About this file
This document is an application response form for a transitional housing supportive case management (SCM) program RFR (Request for Response) issued by the Commonwealth of Massachusetts Executive Office of Health and Human Services Department of Public Health. The RFR seeks proposals for transitional housing programs that provide SCM services, community-based supports, and a supportive living environment to assist clients in the recovery process. Applicants must provide details on their experience, partnerships, staffing, service delivery, quality improvement, and program operations. Proposals must be submitted online through the COMMBUYS system by the designated due date.
The application requires applicants to provide information on their organizational structure, mission alignment, target populations, housing details, service elements, staff qualifications and training, referral and case management processes, and site control documentation. Pricing information is not requested, as this appears to be a program grant opportunity rather than a traditional procurement. Applicants must demonstrate cultural competence and linguistically appropriate service delivery to meet the diverse needs of the client population. Successful proposals will be awarded a contract to operate the transitional housing SCM program, with contract term details to be determined.
