SECTION 01010 – SUMMARY OF WORK
NOTE: The Davis-Monthan Air Force Base Design Compatibility Guidelines (DMDCG) requirements and those defined in the following section may directly affect the Contractor’s overhead for completing the scope of work. Failure on the part of the Contractor to adequately review this section and identify/clarify any requirements herein will not constitute justification of Contractor claims for additional design and/or construction costs.
1. SUMMARY OF WORK
1.1 Work Included in this Contract: The following descriptions are for general reference only, and shall not be construed as a complete list of all operations to be performed. Work is expected to be accomplished under the issuance of multiple separate and distinct task orders throughout the contract ordering period. Perform all operations associated with street and parking lot repair to include demolition, utility work, subgrade preparation, concrete curb, base course, asphalt surface, sidewalk, striping, drainage, etc. per the following item descriptions:
0001AA – Mobilization and De-mobilization (Light): Transport of personnel and equipment to and from the job site to accomplish all procedures necessary to perform the construction and/or repair of a pavement facility, e.g. street, parking lot, etc. Included is up to a 25 mile haul distance (50 mile round trip for mob/demob) for one piece of light (up to 3 ton) equipment (i.e. sweeper, bobcat, etc.) hauled on a 3-ton capacity towed trailer. A quantity of one includes mobilization and de-mobilization of one piece of equipment.
0001AB – Mobilization and De-mobilization (Medium): Transport of personnel and equipment to and from the job site to accomplish all procedures necessary to perform the construction and/or repair of a pavement facility, e.g. street, parking lot, etc. Included is up to a 25 mile haul distance (50 mile round trip for mob/demob) for one piece of medium (more than 3 ton and up to 20 ton) equipment (i.e.
backhoe, loader, grader, etc.) hauled on a 20-ton capacity towed trailer. A quantity of one includes mobilization and de-mobilization of one piece of equipment.
0001AC – Mobilization and De-mobilization (Heavy): Transport of personnel and equipment to and from the job site to accomplish all procedures necessary to perform the construction and/or repair of a pavement facility, e.g. street, parking lot, etc. Included is up to a 25 mile haul distance (50 mile round trip for mob/demob) for one piece of heavy (more than 40 ton and up to 40 ton) equipment (i.e. roller, scraper, cold plane miller, etc.) hauled on a 40-ton capacity towed trailer. A quantity of one includes mobilization and de-mobilization of one piece of equipment.
0001AD – Mobilization and De-mobilization (Truck Mounted Crane): Transport of personnel and equipment to and from the job site to accomplish all procedures necessary to perform the construction and/or repair of a pavement facility, e.g. street, parking lot, etc. Included is up to a 25 mile haul distance (50 mile round trip for mob/demob) for one truck-mounted crane up to a 75-ton capacity. A quantity of one includes mobilization and de-mobilization of one piece of equipment.
0001AE – Cold Mill Asphalt Pavement: Cold plane asphalt pavement from 1-inch to 3-inches in thickness. Clean the milled surface of loose debris and load into trucks for removal from site. Typical task order will be no more than 10,000 square yards. Work includes loading of milled asphalt into trucks but does not include removal from site (hauling) nor disposal. Work will be accomplished in accordance with Section 02070 of the Contract Specifications.
0001AF – Remove Asphalt Pavement up to 3-inches thick: Removal of asphaltic concrete pavement by means other than cold plane milling. Work includes loading of removed asphalt into trucks but does not include removal from site (hauling) nor disposal.
0001AG – Pulverize 4-inches Asphalt Pavement, Stabilize to 6-inches in-place: Pulverize asphalt roadways and towpaths to a depth of 4-inches in place and blend with existing base. Typical task order will be no more than 15,000 square yards. Provide Portland cement delivered and spread evenly over pulverized area at a rate of 6 percent by volume and mix evenly to a depth of 6-inches providing cement soil stabilization.

0001AH – Pulverize 8-inches Asphalt Pavement, Stabilize to 12-inches in-place: Pulverize asphalt roadways and towpaths to a depth of 8-inches in place and blend with existing base. Typical task order will be no more than 15,000 square yards. Provide Portland cement delivered and spread evenly over pulverized area at a rate of 6 percent by volume and mix evenly to a depth of 12-inches providing cement soil stabilization.
0001AI and 0001AJ – Sawcut Concrete 4"- 6" Thick: Wet saw concrete pavement and/or curb to a depth of 6-inches. Remove residue from adjacent surfaces by water jet and/or vacuum sweeping or other suitable means.
0001AK and 0001AL – Sawcut Asphalt to 4" Thick: Wet saw asphalt pavement to a depth of 4-inches. Remove residue from adjacent surfaces by water jet and/or vacuum sweeping or other suitable means.
0001AM – Remove Concrete Sidewalk, Haul and Dispose off-base: Assumes sidewalk thickness of 4-inches. Includes loading, hauling and dumping fees.
0001AN – Remove Concrete Curb/Gutter, Haul and Dispose off-base: Assumes concrete curb and gutter section in accordance with the DMDCG. Includes loading, hauling and dumping fees.
