See CONTINUATION Page This solicitation is for a Indefinite Delivery/ Indefinite Quantity requirement is for year-round grounds maintenance and and landscaping services to be provided to the East Orange and Lyons campuses of the New Jersey VA Healthcare System located at: 385 Tremont Avenue, East Orange NJ 07018 and 151 Knollcroft Rd, Lyons, NJ 07939. The period of performance shall be from 10/01/2020 through 09/30/2021 to include four option years. These services shall include lawn maintenance services, mulching services, planting bed maintenance services, tree and shrub maintenance services, street sweeping services, and snow removal services on a per push” basis.

B.3 PERORMANCE WORK STATEMENT (PWS)
A) This Indefinite Delivery/ Indefinite Quantity requirement is for year-round grounds maintenance and landscaping services to be provided to the East Orange and Lyons campuses of the New Jersey VA Healthcare System located at: 385 Tremont Avenue, East Orange NJ 07018 and 151 Knollcroft Rd, Lyons, NJ 07939. The period of performance shall be from 10/01/2020 through 09/30/2021 to include four option years. These services shall include lawn maintenance services, mulching services, planting bed maintenance services, tree and shrub maintenance services, street sweeping services, and snow removal services on a per event or “per push” basis.
B.3.1 SERVICES FOR EAST ORANGE CAMPUS OF THE NEW JERSEY VA HEALTHCARE SYSTEM
B.3.1.1 GROUNDS MAINTENANCE AND LANDSCAPING
B.3.1.1.1 GENERAL
A) Provide all labor, equipment, supervision, tools, and all materials necessary to provide maintenance, care and housekeeping of all exterior landscaped areas (approximately 36 acres) to include a turf management program to be performed between April 1st and November 30th of each calendar year as defined in the B.3.1.1 GROUNDS MAINTENANCE AND LANDSCAPING of this Performance Work Statement.
B) A facility site plan indicating work areas shall be provided by the COR after award.
C) The contractor shall possess the following licenses: NJ State Fertilizer License, State of NJDEP Commercial Pesticide Applicator License with Categories 3A – 3B and State of NJDEP Business License
D) The contractor shall provide Safety Data Sheets SDS for all chemicals to be used to the Contracting Officer’s Representative (COR) for approval before use.
E) All work shall be performed during the hours of 8:00 AM to Sunset, Monday thru Saturday excluding Federal Holidays (defined as: New Year's Day, Birthday of Martin Luther King, Jr., Washington's Birthday, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day and Christmas Day)
F) Upon arrival to perform any grounds maintenance and landscaping services, the Contractor shall report to the Boiler Plant Foreman in Bldg. 8, 973-676-1000 Ext. 1882.
G) Upon completion of tasks describer under section B.3.1.1 GROUNDS MAINTENANCE AND LANDSCAPING the contractor shall provide a copy of service report to the COR. Service reports shall be signed by both the contractor and COR at completion of work. An electronic version of same report shall be sent to the COR. Service reports shall include tasks completed to include areas worked on, number of workers involved and hours worked.
B.3.1.1.2 GROUNDS MAINTENANCE
A) All grass on the facility shall be mowed at a height of two (2) to three (3) inches. Mowing shall be done between April 1st and November 30th, and be completed frequently enough so that no more than 1/3 of the grass blade is removed per mowing with a maximum number of 33 cuttings to be performed in each period of performance of this contract. All enclosed areas with grass shall be mowed and maintained. The childcare / play area and all enclosed areas along with all gardens of any type will be properly maintained including any weeding, mulching and application of herbicide as needed to present a clean manicured appearance. All shrubs and bushes shall be trimmed to the correct height and shape as determined by the COR. Concrete curbs and walks that border lawn areas will be mechanically edged by blade monthly and managed as needed to present a manicured appearance. Perimeter fencing will be kept clear of any kind of foliage. Care must be taken to avoid damaging parked vehicles. Some areas will have to be maintained in the evening hours or on weekends when the areas are generally empty due to the hazards of flying debris caused by string trimmers and other lawn care equipment.
B) Herbicide, seeding and fertilization schedule shall be as follows:
1. Spring pre-emergent in April
2. Spring fertilization and grass seeding where needed in May
3. Seasonal weed and feed in June, July and August
4. Fall fertilization and grass seeding where needed in September
5. A post emergent herbicide shall be applied eight (8) times per season to control the emergence of weeds and grass in curbs, pavement cracks, pavers, concrete expansion joints and parking lot dividers.
C) Maintenance of fallen leaves and debris during the months of October and November shall be provided on a weekly basis or more often if needed to maintain a clean, neat appearance. The contractor shall remove leaves from lawns, parking areas and primary focal points.
