About this file
This is a Vendor's Statement of Compliance form required by the City of Owensboro, Kentucky, pursuant to Kentucky Revised Statutes (KRS) 45A.343, applicable to the RiverPark Parking Garage Elevator Modernization project. The form serves as a prerequisite certification that prospective contractors must complete and submit as part of the bidding process. Vendors are required to attest that they have not been found in violation of specified Kentucky statutes within the five-year period preceding submission and acknowledge their obligation to maintain compliance throughout the contract term. The statement addresses compliance requirements under KRS Chapters 136 (corporate taxes), 139 (sales and use taxes), 141 (income taxes), 337 (wage and hour), 338 (occupational safety), 341 (unemployment), and 342 (workers compensation).
The form requires authorized company officers to certify compliance status and acknowledge the consequences of nondisclosure or noncompliance. Vendors must recognize that failure to disclose violations or maintain compliance during contract performance will authorize the City of Owensboro to cancel the contract and disqualify the vendor from receiving city contracts for a two-year period. This certification document contains blank fields for the contractor to insert their company name, authorized representative information, and the execution date, and serves as a binding attestation of regulatory compliance as a condition of contract eligibility for the elevator modernization project.
