2. Section E.3, Volume III, Factor 2 Past Performance is hereby updated as follows

(changes are highlighted in yellow);

FROM:

VOLUME III:

Factor 2 – Past Performance

The offeror shall complete Attachment C Past Performance Questionnaire in its entirety, listing a total of three contracts/orders for work performed in the past three years or currently being performed, for HCCH to support healthcare EDI and conversion of paper-based Health claims and associated medical documentation to electronic format. The Government will make the determination of relevance (based on size, scope, and complexity of requirement).

Offerors are advised that the solicitation considers past performance to be relevant which reflects the following:

1. Proven history of completing a minimum of 9.5 million EDI transactions per month.

2. Proven history of converting a minimum of 700,000 clean paper-based forms per month to HIPAA compliant EDI transactions within 2 business days of receipt and a minimum of converting 2.1 million total pages of paper per month into digital format.

Contracts/orders listed may include those entered into with the Federal Government, agencies of state and local governments and commercial concerns. Offerors may also submit past performance information regarding predecessor companies, proposed first-tier subcontractors that will perform major or critical aspects of the requirement. Offerors may also submit past performance information of a proposed first-tier subcontractor. For the purposes of this solicitation, a first-tier subcontractor is defined as an entity proposed to conduct 20% or more of the total contract effort. Data concerning the prime contractor shall be provided first, followed by each proposed first-tier subcontractor, in alphabetical order.

Recency and Relevancy:

The Government will conduct a past performance assessment based on the relevancy (size, scope, and complexity) and recency (within the last 3 years) of the offerors past performance, as well as that of its subcontractor(s), as it relates to the probability of successful accomplishment of the required effort.

Past Performance Questionnaires (PPQ - Attachment C) may be used in lieu of CPARS if the Offeror does not have a CPARS rating to submit. Each offeror’s past performance will be evaluated based on CPARS and/or PPQ Ratings in the factors below. Summary shall address the following elements:

• Relevant CPARS/PPQs are similar in size for total dollar value, scope for type of contract, and complexity for tasks performed as defined above.

• Quality of Product or Service – compliance with contract requirements – technical excellence and know-how – CPARs Ratings or PPQs with past organizational contracts or programs – responsiveness and accuracy of communication – assignment of qualified and appropriate personnel – proactive in suggesting designs, tools, or layout to improve look and accessibility of a site – demonstrated strategies and plans for program/project management and maintenance to include Congressionally Mandated Reports.

• Timeliness of Performance – meets milestones and deadlines – quick turnaround time on requests – promptly communicates issues– reliable – stays on schedule – responsive to technical direction – completes tasks on time.

• Problem Resolution – anticipates, avoids, or mitigates problems – proactive in monitoring web site for content freshness – satisfactorily overcomes or resolves problems – prompt notification of problems – recommends viable solutions.

• Cost Control – consistently within task budgets – current, accurate, and complete billings – costs properly allocated – unallowable costs not billed – relationship of negotiated costs to actual costs – cost efficiencies.

• Business Relations – effective management – accountability for employee performance – business-like concern for customer’s interests – effective use of web technology – effective supervision of staff – reasonable/cooperative behavior – flexible – maintains high employee morale.

The offeror shall instruct the POC at the referenced entity to return a copy of the completed PPQ directly to the CO and the CS at Terri.Duenas@va.gov and Justin.Sink@va.gov NO LATER THAN THE CLOSING DATE OF THE SOLICITATION (inclusive of any closing date extensions granted via amendment).

The Government is not required to contact references provided by the offeror. Also, references other than those identified by the offeror may be contacted by the Government to obtain additional information that will be used in the evaluation of the offeror's past performance. The Government may consider past performance information obtained through other sources which is reasonably available and determined to be relevant. Past performance information obtained will be used for both the responsibility determination and best value decision.

Offerors shall indicate on Attachment C if they have no record of relevant or recent past performance.