D) A general spring clean-up shall be provided to remove leaves and debris accumulated over the winter months from lawns and beds. This includes the removal of sand and cinders from lawn areas that may have accumulated over the winter. The removal and disposal of all debris generated during the spring and fall clean-up shall be legally disposed of off-site.
E) The mulch located in the areas listed below shall be maintained April 1 through October 1of each calendar year to maintain a uniform appearance
1. All islands around Jacobs circle
2. All areas to the right and left of the front entrance
3. Circle of flags island on Gregg drive
4. Circle of flags island on Oresko St.
5. Front and side of building 5
6. Island at front entrance of building 15
7. Child care play area
8. All gardens
F) The dark brown organic mulch at these locations shall be maintained at a depth of 2- to 4-inches (5- to 10-cm)
G) The mulch shall be raked as needed to break up any matted layers and refresh its appearance.
H) The mulch shall be kept a few inches away from all shrubs, flower and trees.
I) Rubber mulch shall be replaced or refreshed where needed.
B.3.1.2 TREE MAINTENANCE
B.3.1.2.1 GENERAL
A) Provide all labor, equipment, supervision, tools, and all materials necessary to provide tree maintenance services to include : trimming and thinning services including removal of dead trees and stump grinding on grounds totaling 36 +/- acres. Tree maintenance services shall be provided as defined in the B.3.1.2 TREE MAINTENANCE of this Performance Work Statement.
B) A facility site plan indicating work areas shall be provided by the COR after award.
C) All work shall be performed during the hours of 7:30 AM and 3:30 PM, Monday thru Friday excluding Federal Holidays (defined as: New Year's Day, Birthday of Martin Luther King, Jr., Washington's Birthday, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day and Christmas Day) during the last full week of the month.
D) All work performed under section B.3.1.2 TREE MAINTENANCE shall be done in accordance with ANSI Z133 2017 The American National Standard for Arboricultural Operations - Safety Requirements and ANSI A300 Standards - Tree Care Industry Association.
E) Upon arrival to perform any tree services, the Contractor shall report to the Boiler Plant Foreman in Bldg. 8, 973-676-1000 Ext. 1882 or Boiler Plant Operator Bldg. 8, Ext. 1264.
F) Upon completion of tasks describer under section B.3.1.2 TREE MAINTENANCE the contractor shall provide a copy of service report to the COR. Service reports shall be signed by both the contractor and COR at completion of work. An electronic version of same shall be sent to the COR. B.3.1.2.2 CONTRACTOR QUALIFICATIONS
A) The contractor shall be registered with the New Jersey Board of Tree Experts and employ one Licensed Tree Care Operator or Licensed Tree Expert. Proof of registration and licensing shall be provided to the COR before work can commence
B) An individual that has completed the OSHA 30hr Safety Training Certification a shall be on site when any work under section B.3.1.2 TREE MAINTENANCE is performed. OSHA 30hr Safety Training Certification certificates and Tree Care Operator Licenses shall be provided to the COR or approval before an individual can be considered acceptable to meet this requirement. It is recommended but not required that all personnel performing work under section B.3.1.2 TREE MAINTENANCE have completed the OSHA 10hr Safety training course.
B.3.1.2.3 CONTRACTOR VEHICLES AND EQUIPMENT
A) All vehicles used directly for tree maintenance services shall possess NJTC# issued by the Board of Tree Experts and be properly registered with the State Department of Motor Vehicles.
B) All operators of all vehicles used directly for tree maintenance services shall possess a current New Jersey Commercial Driver’s License (CDL). The Contractor shall submit copies of CDL licenses for individuals operating vehicles possessing NJTC#s to the COR for approval before they are allowed to operate the vehicle on VA property.
C) Contractors’ vehicles shall be routed through major thoroughfares whenever possible and shall not be routed through residential areas immediately adjacent to the East Orange Campus.
D) Equipment used to perform service shall be kept secure from all patients and visitors at all times to include saws, pole saws, pruning equipment, and fuel cans.
B.3.1.2.4 SCHEDULED SERVICES
A) Small Trees up to 5” Caliper or up to 12’in height shall be spot pruned once per season in the spring employing the standards of stated in ANSI A300 Standards - Tree Care Industry Association to remove dead or damaged branches and to develop the natural form of the plant and its intent in the landscape.
