About this file
This is a Vendor's Statement of Compliance form pursuant to Kentucky Revised Statutes (KRS) 45A.343, required by the City of Owensboro, Kentucky for contract eligibility. The document serves as a certification that vendor applicants have not violated specified Kentucky statutes within the five-year period preceding contract submission and commits vendors to ongoing compliance throughout the contract term. The form applies to the Owensboro Police Department Uniforms contract, which seeks a qualified company with a minimum of five years in business to provide uniforms, shoes, leather goods, and associated equipment to the police department. All items will be awarded to a single successful bidder, with all equivalents or alternates requiring approval by the Owensboro Police Chief or designee and the Purchasing Manager prior to bid opening. The submission deadline for substitute item requests is 5:00 PM Central Time on March 27, 2026.
The vendor compliance statement requires certification regarding adherence to Kentucky statutes covering corporate taxes (KRS 136), sales and use taxes (KRS 139), income taxes (KRS 141), wage and hour requirements (KRS 337), occupational safety (KRS 338), unemployment insurance (KRS 341), and workers compensation (KRS 342). Non-disclosure or non-compliance with these requirements provides grounds for contract cancellation and disqualification from future City of Owensboro contract awards for a period of two years. The form requires signature by a duly authorized company officer and must be completed with specific dates and company identification information before submission to the City.
