About this file

This is a sworn statement form required pursuant to Kentucky Revised Statute (KRS) 45A.395 for bidders and offerors on the City of Owensboro's RiverPark Parking Garage Elevator Modernization project in Kentucky. The statement serves as a prerequisite for determining bidder responsibility and must be completed and submitted by any contractor seeking to bid on this elevator modernization contract. The form requires the bidder or offeror to certify under penalty of perjury that neither they nor, to the best of their knowledge, anyone acting on their behalf has knowingly violated any provision of Kentucky's campaign finance laws and that contract award will not violate such laws.

The statement must be signed by an authorized representative of the bidding company and dated, with the company name, signatory name, and title clearly identified. The form defines "knowingly" as awareness or the reasonable should-have-been-aware standard regarding conduct or circumstances described in statutes defining offenses. This certification is a mandatory compliance requirement for all bidders on this Kentucky state and local contract opportunity and must be completed as part of the bid submission process.