B) A general spring clean-up shall be provided in coordination with the with coordination with the COR to removal of dead or dying branches, or branches that present a hazard to buildings and personnel
1. The vendor shall only remove branches that are ½ inch in caliper diameter or larger.
C) All wooden debris from tree maintenance services shall be removed off the grounds of the East Orange Campus for proper disposal at the end of each work day. The Contractor shall assume all responsibility for compliance with all Federal, State, and Municipal Laws, rules, and regulations governing the handling, transportation, and disposal of vegetative waste. Before commencing work the Contractor shall furnish copies of all required licenses and permits. In addition, evidence shall be submitted that all receiving sites possess the required permits and licenses to accept waste from the Contractor.
D) The Contractor shall be responsible for all cleanup due to performance of this contract. The Contractor shall be accountable for all cleanup costs and restoration costs required due to damage of VA property through neglect or accident as a result of contractor employee actions.
B.3.1.2.5 UNSCHEDULED SERVICES
A) The contractor shall be available upon request of the COR to provide unscheduled tree maintenance services to include:
1. Removal of dead or dying trees
2. Removal of dead or dying branches, or branches that present a hazard to buildings and personnel
3. Stump removal or grinding (include adding soil, grade and seed stump location)
4. Removal or trimming of trees felled or damaged by storms
B.3.1.3 STREET SWEEPING SERVICES
B.3.1.3.1 GENERAL
A) Provide all labor, equipment, supervision, tools, and all materials necessary to provide to provide year-round monthly street sweeping services of all roads and parking lots to include: a vacuum truck and operator, removal and transport of sweepings for disposal from the grounds of the East Orange Campus. Street sweeping services shall be all as defined in the B.3.1.3 STREET SWEEPING SERVICES of this Performance Work Statement.
B) A facility site plan indicating work areas and routes will be provided by the COR.
C) All work shall be performed during the hours of 4:30 PM and 11PM, Monday thru Friday excluding Federal Holidays (defined as: New Year's Day, Birthday of Martin Luther King, Jr., Washington's Birthday, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day and Christmas Day) during the first full week of the month.
D) Connection to the water system to be coordinated with the COR and shall only be permitted with the use of an approved backflow preventer.
E) The Contractor shall be responsible for all cleanup due to performance of this contract to include spillage of any kind. The Contractor shall be accountable for all cleanup costs and restoration costs required due to damage of VA property through neglect or accident as a result of contractor employee actions.
F) Upon arrival to perform any tree services, the Contractor shall report to the Boiler Plant Foreman in Bldg. 8, 973-676-1000 Ext. 1882 or Boiler Plant Operator Bldg. 8, Ext. 1264.
G) Upon completion of tasks describer under section B.3.1.3 STREET SWEEPING SERVICES the contractor shall provide a copy of service report to the COR. B.3.1.3.2 VEHICLE AND OPERATOR REQUIREMENTS
A) Vehicles used for street sweeping services shall be properly registered with the State Department of Motor Vehicles, the Board of Public Utilities, and the NJ Dept. of Environmental Protection.
B) Vehicles used to sweep streets shall be capable of measuring and reporting the volume of waste material removed. Truckload capacity shall be plainly marked on sweeping vehicle(s).
C) Vehicles transporting waste to a licensed collection point shall have an A-901 License to Transport Solid Waste
D) If the contractor is using a dumpster to transport waste off of VA property, the Contractor shall indicate this in their quote and provide details of the size of the dumpster. The use of a dumpster shall only be on site during street sweeping operations and its use shall be coordinated with the COR before use.
E) In addition, evidence shall be submitted that all receiving sites possess the required permits and licenses to accept waste from the Contractor.
F) Vehicles shall be routed through major thoroughfares whenever possible and shall not be routed through residential areas immediately adjacent to the East Orange Campus.
G) All vehicle operators shall possess a Commercial Driver’s License from the state of New Jersey. The Contractor shall submit copies of all street sweeper operators to the COR for approval before they are allowed to operate a street sweeper on VA property.
B.3.1.4 SNOW AND ICE REMOVAL SERVICES
B.3.1.4.1 GENERAL
A) Provide all labor, equipment, supervision, tools, salt, calcium and all materials necessary to remove snow and ice from the streets, parking lots, sidewalks, steps and ramps at the East Orange VA Medical Campus on a per event basis between November 1st and April 30th of each calendar year as defined in the B.3.1.3 SNOW AND ICE REMOVAL of this Performance Work Statement.
B) Resources to be provided for services under this section include snowplow trucks for roads and parking lots, individuals to shovel, spread salt and operate snow blowers for walkways, and loaders and dump trucks to attend to or remove snow piles generated by snow removal activities. All services provided shall be invoiced in an hourly rate based on the number and types of resources requested for a given weather event. The East Orange VA Medical Campus is requesting that the contractor reserve the use of one front loader to be located for the season at the facility, two snowplow trucks and eight individuals for clearing stairs and walkways each snow season.
