About this file

This document is a Bidder Affirmation form for the University of Texas Health Science Center at San Antonio (UTHSCSA), specifically related to a Request for Bid (RFB) for Medical Scrubs Uniforms. The solicitation seeks qualified proposers to provide high-quality medical uniforms for clinical and non-clinical staff across multiple departments, with requirements for durable, comfortable, professionally styled uniforms available in various sizes and styles for men and women. Optional embroidery services for University Logo, Department Name, and Employee Name are also requested. The RFB may result in a multi-award contract to several proposers meeting the specified requirements, with no guaranteed volume of services or compensation.

The bidder affirmation document outlines extensive legal certifications and representations that proposers must make, including compliance with state and federal laws, antitrust regulations, tax obligations, and specific Texas government code provisions. Key certifications cover areas such as not boycotting Israel, not engaging with Iran or Sudan, no discrimination against firearm entities, compliance with COVID-19 documentation requirements, and adherence to executive orders regarding foreign adversaries. The document emphasizes the bidder's responsibility to defend, indemnify, and hold harmless the University, UT System, and the State of Texas from any claims arising from the proposal or subsequent contract performance.
