment, walking-track hardware) versus what VA will remove before flooring work begins.
VA will move all equipment and related items on final approved schedule. Contractor will not be moving equipment. Any delay due to equipment will be at no cost to contractor.
Floor mounted parallel bars in 2 locations will remain in place and flooring will be installed around it. Anything wall mounted shall remain in place.
Working hours The SOW states installation will occur during normal working hours (7:00am–4:30pm) unless subsequently requested and authorized by VA, and that this clinical area may require a basic 2-phase schedule and potentially off-hour work.
Please confirm:
· Are 7:00am–4:30pm ET, Monday–Friday, the default approved hours for all flooring work? YES
· Will VA require nights or weekends for either phase, or are off-hours only if the contractor requests them and VA authorizes? Only if authorized
· Are there blackout dates, noise limits, or clinic hours when demolition or adhesive work is not allowed? Not anticipated at this time.
Product specification: The Statement of Work issued under Amendment 0003 specifies a product equal or equivalent to Mohawk Original Hero Rubber, 24" x 24", 4mm total thickness, made in the USA, with proposed colors 738 Dream for the field and 565 Navigate for the inset track. The Equal Product Salient Characteristics attachment specifies Mohawk Creative Terrain II, 36" x 36", and the brochure at the link provided in Amendment 0003 is the Creative Terrain II brochure. Please confirm which product governs.
Product availability relative to the required schedule: The manufacturer advises that Creative Terrain II is currently at a 20-week lead time, that samples for that product would not ship until late September, and that waterjet fabrication for the inset track is not performed in-house for that line. Original Hero Rubber is advised as stock or 4 to 6 weeks with factory waterjet capability. The Statement of Work states a material lead time of 6 to 8 weeks, requires sample approval by the VA Interior Designer prior to order, and schedules the work for December 2026 or January 2027. Please confirm whether the December 2026 / January 2027 window is firm.
January 2027 window is firm and based on end user scheduling limitations. VA reserves right to waive samples for approval based on meeting lead time.
Wage determination: The document provided as the wage determination for this project is General Decision NY20260001, construction type Heavy, which contains dredging classifications and no building trades. Please confirm the wage determination applicable to this requirement. Please also advise the applicable classification for resilient floor laying, as General Decision NY20260007 lists no floor coverer or soft floor layer classification.
An amendment was issued on 8/19/2026 updating the Wage Determination from 20260001 to 20260007.
In response to the classification of Floor Coverer or Soft Floor layer:
The Contracting Officer (CO) is requesting guidance from the Department of Labor (DOL). Pending receipt of DOL guidance, the General Contractor (GC) should identify and utilize the most comparable existing classification applicable to the work. Upon receipt of DOL guidance, the CO will determine whether a request for a new classification is required.
Substrate moisture: The Statement of Work attributes the condition of the existing flooring to inherent moisture in the basement slab. Please advise whether relative humidity or moisture vapor emission test results are available, and whether a moisture mitigation system is within the scope of this requirement.
Include moisture barrier underlay in bid. No testing is authorized.
Quantities: Please confirm that 5,500 square feet of flooring and 600 linear feet of wall base are the quantities on which quotes are to be based, and whether those figures are net of the overage referenced in the Statement of Work.
Net overage.
Limitations on subcontracting certificate: FAR 52.219-14 is marked at paragraph (e)(4), construction by special trade contractors. The VAAR 852.219-75 certificate attachment does not indicate a checked box. Please confirm the box to be certified.
Special Trade Contractors as identified in FAR 52.219-14(e)(4) Drawing scale: Please confirm the plot scale of Drawing 015H-PT-101 Revision 1. The title block reads 1/8" = 1'-0" and the dimensions shown on the sheet scale at 3/32" = 1'-0".
Error – drawing is at 3/32” = 1’-0”.
