EXECUTIVE SUMMARY

TTL Associates, Inc. (TTL) conducted a National Emission Standard for Hazardous Air Pollutants

(NESHAP) Asbestos Survey in the Quarter Buildings (Building numbers 23, 24, 25, 26, 27, 28, and 29) as well as their associated garages (Building numbers 31 and 32) at the Edward Hines, Jr.

Veterans Affairs Medical Center (Hines VAMC) located in Hines, Illinois. The purpose of the survey was to identify asbestos-containing materials (ACM) throughout the above-mentioned areas prior to demolition activities.

The survey was conducted from October 19, 2020 to October 22, 2020 by Mr. Robert Serlin of

TTL.

TTL’s scope of work included the following:

• Surveying the Quarter Buildings (Building numbers 23, 24, 25, 26, 27, 28, and 29) as well as their associated garages (Building numbers 31 and 32) for suspect ACM.

• Collection of suspect ACM bulk samples for analysis by Polarized Light Microscopy (PLM) to determine asbestos content.

TTL reviewed previous ACM data (Triennial Asbestos Re-inspection Report – January 2011) provided by Guidon Design, Inc. (Guidon) to assess if previously identified ACM was present in the buildings. Any suspect materials that were not identified and sampled in the previous data report were identified, quantified and samples were collected.

Asbestos Survey

TTL identified 137 suspect materials in the Quarter Buildings (Building numbers 23, 24, 25, 26, 27, 28, and 29) as well as their associated garages (Building numbers 31 and 32). A total of 301 samples were collected and 507 layers were analyzed. The laboratory analysis of the samples was conducted by International Asbestos Testing Laboratories (IATL) using polarized light microscopy (PLM) method. The roof materials on the buildings (HSA 1993901-23-09, 1993901-

24-23, 1993901-25-20, 1993901-26-20, 1993901-27-8, 1993901-28-25, 1993901-29-23 and

1993901-31/32-05) were assumed to be asbestos-containing due to the height of the roofs and were not sampled. The mastic associated with the cream floor tile (HSA 1993901-28-18) and mastic associated with the white ceramic floor tile (HSA 1993901-28-19) both in Building 28 contained less than one percent asbestos by standard PLM and was further analyzed by point count analysis.

The mastic is not asbestos-containing as defined by the United States Environmental Protection

Agency (U.S. EPA).

1.0 INTRODUCTION

TTL Associates, Inc. (TTL) conducted a National Emission Standard for Hazardous Air Pollutants

(NESHAP) Asbestos Survey in the Quarter Buildings (Building numbers 23, 24, 25, 26, 27, 28, and 29) as well as their associated garages (Building numbers 31 and 32) at the Edward Hines Jr.

Veterans Affairs Medical Center (Hines VAMC) located in Hines, Illinois.

1.1 Project Purpose and Objectives

TTL conducted the survey in the all areas of the Quarter Buildings (Building numbers 23, 24, 25, 26, 27, 28, and 29) as well as their associated garages (Building numbers 31 and 32) to determine the presence of asbestos-containing materials (ACM) which may require removal prior to demolition activities.

TTL reviewed previous ACM data (Triennial Asbestos Re-Inspection Report – January 2011) provided by Guidon Design, Inc. (Guidon) to assess the presence of previously identified ACM in the Quarter Buildings (Building numbers23, 24, 25, 26, 27, 28, and 29) as well as their associated garages (Building numbers 31 and 32). Suspect materials not identified in the previous data were surveyed, quantified and sampled.

1.2 Personnel

The non-destructive survey was conducted by Mr. Robert Serlin of TTL. Mr. Serlin is an Illinois

Department of Public Health (IDPH) certified Asbestos Inspector. A copy of Mr. Serlin’s certification is included in Appendix A.

TTL Project No.1993901 Page 2

2.0 ASBESTOS SURVEY

This section documents the methodology and results of the asbestos survey.

2.1 Homogenous Areas

Each accessible area within the Quarter Buildings (Building numbers 23, 24, 25, 26, 27, 28, and

29) as well as their associated garages (Building numbers 31 and 32) was surveyed which included the identification of suspect materials and the definition of homogeneous sampling areas (HSA), assessment of the condition of each material, estimation of the approximate quantity of the suspect

ACM, and collection and analysis of representative bulk samples from each identified HSA. An

HSA is defined as a material that exhibits similar physical characteristics (e.g., texture, surface color, and appearance) and was applied or installed during the same construction period (if known) as determined by TTL’s inspector utilizing professional judgment, experience, and historical building information. TTL only sampled materials not identified in the previous survey data.

2.2 Sampling and Analysis Methods

TTL provided a certified Illinois Department of Public Health (IDPH) certified Asbestos Inspector to conduct the non-destructive survey of the Quarter Buildings (Building numbers 23, 24, 25, 26, 27, 28, and 29) as well as their associated garages (Building numbers 31 and 32).

