About this file
This is a Cost Proposal Form for the Utah Department of Transportation's RFP regarding Utah Broadband telecommunication expertise. The solicitation seeks a single contractor to provide legal and regulatory expertise to guide the Utah Broadband Center on administration, compliance, monitoring, and subgrantee contracting for the federal Broadband Equity, Access, and Deployment (BEAD) Program. The contractor must understand the legal implications and framework of the telecommunication industry and adhere to Utah Department of Transportation standards and specifications. The form requires offerors to submit cost proposals as a separate document from technical responses, with no modifications to the prescribed format permitted.
Pricing is based on a blended hourly rate that serves as the maximum billable rate for all services, inclusive of all labor costs and profit. Offerors must also provide position-specific labor rates for Senior Manager/Executive, Mid-Level Staff, and Support Staff positions. Evaluation of cost proposals uses a comparative formula in which the lowest proposal receives 100 percent of available cost points, with all other proposals receiving points based on their percentage variance above the lowest price. Proposals exceeding 200 percent of the lowest proposed price receive no points. The contract will authorize payment for labor based on agreed-upon rates and allow reimbursement of pre-authorized pass-through direct expenses with supporting invoices. Travel costs are reimbursed at the current per diem rate established by the State of Utah Travel Policy.

Cost Proposal
To be submitted as a separate document from the Technical Response document Offeror Name:
Cost is to be submitted based on the following:
(Any deviation from this format may result in disqualification of proposal.)
Provide a blended hourly rate for the tasks described in the RFP. The “blended” hourly rate is the maximum billable rate for all services and must include all labor costs for staff, and profit. Labor may include account supervision, planning, consultation, and other labor expenses required to accomplish the needed tasks.
Blended Hourly Rate:
$________ per/hr.
Note: The Blended Hourly Rate will be used as the basis for assigning Cost Points in the RFP evaluation. The Position specific labor rates are for contracting purposes.
Additionally, provide the following specific position labor rates:
Position
Associated Rate
1. Senior Manager/ Executive
$
2. Mid-Level Staff
$
3. Support Staff
$
Purchasing will use the following cost formula: The points assigned to each offeror’s cost proposal will be based on the lowest proposal price. The offeror with the lowest Proposed Price will receive 100% of the price points. All other offerors will receive a portion of the total cost points based on what percentage higher their Proposed Price is than the Lowest Proposed Price. An offeror whose Proposed Price is more than double (200%) the Lowest Proposed Price will receive no points. The formula to compute the points is: Cost Points x (2- (Proposed Price/Lowest Proposed Price)).
IMPORTANT: Offerors may not change/modify the format of this cost form. The State relies on this cost sheet to accurately and fairly compare offeror costs. Cost Proposals received in a modified or alternate format may be disqualified.
*Costs of travel will be reimbursed at the then-current per diem rate set by the State of Utah Travel Policy, as found at: http://www.rules.utah.gov/publicat/code/r025/r025-007.htm The resultant contracts will specifically authorize payment for time spent on work based on the agreed upon labor rates, it also is the intent of the contract to allow for the payment of pass-through direct expenses (accompanied with invoices) as pre-authorized by the project manager.
