About this file
This is a sworn statement form required pursuant to Kentucky Revised Statute 45A.395 for bidders and offerors seeking state and local contract opportunities in Kentucky. The document serves as a prerequisite certification for establishing bidder responsibility and must be completed by an authorized representative of the bidding organization. The statement requires the bidder to affirm under penalty of perjury that neither the signatory nor anyone acting on behalf of the bidder has knowingly violated any provision of Kentucky's campaign finance laws, and that contract award will not result in any campaign finance law violations. The form includes standard certification language defining "knowingly" as awareness or reasonable awareness that conduct or circumstances are of a particular nature, and provides blank fields for the company name, authorized signatory, title, date, and month/year of execution.
The related contract opportunity referenced is for the City of Owensboro Police Department seeking a qualified vendor with a minimum five-year business history to provide uniforms, shoes, leather goods, and associated equipment. All items will be awarded to a single successful bidder, with any substitutions or equivalents requiring prior approval from the Police Chief or designee and the Purchasing Manager before the bid opening. The submission deadline is 5:00 PM Central Time on March 27, 2026. No specific pricing, set-asides, or funding sources are detailed in the statement form itself, though Kentucky campaign finance compliance and responsible bidder determinations are mandatory prerequisites to contract award.
