About this file

This document is a historical record of medical assistance cases requiring Medevac services provided under a NASA contract during the period of 2011 through June 30, 2026. The contractor maintained records of medical incidents occurring during contractual travel, categorizing them by severity level and documenting whether commercial flight transport back to the United States was necessary. The severity classification system comprises four levels: Level 1 for minor incidents, Level 2 for more involved incidents requiring professional hospital or clinic care but treatable locally, Level 3 for incidents requiring surgery or commercial flight to the US, and Level 4 for air ambulance transport during critical emergencies.

From 2014 through 2024, the contractor responded to 13 documented medical cases across Japan, Italy, Germany, Mexico, and Russia. Nine cases were classified as Level 1 (minor incidents such as esophageal obstruction, eye floaters, dermatologic issues, potential bat bite, and COVID-positive notification), requiring only advisory services or local care without transport. Two Level 2 cases involved foot infection and gastrointestinal infection, both managed through local hospital care with language assistance and medical consultations. Four Level 3 cases required emergency surgical intervention or commercial flight home, including appendicitis cases in Russia (2017 and 2022), a fractured arm and abdominal pain case in Japan (2018), knee surgery in Russia (2019), and a leg injury in Germany (2024), with the contractor providing translation assistance, bilingual drivers, and commercial flight arrangements as contractually triggered.

Actual Cases of Medical Assistance Needed

The following list is a running total of medical cases during the contractual period of 2011 - June 30, 2026. In these cases, the majority did not need any transports. In the incidents where commercial transport was needed, NASA and the contractor agreed that the medical condition warranted a commercial flight home based on the triggers defined in the contract.

Year Severity Country Description 2014 Level 1 Japan Esophageal Obstruction. No specific care needed. No flight back from contractor.

2014 Level 1 Italy Vitreous Detachment. Advisory services. No flight back from contractor.

2016 Level 1 Germany Ophthalmologist care for eye floaters needed advisory services.

Local care needed, no flight back from contractor.

2016 Level 1 Mexico Dermatologic care needed for skin issue. Local care needed, no flight back from contractor.

2018 Level 1 Russia Potential bat bite case in hotel room while two people were sleeping. Local care involved rabies vaccine while in Russia. No flight back from contractor.

2019 Level 3 Russia Knee Surgery needed. Contractor flew the patient back commercially.

2022 Level 3 Russia Appendicitis case required emergency surgery locally. Contractor provided GOP services before the necessary surgical intervention.

• Whenever a medical issue happens to someone on travel requiring Medevac services, a record of when the issue happened and severity level is kept.

– Severity type can provide an idea of how complex treatment may be and are as follows:

• Level 1: Minor incidents

• Level 2: More involved incidents that require professional medical care in a hospital/clinic but can be treated locally without transport to the US

• Level 3: Incidents requiring surgery or commercial flight to the US

• Level 4: Air ambulance transport for critical emergencies
