About this file
This is a Request for Proposal (RFP) form and instructions issued by the State of Utah Administrative Office of the Courts (AOC) for Aggression Replacement Training (ART) services to the Fifth District Juvenile Court serving Washington, Beaver, and Iron Counties. The RFP seeks a single award contract to provide evidence-based ART programming to moderate and high-risk juveniles ages 13-18 years old who are under court jurisdiction and have identified needs related to aggressive or antisocial behaviors. The target population includes males primarily, with capacity to serve females as needed. The RFP document provides a standardized form for company information, certifications, and proposal submission requirements. Proposals must be submitted electronically through the Bonfire U3P Utah Public Procurement Place website or delivered in writing to the AOC Purchasing Department at 450 South State Street, Salt Lake City, UT 84114-0241 by the posted due date and time. The solicitation number, due date, and "Private Bid" designation must appear on the envelope for written submissions. Proposals are valid for 60 days from the due date and may not be withdrawn during this period.
Pricing should be submitted separately on the attached cost sheet, with unit prices and total prices clearly identified for each item offered. Where freight is listed as a separate line item, suppliers must provide FOB Origin Less Freight pricing and indicate total freight costs on the Freight Cost line; otherwise, pricing should be FOB Destination Freight Prepaid. Suppliers must include their Utah Sales Tax ID number or Vendor Code with their bid as required by Utah Code 59-12-106(4). The AOC reserves the right to conduct best and final offer discussions with responsive offerors, request samples or manufacturer literature at the supplier's expense, and award multiple contracts if determined to be in the state's best interest. Offerors must certify they are not debarred or suspended from federal transactions and may claim protected information status for proprietary content by completing the Confidentiality Claim Form. The evaluation will consider price and other evaluation factors outlined in the RFP evaluation score sheet, and award will be made to the responsible offeror whose proposal is most advantageous to the AOC.