C) A facility site plan indicating work areas shall be provided by the COR after award.
D) The Contractor shall be responsible for complete the snow removal and ice control at this facility ensuring that all roads, parking lots and walkways are kept open and passable at all times. It is expected that sufficient machinery and manpower will be on site to perform this task. All locations that are covered in this contract are listed below. If there is an area that has been omitted by error, it is expected that the contractor will be responsible for clearing and treating the area when the omission is discovered. Snow removal shall proceed continuously, and in the order as listed below, to include all roads, parking lots and sidewalks to the satisfaction of the COR prior to being discontinued.

E) The Contractor shall supply the COR with 24 hour emergency phone number to request snow removal services.
F) The Contractor shall be notified by COR or their designated representatives between 12 – 18 hours prior to snow event and shall be on station at the time designated by the COR or individuals authorized to call in services when the COR is not available.
G) The Contractor shall receive a guaranteed minimum payment of six (6) hours per event for each vehicle and individual requested. This guarantee only applies if all requested resources arrive at or before the arranged time with the COR for the given event ready to operate. If this condition is not met then the guarantee is not applicable and each resource shall only be paid from the time it was confirmed on-site and for the actual hours worked. This guaranteed minimum shall not apply to the use of vehicles to remove excess snow from the premises.
H) Upon arrival to perform any snow and ice removal, the Contractor shall report to the Boiler Plant Foreman in Bldg. 8, 973-676-1000 Ext. 1882, Boiler Plant Operator Bldg. 8, Ext. 1264 or Chief, Operations, 973-676-1000 Extension 2029, Cellphone 908-413-2762.
I) Salt provided by Contractor shall be stored inside back of Building 6. Salt is to be provided in bags only. No bulk salt to be delivered to site.
J) If any snow piles created from plowing become so large that it becomes a hazard to the facility’s personnel, patients and visitors, the Contractor shall at the COR’s request, truck the snow away to an onsite location provided by the COR or in the event that there is no room at the facility for this snow it will be taken offsite and disposed of in accordance with local or government regulations.
K) Upon completion of tasks described under section B.3.1.3 SNOW AND ICE REMOVAL the contractor shall provide a copy of service report to the COR. Service reports shall be signed by both the contractor and COR at completion of work. An electronic version of same shall be sent to the COR. Service reports shall include tasks completed to include areas worked on, number of workers involved and hours worked.
B.3.1.4.1 VEHICLE AND OPERATOR REQUIREMENTS
A) Vehicles used for snow and ice removal services shall properly registered with the State Department of Motor Vehicles B) Snowplow vehicles shall have a minimum Gross Vehicle Weight (G.V.W.) of 20,000 lbs
C) The G.V.W. shall be defined as the total weight of the vehicle on its tires as it rests or rolls on the road, including chassis, cab, body, equipment, oil, water, fuel, the driver and the maximum payload and/or ballast. Each truck shall have appropriate ballast to plow the varying amounts of snow. TRUCKS FOUND TO BE WITHOUT BALLAST AND INCAPABLE OF PUSHING A FULL BLADE OF HEAVY SNOW WILL BE AUTOMATICALLY TAKEN OFF PAYABLE TIME AND RELEASED.
D) The Contractor shall be responsible for fuel, ballast, chains, repairs, insurance, safety lighting and all other items necessary for operations. All equipment must be refueled with a minimum loss of operating time. The Contractor shall receive no compensation for those hours during which the truck or equipment fails to operate due to mechanical trouble or lack of proper ballast. These hours shall be deducted from the six (6) minimum if applicable.
E) All trucks providing snow plowing/removal services shall be equipped with amber safety lights and shall be of the strobe, LED, or rotating type. The truck shall be equipped with the amber safety lights mounted to be visible from 360 degrees or a minimum of two safety lights, one mounted on the back of the truck and one in the front. The safety lights are required in addition to the vehicle’s normal hazard lights and must be visible from the rear of the truck. Also, reflective conspicuity markings shall be placed on the rear and sides of the vehicle in conformance with New Jersey and Federal Motor Vehicle Regulations and Safety Standards.
F) Vehicles shall be routed through major thoroughfares whenever possible and shall not be routed through residential areas immediately adjacent to the East Orange Campus.
G) All vehicle operators shall possess a Commercial Driver’s License Class B with air brake endorsement from the state of New Jersey. The Contractor shall submit copies of all snowplow operators to the COR for approval before they are allowed to operate a snow plow on VA property.
