STATEMENT OF WORK

Removal of Resilient Tile Flooring/Carpet Tiles and Installation of Commercial LVP Flooring with 6" Cove Base at VBA Boise Regional Office

1.0 OBJECTIVE: The objective of this contract is to remove approximately 2000 square feet of existing resilient tile squares and replace it with commercial-grade Luxury Vinyl Plank (LVP) 20mil flooring, including the installation of a 6" cove base and thresholds. The 2000 sq ft includes two offices approximately 10x15 each, requiring carpet tile removal.

2.0 SCOPE OF WORK: The contractor is responsible for providing all labor, materials, tools, equipment, and services necessary to complete the project as described below:

2.1 Pre-Work Preparation

2.1.1 Conduct a site inspection and document the existing conditions. Confirm square footage requirements. See Attachment One - Facility Drawing with areas highlighted in yellow.

2.1.2 Finalize color/design and coordinating cove base color selections for LVP with COR from the product line(s) specified in the contract award. Provide samples on-site for COR to evaluate. The COR will approve the final selection prior to installation. This is an example of the desired color family and pattern of LVP.

2.1.3 The contractor shall submit a Material Submittal for the actual flooring and flooring adhesive, for Government approval, in writing, prior to installation. The contractor is responsible for all submittals meeting VA specifications and requirements.

2.1.3 Move furniture, refrigerators and other items from the work areas to allow access to the flooring. Contractor will coordinate with the COR or facility manager to determine what items need to be moved and their temporary storage location. The two offices have modular desk surface sections totaling approx. 5’x8’ mounted to wall brackets which will need to be dismounted and reinstalled. See photos below.

2.1.4 Implement measures to protect non-work areas from dust and debris during the removal and installation process.

2.2 Removal of Existing Flooring

2.2.1 Remove approximately 2000 square feet of existing resilient tile flooring and carpet tiles.

2.2.2 Dispose and remove from work site the removed flooring and any resulting debris in compliance with local, state, and federal regulations.

2.2.3 Protect non-work areas from debris and dust using appropriate abatement methods such as plastic tarps and tape.

2.3 Subfloor Preparation

2.3.1 Inspect the subfloor for damage or irregularities and report any issues to the facility manager.

2.3.2 Prepare the subfloor as necessary to ensure a smooth and level surface for the installation of the new flooring. Preparation may include cleaning, patching, and sanding.

2.4 Installation of Commercial LVP Flooring

2.4.1 Install 25mm commercial LVP flooring in accordance with the manufacturer's specifications and industry best practices.

2.4.2 Ensure the flooring is properly aligned, with minimal seams and a consistent appearance throughout the entire installation area. Flooring will flow seamlessly between the hallway and breakroom area.

2.4.3 Use adhesive as recommended by the LVP flooring manufacturer.

2.5 Installation of 6" Cove Base and Thresholds

2.5.1 Install 6" cove base around the perimeter of the 2000 square foot installation area.

2.5.2 Ensure the cove base is securely attached to the wall and provides a neat and uniform finish.

2.5.3 Install transitional thresholds to carpet for 7 single doors and one double door entry way. Three single door threshold transitions will be from LVP to travertine tile. Five thresholds will transition from LVP to concrete or tile.

2.6 Post-Work Clean-Up

2.6.1 Clean the newly installed flooring and remove any adhesive residue or debris.

2.6.2 Return furniture and other items to their original locations.

2.6.3 Remove and dispose of all project-related waste materials. Ensure the work area is left in a clean and orderly condition.

3.0 PERFORMANCE OF WORK:

3.1 The Regional Office building is located on the VA Medical Center campus. Services shall be performed at the following location: Boise Regional Office (RO), 444 West Fort Street, Boise, ID 83702.

3.2 The Regional Office is expected to close for the duration of this project to prevent disruption of daily operations from interfering with the flooring. The building shall close for no longer than 5 weekdays. Work may be performed on weekends. There are no restrictions on duty hours. Project must be completed within a timeframe of 7-9 days (5 weekdays and up to two weekends.)

3.3 Installation shall commence within 45 days of contract award.

4.0 DELIVERABLES

4.1 Material submittals, as indicated

4.2 Documented pre-work site inspection forwarded to the Contracting Officer and COR.

4.3 Final inspection report detailing the completed work forwarded to the Contracting Officer and COR.

5.0 QUALITY ASSURANCE

5.1 The contractor shall implement a quality control plan to ensure all work meets the specified requirements.

5.2 The final installation will be inspected and approved by the COR/facility manager or designated representative.

6.0 CONTRACTOR IDENTIFICATION AND FACILITY SECURITY

6.1 The Contractor shall identify a minimum of 2 personnel (foremen/management) to be fingerprinted and issued a PIV-I badge. These personnel must be available to be fingerprinted a minimum of 10 days prior to commencement of work. At least one of these personnel must be on-site at all times while work is being performed and will be responsible for escorting other contractor personnel, providing building access, and ensuring the building is secured when daily work is completed. Upon project completion, the PIV-I badges will be returned to the Government.

6.2 The Contractor shall initiate an identification program for each of its employees, subcontractors, and subcontractor employees when on the VBA Regional Office property. Each person will be identified with a unique readily visible badge identifying the person and that the person is under the direction of the General Contractor. Every employee or subcontractor shall wear the badge displayed prominently at all times when on site.

6.3 Preferred contractor access and parking will be in the rear parking lot of the facility. Exterior facility doors will not be physically propped open at any time. If an exterior door needs to be held open for material movement, close-in work, etc., the door will be manned by contractor personnel at all times to prevent unauthorized entry to the Regional Office.

7.0 SECURITY REQUIREMENTS

7.1 Security Plan

7.1.1 The security plan defines both physical and administrative security procedures that will remain effective for the entire duration of the project.

7.1.2 The General Contractor is responsible for assuring that all subcontractors working on the project and their employees also comply with these regulations.

7.2 Security Procedures

7.2.1 All general contractor’s employees, subcontractors, and material suppliers entering the VBA consent to search by VA police at any time. The areas of search shall be all encompassing including but not limited to: their person; their personal effects; personal vehicles, as well as any and all contractor vehicles, equipment, trailers, offices, storage shed, tool boxes and any and all containers. Weapons are not permitted on VA property.

7.2.2 No photography of VA premises is allowed without written permission of the Contracting Officer.

7.2.3 VA reserves the right to close down or shut down the project site and order General Contractor’s employees off the premises in the event of a national emergency. The General Contractor may return to the site only with the written approval of the Contracting Officer.
