About this file
This certification forms attachment is submitted as part of ITB #26-026 issued by the Kenton County Airport Board for the Two (2) Digester Tanks Aeration Equipment Replacement project at Cincinnati/Northern Kentucky International Airport. The document contains six required certification forms that all bidders must complete, sign, and submit with their proposals. The certifications cover non-debarment or suspension status, Buy American compliance for equipment and building projects, disclosure of lobbying activities, worker's compensation insurance compliance per Kentucky law, tax delinquency and felony conviction status, and domestic preferences for procurements. All bidders must indicate their current status regarding these certifications, with responses made under penalty of perjury and notarized where required. A mandatory pre-bid meeting and site visit is scheduled for Thursday, April 23, 2026 at 10:00 am at CVG Centre Administrative Offices in Erlanger, Kentucky, with sealed bids due no later than Thursday, May 7, 2026 at 2:00 pm Eastern Time through KCAB's online bidding portal and remaining valid for a minimum of 120 days.
Since this is a federally funded project subject to FAA requirements, bidders must demonstrate compliance with Buy American preferences, Disadvantaged Business Enterprise participation goals under 49 CFR Part 26, Davis-Bacon prevailing wage requirements if installation services are included, and Executive Order 11246 affirmative action and equal employment opportunity requirements. For Buy American certification, bidders must select one option: installing only U.S.-produced steel and manufactured products, installing only U.S.-manufactured construction materials as defined, installing FAA-waivered products, or installing excepted articles and materials. Bidders unable to achieve 100 percent Buy American compliance may qualify for a Type 2, 3, or 4 waiver by submitting formal documentation including content percentage worksheets, market research findings, component cost analyses, or comparable bid comparisons within 15 calendar days of selection as the responsive bidder. Bidders must also certify they have no unpaid Federal tax liabilities assessed and not being paid under agreement, have not been convicted of criminal violations under Federal law within the preceding 24 months, and comply with worker's compensation and unemployment insurance requirements under Kentucky law. False statements regarding these certifications may result in criminal prosecution under Title 18 of the United States Code and civil penalties of not less than $10,000 and not more than $100,000 for each violation.
