About this file

This is a Fee Proposal Form (Form F) for RFQ #26-SWU08, a Stormwater Facility Vegetation Management contract opportunity in Wisconsin. The form requires vendors to submit comprehensive fee proposals that cover all direct and indirect necessary expenses for environmental and ecological services. The contract scope involves services across multiple staff position levels, including Senior Scientist/Director, Senior Biologist/Scientist II, Staff Biologist/Scientist, Environmental Scientist, Crew Leader, and Technician positions. Vendors must provide current hourly rates for each position category and detailed position descriptions that outline the qualifications and responsibilities of personnel. The form indicates this is a framework agreement for placing qualified vendors on a bidding list for upcoming stormwater vegetation management projects, with the City of the requesting jurisdiction retaining the authority to authorize additional services only through written consent from its designated representative.

The fee proposal must be all-inclusive, with compensation covering all services, project deliverables, required meetings with City Council and related boards or commissions, and all contractor and subcontractor costs. Vendors must indicate their acceptance of three payment methods: credit card payment without additional fees, Electronic Funds Transfer (EFT), and piggybacking authorization for other government agencies. For any services beyond those described in the agreement, vendors must provide hourly rates for additional staff positions and applicable overhead rates on an attached fee sheet. Proposers must sign and submit the form with the legal company name and signature of an authorized representative, attesting to the truthfulness and conformity of all submissions with the City's requirements.
