About this file

This is a Drug-Free Workplace Affidavit form that serves as a certification document for firms responding to state and local contract opportunities. The document requires the undersigned to certify that their firm has implemented comprehensive drug-free workplace policies and procedures as a condition of bidding on commodities or contractual services.

The affidavit outlines six mandatory requirements that responding firms must meet, including publishing workplace drug prohibition statements, educating employees about drug abuse dangers and available assistance programs, distributing drug-free workplace policies to all employees engaged in the proposed work, establishing employee notification procedures for drug-related convictions within five days, implementing sanctions or rehabilitation requirements for convicted employees, and maintaining ongoing good faith efforts to sustain a drug-free work environment. This certification appears to be a standard compliance requirement for government contracting opportunities, ensuring that all potential vendors maintain appropriate workplace safety and legal standards.