0001AO – Remove Concrete Slabs (6-10" thick), Haul and Dispose off-base: Assumes non-reinforced concrete. Includes loading, hauling and dumping fees.
0001AP – Excavation: Removal of existing earthen material to include, but not limited to, subgrade and aggregate base material. Work includes removal and loading into trucks but does not include removal from site (hauling) nor disposal. Work will be accomplished in accordance with Section 02202 of the Contract Specifications.
0001AQ – Hauling - on base (4 mile round trip): Hauling of loaded materials, typically milled asphalt, to an on-base location as identified by the Contracting Officer.
0001AR – Hauling - off-base: Hauling of loaded materials to a legal disposal site up to 30 mile round trip. Includes dumping fees assuming typical pavement waste materials. Provide disposal manifests as a submittal item in accordance with paragraph 1.2 of this section of the Contract Specifications.
0001AS – Embankment: Placement and compaction of earthen fill material, to include borrowed stockpile, on-site excavated or imported fill. Does not include cost of material or delivery. Work will be accomplished in accordance with Section 02202 of the Contract Specifications.
0001AT – Clean Fill: Material delivered on-site meeting the requirements of Section 02202 of the Contract Specifications.
0001AU – Surveying - 1 Man Crew: Provide surveying to include, but not limited to, topographic survey, construction staking and boundary survey utilizing a single man crew.
0001AV – Surveying - 2 Man Crew: Provide surveying to include, but not limited to, topographic survey, construction staking and boundary survey utilizing a two man crew.
0001AW – Surface Preparation: Scarify to a minimum 6-inch depth, compaction and grading in preparation for surface to receive subsequent layer of materials. Work will be accomplished in accordance with Section 02202 of the Contract Specifications.
0001AX – Concrete Curb & Gutter: Construct in-place concrete curb and gutter. Work will be accomplished in accordance with Section 02621 of the Contract Specifications and Detail DM-C003.
0001AY – Concrete Vertical Curb: Construct in-place concrete vertical curb. Work will be accomplished in accordance with Section 02621 of the Contract Specifications and Detail DM-C003.
0001AZ and 0001BA – Concrete Sidewalk: Construct in-place concrete sidewalk to a width of 5-feet and a thickness of 4-inches. Work will be accomplished in accordance with Section 02620 of the Contract Specifications.

0001BB – Concrete Accessibility Ramp at 6-inch Curb Reveal: Construct in-place concrete accessibility ramp with a curb reveal height of 6-inches. Work will be accomplished in accordance with Section 02620 of the Contract Specifications and Detail DM-C25A.
0001BC – Concrete Accessibility Ramp at 8-inch Curb Reveal: Construct in-place concrete accessibility ramp with a curb reveal height of 8-inches. Work will be accomplished in accordance with Section 02620 of the Contract Specifications and Detail DM-C25B.
0001BD – Aggregate Base Course: Material delivered on-site, placed and compacted in preparation for surface to receive subsequent layer of materials. Work will be accomplished in accordance with Section 02220 of the Contract Specifications.
0001BE and 0001BF – Bituminous Prime Coat: Material delivered on-site and applied to the prepared surface. Work will be accomplished in accordance with Section 02601 of the Contract Specifications.
0001BG and 0001BH – Bituminous Tack Coat: Material delivered on-site and applied to the prepared surface. Work will be accomplished in accordance with Section 02602 of the Contract Specifications.
0001BI – Bituminous Surface Course (2"): Material delivered on-site, placed and compacted. Work will be accomplished in accordance with Section 02612 of the Contract Specifications.
0001BJ – Bituminous Surface Course (3"- placed in 2 lifts): Material delivered on-site, placed and compacted. Work will be accomplished in accordance with Section 02612 of the Contract Specifications.
0001BK – Bituminous Surface Course (3"- placed in 1 lift): Material delivered on-site, placed and compacted. Work will be accomplished in accordance with Section 02612 of the Contract Specifications.
0001BL – Adjust Manhole Frame and Cover to Grade: Provide vertical adjustment of manhole frame and cover to the final grade of the surrounding pavement or adjacent grade as indicated on the task order documents. Place a concrete collar around the manhole frame to a depth of 6-inches.
0001BM – Adjust Valve Riser, Frame and Cover to Grade: Provide vertical adjustment of water or natural gas valve riser pipe, frame and cover to the final grade of the surrounding pavement or adjacent grade as indicated on the task order documents. Place a concrete collar around the manhole frame to a depth of 6-inches.
0001BN – Crack Seal Asphalt Pavement: Material delivered on-site and applied to the prepared surface. Work will be accomplished in accordance with Section 02616 of the Contract Specifications.
0001BO – Fog Seal Asphalt Pavement: Material delivered on-site and applied to the prepared surface. Work will be accomplished in accordance with Section 02507 of the Contract Specifications.
0001BP – Painted Reflective Pavement Markings: Material delivered on-site and applied to the prepared surface. Work will be accomplished in accordance with Section 02580 of the Contract Specifications.
