About this file

This is a Certification Regarding Debarment, Suspension, and Other Responsibility Matters form issued by the Incorporated County of Los Alamos Procurement Division in New Mexico for the SMART Recovery for Juveniles contract opportunity. The form requires offerors and bidders to certify their eligibility and responsibility status as a condition of proposal submission and contract award consideration. Offerors must confirm that neither they nor their principals are currently debarred, suspended, proposed for debarment, or excluded from covered transactions by any federal, state, or local agency. The certification covers a three-year lookback period and requires disclosure of any convictions, civil judgments, indictments, or charges related to fraud, criminal offenses, antitrust violations, embezzlement, theft, forgery, bribery, or falsification of records. Additionally, offerors must certify they have not violated contract provisions, experienced preventable failure to perform, breached ethical standards under the County Procurement Code, or had public contracts terminated for cause or default within the three-year period.

The form also establishes employment and family relationship restrictions, prohibiting current or former County employees and immediate family members of County employees or public officials from serving as offerors. Offerors unable to certify any statement in the form must provide written explanation as an attachment. The form specifies that falsification of any statement constitutes grounds for rejection of the proposal, non-consideration for award, or rescission of a contract award. An authorized signer must execute the certification with their name, title, date, and signature, confirming the statements made to the best of their knowledge and belief.
