B.2 STATEMENT OF WORK (SOW)

B.2.1 GENERAL

1. This firm fixed price requirement is for a triennial (occurring once every three (3) years) electrical study to include the maintenance, inspection, testing, and/or calibration of the Electrical Power Distribution System and all of its components located at the Bath (76 Veterans Avenue, Bath, NY 14810) and Canandaigua (400 Fort Hill Avenue, Canandaigua, NY 14424) VA Medical Centers (VA Finger Lakes Health Care System) beginning on 08/11/2025 and not to exceed forty-five (45) days.

2. In the following SOW, all required work is referenced as “maintenance and testing.”

3. In the following SOW, the term “contractors” is referencing Qualified Electrical Contract Professionals.

4. The contractors shall provide all necessary tools, equipment, and Personal Protective

Equipment (PPE) to perform all required work safely, effectively, and timely. All tools, equipment, and PPE shall comply with Occupational Health & Safety Administration (OSHA) Standard 29 Code of Federal Regulations (CFR) 1910, Subpart I, and National Fire Protection Association (NFPA) 70E. Contractors shall provide documentation that all instruments, test equipment, tools, and PPE have current calibration, prior to the commencement of work. Dated calibration labels shall remain visible on all instruments, test equipment, tools, and PPE.

B.2.2 SCHEDULE

1. All work shall take place during normal business hours: Monday-Friday, 8:00am to 4:30pm, excluding Federal Holidays (New Year’s Day, Birthday of Martin Luther King, Jr., Presidents Day, Memorial Day, Juneteenth, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day and, Christmas Day).

2. All services and repairs shall be performed during normal business hours, unless otherwise discussed with the Contracting Officer Representative (COR).

B.2.3 QUALIFICATIONS

1. All contractors shall be experienced and properly trained electrical contractors with licensed permanent staff to perform all the required work. The primary interest/specialty of the contractor shall be electrical contracting (construction, repair, testing, and maintenance of industrial and commercial electrical distribution systems).

2. Contractors shall be certified by the InterNational Electrical Testing Association (NETA) or an equivalent certifying organization as Certified Technicians; Each having completed the OSHA approved 10-hour construction safety training.

3. Contractors shall possess safety trainings (on the job or classroom type) in electrical safety outlined in OSHA Standard 29 CFR 1910, Subpart S – Electrical, and NFPA 70E – Standard for Electrical Safety in the Workplace.

a. The contractors shall provide training certifications indicating each technician is a “Qualified Person” as defined by NFPA 70E. Training certifications shall be submitted to the Contracting Officer prior to work. If no training certifications are available, the contractor employer shall certify that each individual has met this requirement in writing and submit this information to the Contracting Officer, prior to work.

4. Contractors shall possess technical training and a track record of work experience in maintenance, inspection, and testing of Electrical Power Distribution Systems and related components in healthcare, industrial, educational, and commercial facilities for a minimum of five (5) continuous years. Experience shall include but not be limited to the following electrical components:

a. Switchboards and Switchgear (Low and Medium Voltage)

b. Low Voltage Controls

c. Standby Generators

d. Automatic Transfer Switches, Wiring, Transformers, Meters, and Other Electrical

Appurtenances

5. Contractors shall have access to the latest versions of the following references:

a. NFPA 70, National Electrical Code

b. NFPA 70B, Recommended Practice for Electrical Equipment Maintenance

c. NFPA 70E, Standard for Emergency and Standby Power System

d. OSHA Standard 29 CFR 1910, Subparts I & S

e. InterNational Electrical Testing Association, Inc. (NETA) – Maintenance and Testing

Specifications

f. Operating/Maintenance manuals, and specifications of the electrical equipment to be maintained and tested. These documents may be obtained from the VHA Medical Center, or the equipment manufacturers.

1. Work Items, definitions, and references shall comply with the latest edition of the International Electrical Testing Association (NETA) – Maintenance Testing Specifications (MTS) and with the VHA Directive, Electrical Power Distribution System.

2. The contractors shall report deficiencies that are deemed critical or catastrophic to the Contracting Officer, Contracting Officer Representative (COR), and Chief of Facilities Management, immediately so that proper action can be taken.

B.2.5 ELECTRICAL STUDY: MAINTENANCE, INSPECTION, TESTING, AND/OR

CALIBRATION OF THE ELECTRICAL POWER DISTRIBUTION SYSTEM

1. The contractor shall provide a bid proposal to include a proposed schedule that identifies a phasing plan based on which VA Medical Center campus the work is to be performed. This should be in form of a progress chart on a suitable scale to indicate the work that is scheduled for completion by any given date during the work period – the proposed schedule shall be reviewed by the VA Medical Center Facilities Management/Engineering Service;

Any requirements for adjustments the schedule (to minimize disruption to the VA Medical Center) shall be incorporated by the contractor.

B.2.5.1 INFRA-RED SCANNING/THERMOGRAPHIC SURVEY

1. The contractor shall perform an infra-red scanning/thermographic survey on electrical equipment using an infra-red scanning camera to detect hot spots in the Electrical Power Distribution System to detect any loose, broken, or corroded connections in the system.