0001BQ – Painted Non-Reflective Pavement Markings: Material delivered on-site and applied to the prepared surface. Work will be accomplished in accordance with Section 02580 of the Contract Specifications.
0001BR – Thermoplastic Tape Striping (4" wide): Material delivered on-site and applied to the prepared surface. Work will be accomplished in accordance with Section 02580 of the Contract Specifications.
0001BS – Traffic Sign Installation: Material delivered on-site and installed per the most current edition of the Uniform Manual for Traffic Control Devices. Sign shall be 30-inch by 30-inch retro-reflective regulatory or warning sign mounted on a 1-3/4-inch square perforated steel post up to 12-feet high. Post shall be breakaway mounted in a 2-inch square steel post base set in 2,500 psi concrete. Back of sign, post and post base shall be painted in accordance with the DMDCG.

0001BT – Raised Reflective Pavement Markers: Material for and installation of raised reflective traffic lane pavement markers. Marker colors shall be provided in accordance the most current edition of the Manual on Uniform Traffic Control Devices.
0001BU – Curb Mounted Reserved Parking Sign: Material for and installation of 3-inch high by 36-inch long aluminum strip sign, brown background with white lettering. Sign shall be screw mounted to face of concrete curb as specified in the DMDCG.
0001BV – Concrete Drainage Structures: Material delivered on-site and construction of concrete drainage structures, to include but not limited to, sidewalk drainage scuppers, culvert headwalls, etc.
Structures shall range in height from 6-inches to 4-feet and work shall include, but is not limited to, site preparation, form construction/placement, reinforcing steel as required, and concrete placement and finishing, in order to provide a finished product.
0001BW – 6-Inch Thick Concrete Slab (3000 psi): Material delivered on-site and construction of 6-inch thick non-reinforced 3000 psi concrete slab on grade, typically for the purpose of motorcycle parking area. Work shall include, but is not limited to, site preparation, form construction/placement, and concrete placement and finishing, in order to provide a finished product.
0001BX – 8-Inch Thick Concrete Slab (4000 psi): Material delivered on-site and construction of 8-inch thick non-reinforced 4000 psi concrete slab on grade, typically for the purpose of motorcycle parking area. Work shall include, but is not limited to, site preparation, form construction/placement, and concrete placement and finishing, in order to provide a finished product.
0001BY – 6-Inch Diameter Steel Bollards (Concrete Filled): Material delivered on-site and installation of concrete filled 6-inch diameter steel post bollard with concrete base. Bollard shall meet the requirements of Safety Bollards as described in the DMDCG.
0001BZ – 18-Inch Diameter CMP Culvert: Materials and installation of an 18-inch diameter corrugated metal pipe culvert, to include trenching, bedding and backfilling. Work will be accomplished in accordance with Section 02223 of the Contract Specifications.
0001CA – 12-Inch Diameter RCP Culvert: Materials and installation of a 12-inch diameter reinforced concrete pipe culvert, to include trenching, bedding and backfilling. Work will be accomplished in accordance with Section 02223 of the Contract Specifications.
0001CB – Hand Digging: Excavation by hand with shovel in medium soil by common labor.
0001CC – Rock, Rip-rap: Nominal 6-inch random, broken stone, dumped in place over filter-fabric.
Filter fabric shall be anchored/secured at edges with no loose edges of fabric protruding from stone.
No more than one side of stone shall be rounded.
0001CD – Decomposed Granite Ground Cover: Crushed decomposed granite ground cover placed in a ½-inch thick layer on prepared surface. Use locally produced decomposed granite or approved equal.
0001CE – Concrete Testing: Concrete quality control testing performed in accordance with Section 02620 and Section 02621 of the Contract Specifications. Each unit shall consist of a slump test performed the day of concrete placement and a compression test performed at 7 days and at 28 days.
0001CF – Aggregate Base Material Sieve Analysis: AB quality control testing performed in accordance with Section 02220 of the Contract Specifications. Each unit shall consist of one laboratory performed sieve analysis of the AB material to be used.
0001CG – Subgrade/Aggregate Base In-Place Density Test: Quality control testing performed in accordance with Section 02202/Section 02220 of the Contract Specifications. Each unit shall consist of one laboratory performed proctor test and two field density tests as prescribed in the specifications.
0001CH and 0001CI – Asphaltic Concrete In-Place Density & Thickness Test: Quality control testing performed in accordance with Section 02612 of the Contract Specifications. Each unit shall consist of two samples and two cores taken from each day’s production.

0001CJ – Traffic Control: Provide signs, barricades and/or flagmen as necessary to control traffic in the construction area. Each unit shall consist of one day in place of four Type III barricades with an R11-2 Road Closed sign mounted to barricade plus flash units and four sandbags each and ten Type II barricades with flash unit and one sandbag each. Each unit shall also include delivery, setup, maintenance and pickup of signs and barricades. Multiple units per day may be necessary for large areas of construction.
