About this file
This is a Bidder's Checklist for State of California Solicitation Number 25FS0040, issued by the Department of Veterans Affairs for Pharmacy Regimen Services at the Veterans Home of California at Fresno. The solicitation requires bidders to submit comprehensive documentation including a completed Bidder Certification, Bid Rate Sheet, signed Payee Data Record (STD 204), Bidder Declaration (GSPD-05-105), and Contractor Certification Clauses (CCC-4/2017). All questions regarding the scope of work (Exhibit A) must be submitted via email, and bidders should consult the Notice to Prospective Bidders (IFB Letter) for the complete timeline, including response dates and award dates. Additional required submissions include valid California business licensing documentation or equivalent proof of good standing for out-of-state entities, and a sample invoice demonstrating understanding of invoicing requirements.
The checklist indicates conditional submissions for specific bidder categories, including Commercially Useful Function (CUF) Certification if applicable, and Disabled Veteran Business Enterprise (DVBE) Declarations (DGS PD 843) if the bidder qualifies for this set-aside. Bidders must also certify compliance with the Darfur Contracting Act and California Civil Rights Laws. The company name listed on all submitted documents, including licenses, permits, certifications, and insurance, must match exactly with the company name on the Payee Data Record (STD 204). Failure to submit required documents in the prescribed manner may result in delays to contract award.
