GOLF SIM/MULTI SPORT SIMULATOR

STATEMENT OF WORK

Apr 2025

1. SCOPE OF WORK

The contractor will:

• Delivery and installation of the virtual simulator system, including all necessary hardware and software components

• Configuration and testing of the system to ensure proper functionality

• Provide system training for six 45th FSS NAF personnel immediately following the completion of installation. This includes basic operation of the system, launching of software, and basic overview of components installed.

• Provide on-call technical support o While the system is under warranty the Vendor shall maintain a free remote service (via telephone, modem, fax, or internet connection) through which Customer may contact the Vendor for detailed service advice.

o Telephone support shall be available by calling Vendor's USA office between the hours of 9 a.m. and 5 p.m. MST, Monday-Friday.

o When system falls outside of the warranty period, Vendor will provide unlimited telephone support to Customer for a single simulator.

2. PERIOD OF PERFORMANCE

The period of performance for this contract will commence on the date of the contract award and continue for a period of 60 days. Request to extend beyond the 60 days must be provided in writing and must be acceptable to the government.

3. PHYSICAL LOCATION

The work will be performed at the Patrick SFB Shark Lanes Bowling Alley, located at 782 Omalley Rd. Building 732 Patrick SFB, FL 32925. The contractor will ensure that all work is completed in a safe and efficient manner, with minimal disruption to the normal operations of the bowling alley.

4. SUPPLIES AND EQUIPMENT

The contractor will provide all necessary supplies and equipment to complete the installation, set-up, and training, including:

• The multi-sport virtual simulator system, including all hardware and software components

• Installation and testing equipment

• Training materials and documentation

• Any other supplies or equipment necessary to complete the work

• Contractor Responsibility: The Contractor is solely responsible for the removal and disposal of all packaging materials, construction debris, and trash generated during the installation of the golf simulator. This includes, but is not limited to, boxes, packing peanuts, cut-offs, fasteners, and any other waste products.

• Client Premises: The Contractor will ensure that the client's premises are left clean and free of debris upon completion of the installation.

• Disposal Compliance: The Contractor will dispose of all waste materials in accordance with all applicable local and federal regulations.

• Additional Costs: Any costs incurred due to improper disposal of waste by the Contractor will be the sole responsibility of the Contractor.

5. PAYMENT TERMS

The payment terms for this contract will be net 30 days. The contractor will submit invoices IAW contract terms and payment will be made within 30 days of receipt of the invoice.

6. ACCEPTANCE CRITERIA

The acceptance criteria for this contract will include:

• Proper installation and set-up of the virtual simulator system

• Completion of system training for Patrick SFB Shark Lanes Bowling Alley staff

• Verification that the system is functioning properly and meeting all specified requirements

• Receipt of all necessary documentation, to include user manuals, warranty, and technical support contact information.

7. SPECIAL REQUIREMENTS

The following special requirements will apply to this contract:

• The contractor will ensure that all work is completed in compliance with all relevant safety and regulatory requirements.

• The contractor will provide a warranty for the virtual simulator system, for a minimum of covering parts and labor for a period of at least 24 months from the date of shipment.

• Contractor will follow diagram installation requirements per regulation safety standards.

9. INSTALLATION ACCESS: The Contractor must submit a consolidated list of names, date of birth, and Florida State driver’s license numbers to The FSS POC no less than 1 week prior to requiring access to the installation. Personnel who do not have a Florida issued driver’s license must submit their social security number. The FSS POC will submit the list to 45 Security Forces, who will process a local background check on all individuals prior to granting authorization to access the installation. Contactor shall ensure personnel have a current state issued identification card.

• Personnel Information Submission: The Contractor shall provide the Contract Point of

Contact (POC) with a comprehensive list of all personnel requiring access to the installation for the purpose of golf simulator installation. This list must be submitted no less than one (1) week prior to the anticipated access date.

• Required Information: The list must include the following for everyone:

• Full Legal Name

• Date of Birth

• Florida Driver's License Number (if applicable)

• Social Security Number (only if individual does not possess a Florida Driver's License)

• Background Check: The Contract POC will submit the provided information to 45th

Security Forces, who will conduct local background checks on all listed individuals. Access to the installation will be contingent upon successful clearance of these background checks.

• Identification Requirement: All Contractor personnel granted access to the installation must carry a current, valid, government-issued photo identification card (e.g., driver's license, state ID) at all times while on the premises.

• Access Approval:

• Notification of Approval: The FSS POC will notify the Contractor of the approval status for each individual.

• Denied Access: If access is denied for any individual, the Contractor is responsible for providing a suitable replacement who meets the clearance requirements within a reasonable timeframe to prevent project delays. The Contractor remains solely responsible for any additional costs incurred due to personnel access issues.