0001CK – 10-Inch to 12-Inch Thick Concrete Slab (650 psi): Material delivered on-site and placement of concrete slab infill. Slab thickness will vary from 10-inches to 12-inches. Specified flexural strength of concrete shall be 650 psi at 90 days and concrete shall be non-reinforced slab on grade, typically for the purpose of selective airfield concrete pavement repair by replacement. Work will be accomplished in accordance with Section 02622 of the Contract Specifications.
0001CL – 1-Inch by 20-Inch Dowel Bars: Material delivered on-site and installed into edge of existing concrete slab. Includes drilling holes in edge of concrete slab, preparation of dowels prior to installation and placement of dowels into prepared holes using an epoxy adhesive. Work will be accomplished in accordance with Section 02622 of the Contract Specifications.
0001CM – Concrete Spall Repair: Remove spalled edges from concrete pavements by sawcutting and replace with spall patching material. Work will be accomplished in accordance with Section 02623 of the Contract Specifications.
0001CN – Reseal Concrete Pavement Joints: Remove existing joint sealant material in concrete pavement control joints, clean joint reservoir and apply new joint sealant material. Work will be accomplished in accordance with Section 02624 of the Contract Specifications.
0001CO – Concrete Pavement Crack Repair: Sawcut or rout developed cracks in existing concrete pavement. Clean debris from prepared crack and fill crack with an approved epoxy crack sealant material. Provide backer rod as necessary prior to sealant application.
0001CP – Airfield Pavement Paint Removal: Remove paint buildup from asphalt and concrete airfield surfaces without damage to the pavement surface using high-pressure water jets. The pavement surfaces may be smooth or grooved. Work will be accomplished in accordance with Section 02590 of the Contract Specifications.
0001CQ – Airfield Pavement Reflective Paint Application: Material delivered on-site and applied to the prepared surface. Paint colors will include yellow, red, white and black as specified in the individual task order. Work will be accomplished in accordance with Section 02580 of the Contract Specifications.
0001CR – Airfield Pavement Non-Reflective Paint Application: Material delivered on-site and applied to the prepared surface. Paint colors will include yellow, red, white and black as specified in the individual task order. Work will be accomplished in accordance with Section 02580 of the Contract Specifications.
0001CS – Airfield Pavement Restricted Area Warning Sign Paint Application: Material delivered on-site and applied to the prepared surface. Warning sign consists of red and black lettering applied to a white background, 18-inches by 15-inches, in accordance with AFVA 31-107. Warning sign painting stencils will be provided by the Government for use by the Contractor. At the end of use during any applicable task order, the Contractor shall return the stencils to the Government in a clean and undamaged condition. Any stencil determined by the Contracting Officer to be unusable subsequent to use by the Contractor shall be replaced at the Contractor’s expense. Work will be accomplished in accordance with Section 02580 of the Contract Specifications.
0001CT – Airfield Pavement Entry Control Point (ECP) Paint Application: Material delivered on-site and applied to the prepared surface. ECP consists of black lettering applied to a white background.
The white background will be 24-inches tall by 60-feet in length. ECP wording is “ENTRY CONTROL POINT” in lettering that is 18-inches high, 12-inches wide. Letter stroke will be 3.25-inches wide.
See the sample lettering detail attachment to the Contract Specifications. Work will be accomplished in accordance with Section 02580 of the Contract Specifications.

0001CU – Supervision: Superintendent on-site during all periods of work, responsible for making decisions and directing work. Includes superintendent’s pickup truck.
1.2 Salvage and Disposal: All salvageable and non-salvageable material obtained by removal of existing material shall be considered as having been included as a factor in the Contractor's bid price.
a. Salvageable material obtained from existing construction may be reused only if so stated elsewhere in the Contract Documents, and then only when the Contracting Officer approves the material for reuse.
b. Salvageable material shall remain the property of the Government only when specifically called out in the Contract Documents.
c. Salvageable material not reused or otherwise designated to remain the property of the Government shall become the property of the Contractor and shall be removed and disposed of off base in accordance with the environmental controls specified in this Section.
d. Non-salvageable material: Remove all non-salvageable material from the project site and dispose of off base, in accordance with the environmental controls specified in this Section.
1.3 Base Outages & Digging Permits (AF Form 103, BCE Work Clearance Request):
a. Utility Outages: Accomplish all work required in connection with utility outages on weekends or off-duty hours, or as directed by the Contracting Officer. The Contractor shall be responsible for all work associated with the utility outages. Notify the Contracting Officer in writing no less than 20 calendar days prior to the desired date for the utility outage.
b. Road Closures: If road closures are needed during the construction of any portion of the project, a traffic control plan shall be generated by the Contractor in accordance with the traffic regulations specified in this Section. Notify the Contracting Officer in writing no less than 20 calendar days prior to the desired date of closure.
c. Work Clearances: Prior to performing any work that involves digging, the Contractor shall obtain an approved BCE Work Clearance Request (AF Form 103) through the Contracting Officer. The Contractor shall notify the Contracting Officer in writing no less than 24 calendar days prior to the desired date of digging. The written request shall include a site drawing that indicates the area of clearance requested. The Contractor shall provide a physical boundary marking of all areas they wish to be included within the approved work clearance area. Such boundary marking shall be made with white paint PRIOR to the request submission. The Contractor is also responsible for contacting Arizona 811 (Blue Stake) for clearance and marking of any utilities that are not owned and maintained by DMAFB.
