About this file

This document is an Invitation to Bid (ITB) Attachment D for Additional Terms and Conditions to an Enterprise Contract, issued by the State of Florida with Solicitation No. 25-64121511-ITB for an insurance contract. The document modifies the standard Form PUR 1000 by removing multiple sections related to transportation, delivery, packaging, installation, risk, product versions, and cooperative purchasing. The contract will have an initial term beginning on a date specified in the contract, with potential renewal options subject to written agreement between the Department and the Contractor, in accordance with section 287.057(13), F.S.

The document defines key terms such as "Customer" and "State Term Contract", specifying that customers are agencies or eligible users included on the insurance policy, and that the contract is competitively procured by the department for use by agencies and eligible users. For this insurance contract, customers other than the Department cannot amend or modify the contract or insurance policy terms. The contract appears to be a state-level insurance procurement vehicle that will allow various state agencies and eligible users to access the negotiated insurance services under predefined terms and conditions.
