About this file
This is a Darfur Contracting Act Certification form issued by the State of California for solicitation 25FS0040, which pertains to Pharmacy Regimen Services at the Veterans Home of California at Fresno. The form applies to any company that has conducted business activities or operations outside the United States within the previous three years. The specific details regarding scope of work, quantities, response deadlines, and bidder meetings are referenced in Exhibit A and the Notice to Prospective Bidders (IFB Letter), which contain the complete project timeline and submission requirements.
The certification form requires authorized company representatives to complete one of two options: Option #1 involves certification under penalty of perjury that the company is not a scrutinized company as defined under Public Contract Code Section 10476, while Option #2 allows scrutinized companies to submit bids if they have obtained prior written permission from the Department of General Services on a case-by-case basis, with the permission letter to be included with the bid submission. The form requests standard identification information including company name, Federal ID Number, and signature from an authorized company representative.