The Contractor is responsible for independently marking and potholing for security camera conduit/wiring as well as vacuum and oxygen medical lines located around the 355 Medical Group Buildings that may lie within their area of work. The 355 CES as-built drawing files may include drawings for security camera conduit/wiring and medical lines, however it is the Contractor’s responsibility to physically locate and mark the location of these utilities. Subsequent to the Government providing underground utility markings, it is the Contractor’s responsibility to maintain the markings with stakes, paint, whiskers, etc.
1.4 Submittal Procedures:
a. AF Form 66, Schedule of Material Submittals: An AF Form 66, listing the items of equipment and materials for which submittals are required by the specifications will be provided to the Contractor with each Task Order (TO) proposal request. The submittal register will become the scheduling document for all submittal items required for that particular TO and will be used to control submittals throughout that Task Order. During construction, it may be discovered that this list was not all-inclusive and that additional submittals may be required from and shall be submitted by the Contractor, as directed by the Contracting Officer.
b. AF Form 3000, Material Approval Submittal:
1. The AF Form 3000 shall be used for submitting both Government-approved and information-only submittals in accordance with the instructions on the reverse side of the form. The

Contracting Officer shall furnish these forms to the Contractor. This form shall be properly completed by filling out the heading blanks and identifying each item submitted. Special care shall be exercised to ensure proper listing of the specification paragraph and/or sheet number of the contract drawings pertinent to the information submitted for each item(s).
2. The Contractor shall forward all material submittals required by the technical specifications using the AF Form 3000 as stated in paragraph b1 above. The Contractor provides the submittal items in the number of copies as specified on the AF Form 66 with one original of the completed AF Form 3000 to the Contracting Officer no later than the required submission date specified on the AF Form 66 or 10 calendar days from the Notice to Proceed, whichever is later.
3. Identify each submittal by project, Contractor, drawing or detail number, and specification section number, as appropriate. Mark submittals to show the specific item(s) that will be furnished.
Include intended use of the submittal and any other pertinent information necessary for a complete evaluation. If the Contractor provides a submittal which show variations from the contract documents, the Contractor shall completely describe such variations in writing clearly and separately from any other portion of the submittal. Highlighting or marking in any manner the proposed variations is not sufficient to comply with this requirement. This requirement applies irrespective of the provisions of any specification which follow this section and which may be particular to specific item(s).
c. Submit to the Contracting Officer complete information applicable to the item(s) of equipment and materials called for by the Contract Documents, for approval, prior to installing the relevant item(s) of equipment and material. If unapproved item(s) of equipment or material are installed, the Contracting Officer, at his sole discretion and option, may reject the installed equipment and material. If rejected, the Contracting Officer may then pursue, again in his sole discretion and option, remedies including but not limited to the removal and reinstallation of approved item(s) of equipment or material at the Contractor’s expense, extended warranties, and other remedies as may be deemed appropriate and applicable to that portion of the construction.
d. Submittals covering component items forming a system or items that are interrelated shall be scheduled to be coordinated and submitted concurrently. Certifications to be submitted with the pertinent drawings shall be so scheduled.
e. Adequate time (a minimum of 14 calendar days) shall be allowed and shown on the register for distribution, review and approval by both or either the Project Inspector and Contracting Officer.
f. The Contractor, as part of the bid price, shall have a Government-approved, independent, commercial, testing laboratory perform all sampling and testing as called for in these specifications. Test reports shall be properly sealed by a professional engineer.
1.5 Quality Control:
a. Provide quality control over the entire project including but not limited to the items of equipment and materials intended for installation, necessary coordination and services to facilitate the performance of the work, site conditions, and the workmanship of the Contractor personnel and any subcontractors.
b. The Contractor shall become familiar with all the details of the work depicted in the Contract Documents and shall verify all field conditions related to the construction prior to commencement of the work. If conditions are discovered by the Contractor prior to commencement of the work which conflict with the Contract Documents, and which will substantially affect the construction as designed and depicted in the Contract Documents, identify and explain the condition(s) in writing to the Contracting Officer within 48 hours of discovery of the condition(s); and request clarification from the Contracting Officer before commencing work.
c. If site conditions are discovered by the Contractor during the construction which conflict with the Contract Documents, and which affect either a portion of the current construction, or which will substantially affect the construction as designed and depicted in the Contract Documents, identify and explain the condition(s) in writing to the Contracting Officer within 48 hours of discovery of the condition(s); and request clarification from the Contracting Officer before proceeding. All such requests for information shall completely address aspects of the condition, including but not limited to, engineering judgments and engineered solutions, possible costs or deductions, and time extensions.

