The purpose of this amendment is to:
1 – Extend the due date for the Request for Quote to Tuesday, April 28, 2026 at 1PM MT.
2 – Update the Addendum to 52.212-1 Instructions to Offerors to reflect the rescheduling of the Site Visit to Tuesday, April 21, 2026 at 10AM MT and the due date for questions to Thursday, April 23, 2026 at 1PM MT.
3 – Amend the Past Performance Questionnaire to reflect due date NLT Tuesday, April 28, 2026 at 1 PM.
4 – Amend Paragraph 3.3 with 3.0 Performance of Work within the SOW to reflect the correction of installation commencing within 30 days of contract award to 45 days of contract award.
All other terms and conditions remain unchanged.

ADDENDUM TO 52.212.1 - INSTRUCTIONS TO OFFERORS
VBA Boise Regional Office Flooring Replacement
The Government anticipates award of a firm fixed price contract, utilizing FAR Part 12 Simplified Acquisition Procedures, to a single offeror who is deemed responsive and responsible in accordance with the Federal Acquisition Regulation (FAR) 9.104-1, whose offer conforms to the RFQ requirements and is determined to be the best value to the Government. Submit complete quotation response to tamara.feist-hatfield@va.gov NLT Tuesday, April 28, 2026 by 1PM MT.
Offerors are cautioned to follow the detailed instructions fully and carefully, as the Government reserves the right to make an award based on initial offers received, without discussions of such offers
1. Required Registration: Offeror’s must be registered in the System for Award Management (SAM). No contract will be entered into with an unregistered Contractor. Internet access allows you to register by completing an electronic on-line registration application at https://sam.gov/.
2. Service Disable Veteran Owned Small Business (SDVOSB) Set Aside: This solicitation is 100% set-aside for SDVOSB concerns. To be considered for award prospective offerors must be a verified SDVOSB-concern with the Veteran Small Business Certification at https://veterans.certify.sba.gov/ at the date and time set for receipt of offers.
3. Period for acceptance of quotes: The offeror agrees to hold the prices in its quote firm for 30 calendar days from the date specified for receipt of quotes, unless another time period is specified in an addendum to the solicitation.
4. Site Visit: A site visit is scheduled for Tuesday, April 21, 2026 at 10AM MT at the VBA Boise Regional Office located at 444 W. Fort St, Boise, ID 83702. Contractors are encouraged to attend to inspect the facilities. The Government does not assume any cost involved with the site visit. RSVP with contractor information and names of attendees to tamara.feist-hatfield@va.gov NLT Monday, April 20, 2026 at 1PM MT. Attendees will check in with Security at the main entrance and wait for Government escort.
5. Questions: Questions regarding this RFQ or site visit shall be submitted via electronic mail to the Contracting Officer (tamara.feist-hatfield@va.gov). Questions shall be accepted through Thursday, April 23, 2026, 1PM MT. The Government is not obligated to respond to any questions received after that date and time. All questions received within the designated timeframe that the Government considers relevant, will be answered. Responses to questions will be provided as an amendment to the RFQ that will be made available to all potential offeror’s. Telephonic inquires will not be responded to.
6. Oral Statements: No oral statements made by VA employees, or other interested parties, other than the Contracting Officer will take precedence over the written terms and conditions of the solicitation or resultant contract.
7. Amendments: All offerors are responsible for monitoring and downloading any amendments posted on Contract Opportunities. The offeror shall acknowledge all future amendments to this solicitation, if applicable, by completing the ACKNOWLEDGMENT OF AMENDMENTS under Contract Administration Data or including signed copies of the individual amendments or provide signed copies of amendment documents.
QUOTE PREPARATION INSTRUCTIONS:
Contractors responses to the solicitation shall contain the following documentation and forwarded to tamara.feist-hatfield@va.gov NLT than the closing date and time specified.
Part I - Executed Request for Quote:
· Complete all sections within blocks 13 through 16 on the SF18 and indicate the Contractor Unique Entity ID (UEI) and CAGE code within block 11b.
· Provide signed copy of 852.219-75 VA Notice of Limitations on Subcontracting – Certificate of Compliance for Services and Constructions.
· Complete the Hazardous Material Identification and Safety Data (52.223-3) clause included within the solicitation.
· Complete the applicable Buy American Certificate – Construction Materials (52.225-9) included within the solicitation. Offerors leaving the certificate blank are representing to the Government that all materials and supplies are exclusively domestic end products under the Buy American Act.
Part II – Price:
· Insert proposed unit and extended prices in all Contract Line Item Numbers (CLINs). All prices shall be represented in two decimal positions only. Example: $0.27, not $0.27458.
· Offeror shall indicate the value of the contract performed by the prime contractor and the contract value of the subcontractor(s) performing this requirement, as indicated on the Price Schedule in Section A.4.
Part III – Past Performance:
· Submit no more than a total of three (3) Past Performance Questionnaires (PPQs) from contracts/purchase orders/task orders performed within the last three years. To be considered, the PPQs must be completed by the EVALUATOR from their previous contracts purchase orders/task orders and emailed directly by the Evaluator to the Contracting Officer for receipt no later than the time and date quotes are due. All Questionnaires received after the quote due date and time will not be considered. PPQs submitted directly by offeror will not be considered. The Government may contact the Evaluators for any clarifications.
It is the offeror’s responsibility to submit PPQs to the Evaluator from their previous contracts and to ensure these PPQs are emailed to tamara.feist-hatfield@va.gov by the quote due date listed in the solicitation.
Past Performance may be evaluated based on Past Performance Questionnaires, Past Performance Information Retrieval System (PPIRS), Federal Awardee Performance and Integrity Information System (FAPIIS), Electronic Subcontract Reporting System (eSRS), or other databases and sources. All past performance information shall be evaluated for relevancy and contractor performance. If no past performance information is readily available, the Quoter may not be evaluated favorably or unfavorably on past performance.
In evaluating past performance, the Government reserves the right to give greater consideration to information deemed most relevant to the effort described in this solicitation.
