About this file

This is a Scope of Work (SOW) document for a firm fixed-price contract requiring Energy Control Procedures (ECP)/lockout tagout (LOTO) procedures implementation at two VA Medical Centers: Buffalo, NY and Batavia, NY.

The contractor must identify approximately 650 pieces of equipment requiring LOTO procedures, categorized by energy source type (multi-energy, single energy within employee control, or single energy outside employee control). All procedures must be developed, inputted into the Brady Link360 software platform, and submitted for COR approval before publication. Each procedure must include minimum requirements: equipment name and identification number, location, primary and secondary hazard types, energy magnitude, disconnect locations, isolation techniques, LOTO device type, verification steps, photos of isolation points, overhead graphic illustrations, authorized departments, restoration steps, department notifications, PPE requirements, hazard warnings, and QR codes linked to digital procedures. Approved procedures must be printed in full color with anti-UV 10mm lamination and mounted on aluminum sign bases with 6-ring binders. Equipment-specific aluminum energy source identification labels (2" x 3.5") must be installed using aircraft stainless steel safety wire or 3M tape. Outdoor equipment requires 3" x 5" aluminum QR code tags with anti-UV printing. The contractor must provide initial training for up to 15 VA employees on Brady Link360 software, procedure creation, updates, and energy source identification, plus annual training for up to 90 employees (minimum 2 hours instructor-led classroom instruction across four sessions), annual competency certification observations, and annual audits reviewing compliance with current codes and standards.

Contract duration is one base year from award plus four option years. Contractor must comply with OSHA 1910.147 and ANSI/ASSE 244.1 guidance. All contractor employees must have minimum two years' experience with Energy Control Procedures and OSHA 10-hour construction safety certification. An on-site supervisor trained in NFPA 70E and deemed a "Qualified Person" must always be present. Work must be coordinated with COR with 72 hours' notice required; normal business hours are 7:00 AM to 4:30 PM Monday through Friday, though utility shutdowns may require weekend or off-hours work with 14 working days' advance written notice. The facility operates 24 hours daily providing critical care, and contractor work must not compromise fire alarms, nurse call systems, or other life safety systems.
