About this file
This is a Cost Proposal Form for the NC26-43 Market Reporter contract opportunity issued by the State of Utah Division of Purchasing, with the Animal Industry division conducting the procurement. The solicitation seeks a vendor to provide market reports for animal auctions within Utah. The contract term is anticipated to be 1-5 years depending on funding, with vendors required to guarantee pricing for the first year. The cost proposal form specifies that vendors must submit costs for 156 reports per year, with the budget per report fixed at $250.
All cost proposals must be submitted on this standardized form with all cells highlighted in yellow completed, and all line items must be fully loaded to include all implementation and annual maintenance costs necessary to deliver and maintain the solution throughout the contract term. The total proposed annual cost cannot exceed the stated budget parameters, and any proposal exceeding budget will result in disqualification. Cost evaluation will be conducted using a formula whereby the vendor submitting the lowest proposed price receives 100% of the cost points available, and all other vendors receive a proportional percentage of cost points based on the formula: Cost Points × (Lowest Proposed Price/Proposed Price). Any costs not included on this Cost Schedule form will not be permitted in the resulting contract, and any deviation from the specified format may result in proposal disqualification.
