About this file

This is a Past Performance Questionnaire template issued by the Bureau of Engraving and Printing (BEP) for snow and ice removal services under solicitation 2031ZA25Q00071. The questionnaire requires offerors to obtain evaluations from three assessors who can validate their performance on similar contracts within the last 4 years. Relevant past performance must be similar in size, scope, and complexity to the current requirement.

The questionnaire contains five sections covering contract information, assessor details, and performance evaluations across multiple criteria including customer satisfaction, schedule performance, management performance, and quality. Assessors must complete and submit the questionnaire directly to the contracting office by February 21, 2025 at 10:00 AM EST. Performance ratings range from Exceptional to Unsatisfactory, with detailed definitions provided for each rating level. The questionnaire emphasizes the importance of independent assessor completion and submission, rather than through the offeror.
