About this file
Summary
This is an Exhibit D Level 1 Health, Safety and Environmental Specifications document for the Orange County Transportation Authority (OCTA) Request for Quotes (RFQ) 250144 regarding annual safety inspections and certification of aerial lift equipment. The RFQ seeks a qualified contractor to perform annual inspections and certifications of aerial lifts, scissor lifts, LPI lifts, man lifts, and platform lift trucks in accordance with ASTM/ANSI standards. The contract is valued at $12,000 for a two-year term. Written questions or clarifications must be submitted by 4:00 p.m. on September 18th, 2026, with OCTA responding by written addendum no later than September 25th, 2026. The award will be made to the lowest responsive and responsible firm on a lump sum basis, and the successful contractor must comply with all applicable Equal Employment Opportunity Laws and Regulations.
The specifications document establishes comprehensive health, safety, and environmental (HSE) requirements that apply to the contractor, subcontractors, suppliers, and employees. The contractor must comply with all Authority HSE compliance requirements, project site requirements, and applicable transit safety rules while assuming sole responsibility for hazard identification and implementation of appropriate controls. The document mandates immediate notification to the Authority's designated representative of any incidents, including property damage, recordable injuries or near-miss events, environmental incidents such as spills or releases, and inspections by external agencies such as Cal/OSHA, DTSC, or the EPA. A preliminary written incident report must be submitted within 24 hours of any incident, with serious injuries or significant events potentially requiring formal investigation and review. Non-compliance with these requirements may result in corrective action ranging from suspension of work to contract termination at the Authority's sole discretion.