d. If site conditions or defects are discovered during construction, by the Contracting Officer or the Contracting Officer’s designated representative, which are the result of the Contractor’s inadequate quality control or substandard workmanship, whether by the Contractor or a subcontractor, which conflict with the Contract Documents, and which affect either a portion of the current construction, or which will substantially affect the construction as designed and depicted in the Contract Documents, the Contracting Officer may direct the Contractor to identify and explain the condition(s) or defect(s) in writing to the Contracting Officer within 48 hours of discovery of the condition(s). In all such instances, the Contractor shall present no less than three viable options to correct the condition or defect, completely addressing aspects of the condition, including but not limited to, engineering judgments and engineered solutions, possible costs or deductions, and time extensions. The Contracting Officer will then review and approve an option for use by the Contractor. The Contractor shall not proceed with the work until receiving the written permission from the Contracting Officer.
e. The Contractor shall comply fully with manufacturer's instructions, including each step in sequence. If such instructions conflict with the Contract Documents, identify and explain the conflict in writing to the Contracting Officer within 48 hours of discovery of the conflict; and request clarification from the Contracting Officer before proceeding.
f. Testing: The Contractor shall provide samples and test results as required by the Contract Documents. Test reports shall be properly sealed by an independent professional engineer.
g. Cleanup: The Contractor shall keep the work area and surrounding surfaces clean and orderly.
Remove any contractor-caused residue, dirt, or markings immediately upon completion of the work involved. Clean site daily.
h. Exterior surfaces including but not limited to Portland cement concrete, asphalt pavements, and gravel and interior surfaces including but not limited to carpet, tile, ceiling tiles, and gypsum board damaged by construction work or the Contractor’s operations, shall be replaced with materials consistent in quality and workmanship to match existing.
i. It is the Contractor’s responsibility to maintain any and all quality control records and have them available to the Government at all times.
j. Record drawings shall be kept at the job site at all times, and shall be made available upon request by either or both the Project Inspector or the Contracting Officer. The record drawings shall be marked up on a daily basis to show actual “as-built” construction and conditions.
1.6 Experience Requirement:
The Contractor must have in the past been engaged in work of this type for a minimum of five years.
Contractors whom submit a proposal on this IDIQ contract must be prepared to submit notarized affidavits from Government agencies attesting to satisfactory performance of work of at least three projects of this nature during the previous five year period and to the qualifications both in terms of expertise and having proper and adequate equipment to accomplish the work. If submittal is requested by the Government, the affidavits of the successful awardee will be reviewed for approval or disapproval. The notarized affidavits will be submitted within 3 working days after requested.
1.7 Government Furnished Plans:
The contractor will be furnished two hard copies and one electronic copy of any associated project plans upon award of Task Order. After this initial issue of project plans the contractor will be responsible for reproducing any subsequent copies of drawings/plans for their use.
2. CONSTRUCTION FACILITIES AND TEMPORARY CONTROLS
2.1 Traffic Regulation:
a. Barriers: Contractor shall furnish all signs, barricades, and flagmen required to control traffic in the construction area.

1. All signs and barricades shall be in accordance with ANSI D6.1, Manual of Uniform Traffic Control Devices.
2. Minimize interference with the flow of traffic. At no time may access to any facility be prevented.
3. If required by the TO project requirements, the Contractor will prepare and submit, not less than 14 days prior to commencing construction, a proposed plan for routing traffic through or around the work site. This plan shall be included as a submittal item on the AF Form 66, Schedule of Material Submittals.
4. The Contractor will be required to maintain barricades/traffic signs on a 24 hours per day, 7 days per week basis. A representative will be available on short notice at all times to provide maintenance. The Contractor will provide specific attention to correcting signs and barricades after storms or high winds. The Contractor will coordinate removal of project barricades/signs with the Contracting Officer’s Representative under all circumstances.
b. Haul routes:
1. Only routes as designated on the contract drawings/documents or by the Contracting Officer will be utilized by the Contractor for the hauling of materials and/or equipment to and from the project site.
2. The Contractor shall immediately clean up any material spilled along the haul route. This shall include clean up of mud tracked on the road by contractor equipment.
3. For any spill of hazardous material on base, immediately notify the base fire department at
911. The Contractor is responsible for all costs incurred by the Government in responding to or cleaning up any such spill.
c. Access: Contractor shall maintain access to the work site at all times. The Contractor is to perform his work with a minimum of interference to the flow of traffic.
2.2 Safety:
a. Comply with the Corps of Engineers Manual on General Safety Requirements, EM 385-1-1;
Occupational Safety and Health Act (OSHA), Public Law 91-596; and AFI 91-301, Air Force Occupational Safety, Fire Prevention, and Health (AFOSH) Program.
b. Use of flammable adhesives or other similar flammable materials will be in strict accordance with manufacturer's instructions and the Contractor shall take all necessary safety and fire prevention steps to supervise and assure safe execution of work on the contract. This paragraph is applicable to adhesive materials or any other material involving flammable characteristics.
