About this file

This is a California Civil Rights Laws Certification form issued by the Department of General Services Office of Legal Services for solicitation 25FS0040, which pertains to Pharmacy Regimen Services at the Veterans Home of California at Fresno. The certification requirement applies to all bidders and contractors submitting proposals for state contracts valued at $100,000 or above. Bidders must certify compliance with the Unruh Civil Rights Act and the Fair Employment and Housing Act for contracts executed or renewed after January 1, 2017. The form requires an authorized representative to sign under penalty of perjury and provide the proposer's firm name, federal identification number, county and state where executed, and the execution date.

The certification also requires contractors to attest that any internal policies regarding sovereign nations or peoples recognized by the United States government are not applied in violation of California civil rights laws. This mandatory certification must be completed and submitted with the bid or proposal as a condition of contract eligibility. The form serves as a prerequisite compliance document rather than establishing pricing, set-asides, or other commercial terms of the underlying Pharmacy Regimen Services contract.