2. Any problem connections discovered by the contractor shall be replaced with new connectors, and/or tightened with torque wrench to mee the equipment manufacturers specifications.

3. The contractor shall ensure that appropriate safety precautions are taken before, during, and after scanning of the system, since this work must be completed while the Electrical Power Distribution System is energized.

B.2.5.2 SYSTEM FUNCTION TESTS

1. The contractor shall provide a function test for all equipment listed in Section B.3.4.

B.2.5.3 TEST CONDITIONS

B.2.5.4 4-HOUR POWER OUTAGE TEST

1. The contractor shall inspect, clean, and lubricate the main high voltage switchgear during this 4-hour period.

2. The VA Facility Management Electrical staff shall be responsible for a 4-hour test of the Essential Electrical System (EES).

a. This test shall fulfill the requirements of VHA Directive, Electrical Power Distribution System, NFPA 110 (Paragraph 8.4.9.) and The Joint Commission. This test will include opening of all utility service connections serving the VA Medical Center for a minimum of four (4) hours. During the test period, verify operation of all EES components including the transfer to emergency power and return to normal service.

b. The contractor shall include all costs associated with the services of the local utility company for the duration of the test in their bid proposal. The estimated duration of the contractor’s involvement is between eight (8) and twelve (12) hours.

B.2.5.5 GOVERNMENT FURNISHED ATTACHMENTS

1. The contractors shall maintain, inspect, and test inventory of Electrical Power Distribution System equipment.

2. One-line and other informational drawings of the Electrical Power Distribution System are not certified as-built drawings.

3. Electrical power system study information relevant to the scope of this effort, such as coordination study data, short-circuit analysis, arc-flash hazard analysis and available protective device setting information. Data from the previous study may be provided for the purpose of general information and reference.

B.2.6 SERVICE REPORTS

1. The contractor shall complete a written report of the work performed and provide it to the VA within seven (7) days of completion. Information included in the report shall be as follows:

a. Company name, Addresses, Telephone Number, and FAX Numbers

b. Name and signature of contractors who perform the maintenance and testing

c. VA Work Contract Number, Name and Number of VA Contracting Officer

d. Date and Time of Work

e. Copies of contractor’s valid licenses, professional and training certificates

f. Descriptions and model number of specialized tools and equipment used, such as torque wrench or infra-red scanning camera

g. Location, type, name, and nameplate information of electrical equipment to be maintained and tested

h. Descriptions of work items

i. Test Data

j. Reference materials such as equipment manufacturers specifications, coordination study, etc.

k. Remarks on the conditions of electrical equipment. List all deficiencies, if any.

l. Recommended corrective actions, if any.

2. The contractor shall submit four (4) hard copies of the complete written report to the

Contracting Officer within seven (7) calendar days of service visit.

3. The contractor shall submit four (4) copies of the complete written report, and one (1) CD- ROM or DVD of the electronic version of the report in Microsoft Word format to the Chief of Facilities Management/Engineering Service within seven (7) calendar days of visit. All reference materials shall be included in the electronic version of the report, either through scanning or other means of electronic text import methods.

Quotes that do not adhere to the instructions of this solicitation may be determined to be incomplete and may not be evaluated. Quote submissions should include:

1. One signed copy of the SF1449 (page 1) with blocks 17, 30a., 30b., and 30c., contract administration date (page 3) and acknowledgement of amendments section (if applicable) completed.

2. A narrative (limited to 20 pages) to specifically communicate the offeror’s technical ability to meet the requirements of the scope of work. Provide enough detail to address all aspects of the scope of work and the technical evaluation factors in section E.4, 52.212-2 of this solicitation.

Do not assume VA has any standing familiarity with subcontractors, companies, methods, or procedures being referenced. Roles of prime and subcontractors must be clearly delineated.

Specifically identify which tasks will be performed by prime contractors and which tasks will be performed by subcontractors, including all services mentioned in the scope of work.

3. Evidence of all licenses and permits required by federal, state, and local governments for performance of services as described in the scope of work.

E.4 52.212-2 EVALUATION—COMMERCIAL PRODUCTS AND

COMMERCIAL SERVICES (NOV 2021)

The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered.

Responses to this RFQ will contain sufficient information to conduct a comparative evaluation of the following factors:

Technical Capabilities

1. The vendor will provide their proposed methods for meeting the needs of this requirement including, but not limited to, staff qualifications, equipment, processes, procedures, and track record of experience to render all services described in the scope of work safely and in compliance with all laws and regulations.

2. The vendor shall possess and provide proof of certification in InterNational Electrical

Testing Association (NETA) or an equivalent certifying organization as Certified Technicians;

3. All vendor technicians shall each have completed the Occupational Health & Safety Administration (OSHA) approved 10-hour construction safety training.

4. The vendor shall possess safety trainings (on the job or classroom type) in electrical safety outlined in OSHA Standard 29 CFR 1910, Subpart S – Electrical, and the National Fire Protection Association (NFPA) 70E – Standard for Electrical Safety in the Workplace.