2.3 Blasting: No blasting will be permitted.
2.4 Grade Control: Grade site to drain protecting site from puddling or running water or accumulation of standing water in excavations. The Contractor shall provide all surveying required to establish and maintain line and grade stakes necessary to complete the construction.
2.5 Environmental Control:
a. The Contractor shall be required to meet or exceed the requirements and procedures of the most current Davis-Monthan AFB Contractor Environmental Guide in effect at the time of project contract award. A copy of this guide will be provided as part of the solicitation documents. This Guide includes procedures which if adhered to by the Contractor should ensure compliance with the Installation's environmental permits (as maintained 355 CES/CEIE) as well as other regulations to include, but not limited to, training for the ISO 14001 Environmental Management System (EMS). To receive the initial EMS Awareness Training the Contractor will be required to register and login to The Environmental Awareness Course Hub (TEACH) at https://usaf.learningbuilder.com (must use Google Chrome). The detailed registration instructions are provided in Appendix E of the DMAFB Contractor Environmental Guide.

b. Additionally, the Contractor shall be held responsible for complying with all federal, state, and local environmental regulations included as part of the processes in performing the work included in this project.
c. Hazardous material (HAZMAT) and/or regulated wastes may be encountered during construction
- see Appendix A and Attachment(s) A-1 to A-3 of the DMAFB Environmental Guide. These requirements shall be included as a submittal item on the AF Form 66, Schedule of Material Submittals. Contractor shall plan and submit a HAZMAT worksheet (Attachment A-1), including the appropriate SDS and an estimated quantity to be used. Submittals shall be in accordance with paragraph 1.4 of this specification section. The CES Project Manager will coordinate with 355 CES/CEIE for their review of the HAZMAT worksheet who will then return the HAZMAT worksheet to the CES PM along with Attachment A-2 and a determination which states if Attachment A-3 will be required. The Contractor must be in possession of a fully coordinated HAZMAT worksheet, to included Attachment A-2 and A-3 (if A-3 is required), prior to bringing any such materials onto the installation.
d. The Contractor shall obtain permits from the Pima County Department of Environmental Quality (PDEQ), Air Quality Division, for installation of new equipment or use of Contractor equipment which will generate air pollution, for earth-moving activities related to construction, and for asbestos removal or demolition. These permits shall be included as submittal items on the AF Form 66, Schedule of Material Submittals. Submit copies of permits to the Contracting Officer prior to beginning construction. All permit costs shall be considered as having been included as a factor in the Contractor's bid price.
e. The Contractor shall also submit a Construction and Demolition (C&D) Plan (Attch B-1, Contractor Environmental Guide) 3 weeks prior to starting construction. If the C&D Plan indicates potential regulated wastes or hazardous wastes, the contractor shall coordinate any disposal requirements through the Contractor’s representative prior to disposal or manifesting (see Appendix C of the DMAFB Contractor Environmental Guide. For projects involving asbestos abatement, the Contractor shall complete the standard "Notification of Demolition or Renovation" forms and submit to PDEQ and other required agencies. These forms shall be included as a submittal on the AF Form 66, Schedule of Material Submittals. Submit copies of the forms to the Contracting Officer prior to beginning construction or demolition activities. The Contracting Officer Representative will forward the plan and forms to Environmental for review.
f. Contractors shall comply with all regulatory requirements/considerations if the project requires soil disturbance. These requirements may include Storm Water Pollution Prevention Plans (SWPPP), Air Activity Permits (see para d. above), and Natural and Cultural Resource protections. Area disturbance requiring permits as set forth by Arizona Department of Environmental Quality (ADEQ) and PDEQ. The Contractor shall treat areas subject to dust producing activities with liquid palliatives which will not harm regrowth of vegetation, or other methods of dust control. Also, ensure methods to protect native plants, birds and wildlife are included in projects where required.
g. No open burning of trash, brush, or refuse will be permitted.
h. The Contractor shall be responsible for the proper management of both hazardous and non-hazardous waste and the required submittals IAW Section 3 and Appendix C of the Contractor Environmental Guide. The Contractor shall segregate hazardous or regulated materials and non-hazardous or non-regulated materials during demolition; and handle, transport, store, and dispose of these hazardous and non-hazardous materials in accordance with applicable federal, state, and local laws, ordinances, or other rulings having the effect of law, including but not limited to the items listed below. In addition, the Contractor shall obtain proper disposal manifests for both hazardous and regulated materials, and for non-hazardous or non-regulated materials, and submit these manifests to the Contracting Officer. These manifests shall be included as a submittal on the AF Form 66, Schedule of Material Submittals. . The CES Project Manager will forward all manifests to 355 CES/CEIE for their review. The Contractor will submit the original manifest (with the original signature of the Contractor’s superintendent or authorized representative in block 14 attesting to the validity of the manifest) to the Contracting Officer within 3 calendar days of disposal. For wastes regulated as PCBs or RCRA, the Contractor must use DM’s EPA ID number in the Uniform Hazardous Waste Manifest (EPA Form 8700-
22) and the manifest must be coordinated with and signed in block 15 by a designee in DM’s Environmental Management Office at the time of shipment. All handling, transporting, and disposal costs
01010-11 shall be considered as having been included as a factor in the Contractor's bid price. The Contractor will present any questions regarding proper handling or disposal of hazardous or non-hazardous materials to the CES PM through the Contracting Officer.