Suspect ACM samples were collected using a coring device or other means, as appropriate, to collect a cross section of the suspect material. The samples were placed into clean, unused sealable bags and marked with a unique sample identification number. The samples of suspect ACM were transported to International Asbestos Testing Laboratories (IATL) and analyzed by Polarized Light

Microscopy (PLM) using U.S. EPA Method 600/R-93/116. The EPA/600/R-93/116 “Method for the Determination of Asbestos in Bulk Building Materials” requires that all multiple, distinct layers must be analyzed individually. Sample analysis results are provided for each distinct layer of each sample submitted to the laboratory.

IATL is accredited by the National Voluntary Laboratory Accreditation Program (NVLAP), No.

101165-0, which is administered by the National Institute of Standards and Technology (NIST).

The laboratory accreditation number is included in the analytical report.

2.3 Survey Analytical Results

TTL identified 137 materials in the quarter buildings (23, 24, 25, 26, 27, 28, and 29) as well as their associated garages (31 and 32). A total of 301 samples were collected and 507 layers were analyzed. The roof material on the buildings (HSA 1993901-23-09, 1993901-24-02, 1993901-25-

20, 1993901-26-20, 1993901-28-25, 1993901-29-23, and 1993901-31/32-05) were assumed to be asbestos containing due to the height of the roofs and were not sampled. The mastic associated with the cream floor tile (HSA 1993901-28-18) and mastic associated with the white ceramic floor tile (HSA 1993901-28-19) both in Building 28 contained less than one percent asbestos by standard PLM and was further analyzed by point count analysis. The mastic is not asbestos-

TTL Project No.1993901 Page 3 containing as defined by the United States Environmental Protection Agency (U.S. EPA). The following materials were identified or assumed to be asbestos containing as defined by the United

States Environmental Protection Agency (U.S. EPA) based upon the survey conducted by TTL.

Building 23:

• Approximately 150 linear feet (l.f.) of 0-6” aircell pipe insulation (HSA 03)

• Approximately 300 l.f. of 0-6” mag pipe insulation (HSA 04)

• Approximately 20 mud and horse hair pipe fittings on 0-6” mag pipe insulation (HSA 06)

• Approximately 50 l.f. millboard pipe insulation (HSA 07)

• Approximately 1,800 square feet (s.f.) of roof material (HSA 09), Assumed ACM

The survey was a non-destructive survey; therefore, it is possible that pipe insulation may be hidden inside the plaster walls.

Building 24:

• Approximately 2,200 s.f. joint compound associated with drywall (HSA 24-02)

• Approximately 70 l.f. of 4” Black covebase with associated glue (HSA 24-13)

• Approximately 280 s.f. of wood patterned linoleum flooring (2 layers) and associated mastic (HSA 24-15)

• Approximately 14,000 s.f. of roof material (HSA 24-23), Assumed ACM

• Approximately 80 mud fittings on 0-6” pipe insulation (HSA 224-5)

The pipe insulation and hard fittings located in the basement were labeled with an “A”, therefore, they are assumed to be asbestos-containing and were not sampled.

Building 25:

• Approximately 14,000 s.f. joint compound associated with drywall and joint compound

(HSA 25-02)

• Approximately 200 s.f. of wood patterned linoleum flooring (3 layers) (HSA 25-11)

• Approximately 150 s.f. of attic wood pattern flooring (3 layers) (HSA 25-14)

• Approximately 2,800 s.f. of roof material (HSA 25-20), Assumed ACM

• Approximately 60 l.f. of 0-6” aircell pipe insulation (HSA 25-21), Assumed ACM hidden inside the plaster walls. There was no access to Apartment 25 West, therefore millboard pipe insulation (HSA 1993901-25-4) could be present. The pipe insulation and hard fittings located in the basement were labeled with an asbestos danger sticker, therefore, they are assumed to be asbestos-containing and were not sampled.

Building 26:

TTL Project No.1993901 Page 4

• Approximately 2,000 s.f. joint compound associated with drywall (HSA 02)

• Approximately 250 l.f. of 0-6” aircell pipe insulation (HSA 17), Assumed ACM

• Approximately 6 mud fittings on 0-6” aircell pipe insulation (HSA 18), Assumed ACM

• Approximately 1 l.f. of 6-12” aircell pipe insulation debris (HSA 19), Assumed ACM

• Approximately 2,800 s.f. of roof material (HSA 20), Assumed ACM

The pipe insulation and hard fittings located in the basement were labeled with an asbestos danger sticker, therefore, they are assumed to be asbestos-containing and were not sampled. The brown covebase (HSA 1993901-26-13) is negative for asbestos for the covebase material and the associated glue. There was joint compound as part of the sample that tested positive for asbestos.

The joint compound in the brown covebase samples is associated with the drywall system (HSA

1993901-26-02) that contains asbestos-containing joint compound.

