About this file

This is a VA Form 2105 Memorandum from Richard Cox, Contracting Officer's Representative, documenting questions and answers regarding RFI 526-CSI-137 for Site Support for True Beam Linear Accelerator at the James J. Peters VA Medical Center in Bronx, NY.

The Q&A addresses multiple technical clarifications including: a discrepancy between bid pricing sheets and drawings regarding deduct alternates, phasing requirements referencing Drawing GI-002, noise/dust control requirements for concrete trenching work, after-hours work requirements for specific rooms, VA's preference for plastic over glass in casework, millwork specifications, electrical panel and WiFi device replacements, and confirmation that the new LINAC equipment will be furnished and installed by Varian (the manufacturer) in coordination with the VA and General Contractor. The document also confirms that full-time CQC personnel must be hired by the General Contractor, self-leveling compound is required for the floor in room 105, and temporary signage must be provided in work areas.