B.3.2 SERVICES FOR LYONS CAMPUS OF THE NEW JERSEY VA HEALTHCARE SYSTEM
B.3.2.1 GROUNDS MAINTENANCE AND LANDSCAPING
B.3.2.1.1 GENERAL
A) Provide all labor, equipment, supervision, tools, and all materials necessary to provide maintenance, care and housekeeping of all exterior landscaped areas (approximately 214 acres) to include a turf management program to be performed between April 1st and November 30th of each calendar year as defined in the B.3.2.1 GROUNDS MAINTENANCE AND LANDSCAPING of this Performance Work Statement.
B) A facility site plan indicating work area shall be provided by the COR after award.
C) The contractor shall possess the following licenses: NJ State Fertilizer License, State of NJDEP Commercial Pesticide Applicator License with Categories 3A – 3B and State of NJDEP Business License
D) The contractor shall provide Safety Data Sheets SDS for all chemicals to be used to the Contracting Officer’s Representative (COR) for approval before use.
E) All work shall be performed during the hours of 6:00 AM to 3:30 PM, Monday through Friday excluding Federal Holidays (defined as: New Year's Day, Birthday of Martin Luther King, Jr., Washington's Birthday, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day and Christmas Day)
F) Upon arrival to perform any grounds maintenance and landscaping services, the Contractor shall report to Contractor shall report to the Chief, Operations Section, Bldg. 10, Room 209, Ext. 4284.
G) Upon completion of tasks described under section B.3.1.1 GROUNDS MAINTENANCE AND LANDSCAPING the contractor shall provide a copy of service report to the COR. B.3.2.1.2 GROUNDS MAINTENANCE
A) All grass on the facility shall be mowed at a height of two (2) to three (3) inches. Mowing shall be done between April 1st and November 30th, frequently enough so that no more than 1/3 of the grass blade is removed per mowing with a maximum number of 33 cuttings to be performed in a period of performance.
B) All shrubs and bushes shall be trimmed to the correct height and shape consistent with the culture of the plant and its intent in the landscape. Shrubs and Evergreen Trees (where applicable) up to a height of 12’ and in formal in shape shall be sheared twice per season or more as deemed necessary by the Contractor to provide a neat, manicured appearance.
C) Care shall be taken to avoid damaging parked vehicles. Some areas will have to be maintained in the evening hours or on weekends when the areas are generally empty due to the hazards of flying debris caused by string trimmers and other lawn care equipment.
D) All pruning of shrubbery shall include the removal of dead or damaged branches. Once in spring shrubbery shall be inspected for damages occurring over‐winter and pruned in the best interest of the plant. Removal of expired shrubbery or trees is not part of this agreement.
1. Deciduous shrubbery shall be sheared or hand pruned as dictated by the plant’s culture and intent of the VA representative. Flowering shrubs shall only be pruned after their period of bloom.
2. Evergreen non-formal shrubs shall be sheared or hand pruned employing the plunge-‐cut method as described by ANSI A300 Standards - Tree Care Industry Association to remove excess growth and manage plant size and intent while providing the plant to necessary oxygen and light.
3. Evergreen Ground-Cover beds shall be pruned as to provide the correct form and intent of the plant. Delineation shall be maintained wherever evergreen ground-‐covering plants border curbs or paved walks to prevent hazardous conditions.
4. All planting beds and tree saucers shall be delineated (edged) once annually and maintained regularly with a mechanical edger.
5. Lawn trees, described as any ornamental tree within a lawn area and not in a defined planting area, tree grate or other defined landscape area shall receive a cultivated and edged tree saucer. Shade trees shall not be edged unless a pre‐existing saucer exists.
6. Lawn tree saucers or ‘tree rings’ shall be mechanically edged once in spring and this delineation maintained as necessary throughout the season to provide a neat and manicured appearance.
E) All ornamental trees shall be fertilized once in spring using a fertilizer of 10-‐6-‐4 at the rate of 1 Lb. per 1” of trunk caliper at breast height. Shrubbery and ground cover plants shall be fertilized using 10-‐6-‐4 at the rate of 4 Lbs. per 100 square feet of bed areas once in spring. Acid loving trees and shrubs shall be fertilized using a specialty fertilizer intended for such plants.
F) As part of this agreement, Contractor shall employ a comprehensive tree and shrub IPM (Integrated Pest Management Plan). As part of this plan, all trees and shrubs shall be monitored for conditions that could adversely affect the plants. The goal is to prevent or limit damage done to landscape plants by utilizing targeted pest management.