5. The vendor shall possess experience completing maintenance, inspection, and testing of electrical power distribution systems and related components at hospitals of similar size and complexity.

Past Performance

1. The Government shall evaluate the offeror’s past performance. By past performance, the Government means the offeror’s reputation for conforming to specifications and to standards of good workmanship, i.e., reliable service, qualifications, adherence to contract schedules to include both technical and administrative aspects of performance.

The Government shall evaluate past performance based on information that may be obtained from the Offeror or from any other source such as previous commercial or Government contracts. The Government may also use any relevant information in its possession or in the public domain. Offerors without a record of relevant past performance may not be evaluated favorably or unfavorably.

Price

1. Provide pricing by completing the Price/Cost schedule. Offerors may provide pricing in an alternate format. However, acceptance of quotes using alternative pricing formats will be at the discretion of VA.

Following receipt of quotes, the Government will perform a comparative evaluation in accordance with FAR Part 13. The Government will compare quotes to one another to select the offer that best benefits the Government.

Contractors may submit more than one quote; however, contractors are strongly encouraged to submit their best technical solutions and prices in response to this request for quote.

Once the Government determines there is/are a contractor(s) that can provide services that meet the requirements of this request for quote, the Government reserves the right to communicate with only those contractors quoting the best-suited offering to address any remaining issues.

E.5 52.212-3 OFFEROR REPRESENTATIONS AND CERTIFICATIONS—

COMMERCIAL PRODUCTS AND COMMERCIAL SERVICES (MAY 2024)

The Offeror shall complete only paragraph (b) of this provision if the Offeror has completed the annual representations and certification electronically in the System for Award Management (SAM) accessed through https://www.sam.gov. If the Offeror has not completed the annual representations and certifications electronically, the Offeror shall complete only paragraphs (c) through (v) of this provision.

(a) Definitions. As used in this provision—

Covered telecommunications equipment or services has the meaning provided in the clause 52.204-25, Prohibition on Contracting for Certain Telecommunications and Video Surveillance Services or Equipment.

Economically disadvantaged women-owned small business (EDWOSB) concern means a small business concern that is at least 51 percent directly and unconditionally owned by, and the management and daily business operations of which are controlled by, one or more women who are citizens of the United States and who are economically disadvantaged in accordance with 13 CFR 127, and the concern is identified by SBA or an approved third-party certifier in accordance with 13 CFR 127.300. It automatically qualifies as a women-owned small business eligible under the WOSB Program.

Forced or indentured child labor means all work or service—

(1) Exacted from any person under the age of 18 under the menace of any penalty for its nonperformance and for which the worker does not offer himself voluntarily; or

(2) Performed by any person under the age of 18 pursuant to a contract the enforcement of which can be accomplished by process or penalties.

Highest-level owner means the entity that owns or controls an immediate owner of the offeror, or that owns or controls one or more entities that control an immediate owner of the offeror. No entity owns or exercises control of the highest level owner.

Immediate owner means an entity, other than the offeror, that has direct control of the offeror.

Indicators of control include, but are not limited to, one or more of the following: Ownership or interlocking management, identity of interests among family members, shared facilities and equipment, and the common use of employees.

Inverted domestic corporation means a foreign incorporated entity that meets the definition of an inverted domestic corporation under 6 U.S.C. 395(b), applied in accordance with the rules and definitions of 6 U.S.C. 395(c).

Manufactured end product means any end product in product and service codes (PSCs) 1000- 9999, except—

(1) PSC 5510, Lumber and Related Basic Wood Materials;

(2) Product or Service Group (PSG) 87, Agricultural Supplies;

(3) PSG 88, Live Animals;

(4) PSG 89, Subsistence;

(5) PSC 9410, Crude Grades of Plant Materials;

(6) PSC 9430, Miscellaneous Crude Animal Products, Inedible;

(7) PSC 9440, Miscellaneous Crude Agricultural and Forestry Products;

(8) PSC 9610, Ores;

(9) PSC 9620, Minerals, Natural and Synthetic; and

(10) PSC 9630, Additive Metal Materials.

Place of manufacture means the place where an end product is assembled out of components, or otherwise made or processed from raw materials into the finished product that is to be provided to the Government. If a product is disassembled and reassembled, the place of reassembly is not the place of manufacture.

Predecessor means an entity that is replaced by a successor and includes any predecessors of the predecessor.

Reasonable inquiry has the meaning provided in the clause 52.204–25, Prohibition on Contracting for Certain Telecommunications and Video Surveillance Services or Equipment.

Restricted business operations means business operations in Sudan that include power production activities, mineral extraction activities, oil-related activities, or the production of military equipment, as those terms are defined in the Sudan Accountability and Divestment Act of 2007 (Pub. L. 110-174). Restricted business operations do not include business operations that the person (as that term is defined in Section 2 of the Sudan Accountability and Divestment Act of 2007) conducting the business can demonstrate—

(1) Are conducted under contract directly and exclusively with the regional government of southern Sudan;

(2) Are conducted pursuant to specific authorization from the Office of Foreign Assets Control in the Department of the Treasury, or are expressly exempted under Federal law from the requirement to be conducted under such authorization;