Building 27:

• Approximately 1 sink with black sink undercoating (HSA 27-05)

• Approximately 2,800 s.f. of roof material (HSA 27-08), Assumed ACM

Building 28:

(HSA 28-09)

The square pattern linoleum flooring (HSA 1993901-28-9) was over multiple layers of plywood subflooring. There could be other flooring materials present under the plywood subflooring. The survey was a non-destructive survey; therefore, it is possible that pipe insulation may be hidden inside the plaster walls. The brown covebase (HSA 1993901-28-06) is negative for asbestos for the covebase material and the associated glue. There was joint compound as part of the sample that tested positive for asbestos. The joint compound in the brown covebase samples is associated with the drywall system (HSA 1993901-28-02) that contains asbestos-containing joint compound.

Building 29:

• Approximately 2,000 s.f. joint compound associated with drywall (HSA 29-01)

• Approximately 180 s.f. of wood pattern linoleum flooring – 2nd layer (HSA 29-13)

• Approximately 30 s.f. of 12”x12” grey speckled floor tile – 2nd layer (HSA 29-16)

TTL Project No.1993901 Page 5 hidden inside the plaster walls.

Garage 31 and 32:

• Approximately 60 l.f. of window glazing (HSA 02) – Garage 32

• Approximately 40 l.f. of window caulk (HSA 03) – Garage 32

• Approximately 1,000 s.f. of roof material (HSA 31/32-05), Assumed ACM – Garage 1

3.0 CONCLUSIONS/RECOMMENDATIONS

This section summarizes the results of the NESHAP Asbestos Survey and provides conclusions and recommendations.

3.1 Asbestos Survey

The U.S. EPA defines regulated asbestos-containing material (RACM) as: (a) Friable asbestos material, (b) Category I Non-Friable ACM that has become friable, (c) Category I Non-Friable

ACM that will be or has been subjected to sanding, grinding, cutting, or abrading, or (d) Category

II Non-Friable ACM that has a high probability of becoming or has become crumbled, pulverized, or reduced to powder by the forces expected to act on the material in the course of demolition or renovation operations.

Building 23

The following friable ACM were identified and require removal prior to demolition activities that would disturb the material:

• Approximately 150 l.f. of 0-6” aircell pipe insulation (HSA 23-03)

• Approximately 300 l.f. of 0-6” mag pipe insulation (HSA 23-04)

• Approximately 40 l.f. of window caulk (HSA 31/32-03) - Garage 32

NESHAP asbestos regulations require the removal of all RACM from a facility being demolished or renovated prior to beginning any activity that might damage or disturb the material. The Illinois

Environmental Protection Agency (IEPA) requires written notification if the combined amount of

RACM to be removed is at least 260 linear feet, at least 160 square feet, or at least one cubic meter of facility components where length or area could not be measured.

TTL Project No.1993901 Page 11

The IDPH requires a ten-day notification if the amount of RACM to be removed exceeds 3 linear feet or 3 square feet and up to 260 liner feet and 160 square feet. This notification is to be submitted at least ten working days prior to beginning any asbestos abatement.

TTL recommends the removal of the RACM and the ACM that might become RACM based on the project-specific demolition techniques by a licensed asbestos abatement contractor. Based on the condition of the material, the identified ACM may be expected to be a RACM. Therefore, the demolition contractor must be notified of the presence, quantity and location of the material so as to avoid project specific demolition techniques that may render the material friable and RACM.

3.2 Limitations

TTL has made reasonable efforts to identify and quantify suspect ACM based upon the standard of care in the environmental industry existing at the time of the survey. This survey only summarizes the potential presence and estimated quantities of visually observed ACM within the designated areas.

The surveys were non-destructive, therefore, there is a potential for pipe insulation to be hidden behind plaster walls and ceilings. The pipe insulation identified in Buildings 23, 24, 25, and 26 were labeled with either an “A” on the pipe or were labeled with asbestos danger stickers and were not sampled and are assumed to be asbestos-containing. The roof materials on the buildings are assumed to be ACM due to the height of the buildings and were not sampled.

Additional material disturbed during renovation or demolition activities should be evaluated on a case-by-case basis, especially materials that were previously hidden, obscured or inaccessible, to determine if the material is included in this survey. If a given material is not described in this survey or cannot be identified as a non-suspect material, the material should be assumed to contain asbestos and renovation and/or demolition activities should be halted until sampling and analysis can be accomplished. Parties conducting renovation and/or demolition activities should follow all applicable federal, state, and local regulations in handling identified and suspect ACM.

The information contained in the report was based upon specific parameters and regulations in force at the time of the survey. The information herein is only for the specific use of Guidon, Hines

VAMC and TTL, unless written authorization is obtained from TTL. TTL accepts no responsibility for the use, interpretation, or reliance by other parties on the information contained herein, nor does this report represent an instrument of regulatory compliance or an asbestos abatement specification.