i. All radioactive material or equipment using radioactive material must be approved for use prior to bringing it on the base. The written request for approval, along with a copy of the permit for the source and a brief description of the intended activities, shall be submitted to the CES PM through the Contracting Officer no later than thirty (30) calendar days prior to the Contractor bringing such material onto the installation and its use. The CES PM will coordinate with the installation Bioenvironmental Engineering Flight for review and written approval No work involving radioactive materials may proceed until receipt of the approved request. This requirement shall be included as a submittal on the AF Form 66, Schedule of Material Submittals.
j. Any items containing radioactive material, such as radioluminescent exit signs, discovered while performing maintenance, repair, alteration or renovation activities, shall be turned in to the Government by the Contractor through the Contracting Officer for proper disposal.
k. Liquid paints or coatings with lead shall be restricted to no more than 0.06 percent lead by weight of the non-volatile solids for use in or on all base facilities [Part 35 of 24 CFR].
2.6 Security:
a. For entry to the base, submit an entry authorization request letter for each individual requiring access to the project, at least 10 calendar days prior to beginning work. A copy of the format of this letter may be obtained from the Contracting Officer on or before the pre-performance conference. Valid identification with a photograph together with the base pass will be required for all individuals when entering the base.
b. All passes issued must be returned to the Contracting Officer upon termination of the employee or completion of the project, whichever is sooner.
c. Some work under this contract may be located in AMARG, a controlled area, and strict entry control is maintained.
1. To gain access to AMARG, at least 10 calendar days prior to beginning work, all contractor personnel must be listed on an entry access letter (EAL). The information required from the Contractor to obtain access to AMARG shall be coordinated through the Contracting Officer.
2. Daily visitor passes are available at the AMARG gate for personnel with a company escort to vouch for them.
3. All passes issued must be returned to the Contracting Officer upon termination of the employee or completion of the project, whichever is sooner.
d. Some work under this contract may be located in Site 5, a controlled area, and strict entry control is maintained. To gain access to Site 5, all contractor personnel must be listed on an entry authorization list (EAL) as described above. The information required from the Contractor to obtain access to AMARG shall be coordinated through the Contracting Officer.
e. Some work under this contract may be located on the Davis-Monthan AFB Flightline. All Contractor personnel will be required to obtain a flightline operations briefing from the Airfield Manager prior to accessing the flightline area. During the course of the project, the Contractor will be required to check in and check out with the Base Operations Dispatcher daily.

1. During check-in, advise the dispatcher of intended work areas and type of work scheduled, and receive a flightline pass.
2. While on any runway, taxiway, or aircraft parking apron, immediately remove any dirt, debris, or other material spilled or caused by the Contractor's operations.
3. During check-out, brief the dispatcher on completed work and status of any airfield facilities affected by the work.
4. If contractor equipment is crossing a path on the airfield that is being actively utilized by aircraft, the Contractor shall station a flagman at the intersection and ensure that all taxiing aircraft are granted the right of way over contractor equipment.
5. Airfield communication: Maintain communications with base operations and the air traffic control tower at all times when operating within 100 feet of the runway.
6. Furnish all work groups working within 100 feet of the runway with walkie-talkie radios tuned to the ramp net frequency, 162.250 FM. Provide sufficient radios to allow immediate and simultaneous communication with all work groups.
7. When emergency or operational interruptions of work become necessary, respond immediately. When directed by base operations or the air traffic control tower, move all equipment and personnel a minimum of 100 feet from the runway edge.
2.7 Project Identification Sign: Use of project identification signs under this contract are not necessary and are discouraged.
2.8. Photographs: Photography while on or taken in the direction of the Davis-Monthan AFB flightline, to include aircraft parking ramps, taxiways and runway, is not authorized unless the individual taking such photographs is in possession of a Flightline Photography Authorization letter. In the event taking such photographs is necessary to the conduct of the project, the Contractor shall coordinate with the Contracting Officer in writing to obtain an authorization letter.
2.9. Fenced Storage Yard/Staging Area: The Contractor may choose to install and maintain a temporary fenced storage yard at a location designated by the Contracting Officer. All temporary fencing shall be of sufficient strength and height to restrict and minimize entry by unauthorized personnel and to withstand wind gusts. The temporary fencing shall be provided with visual screening for the full height and length, using tan-colored screening or a similar color selected and approved by the Contracting Officer. The temporary fencing and screening shall remain in place and be maintained for the life of this contract. Prior to any fencing being placed, the site shall be cleared and marked for utilities following the procedures identified in paragraph 1.3c above.
2.10. Material Handling and Storage:
a. Handle, store, and protect all products in accordance with manufacturer's instructions.
