About this file
This is a Past Performance Questionnaire template designed for the Library of Congress Inspection of Production Lots Program (Solicitation #030ADV26R0014). The form serves as an evaluation instrument for assessing contractor performance on past or current contracts and will be used during the proposal evaluation process for solicitation 2026-LGD-0045.
The questionnaire is structured in five sections: (1) Contract Identification, which captures the contractor name, agency contract number, prime/subcontractor status, agency name, contract type, period of performance, total contract value, and description of services; (2) Evaluator Identification, requesting the evaluator's name, title, email, and phone number; (3) Evaluation, a scoring matrix covering Quality of Product or Service, Contract Management and Performance, and Transition/Phase-In & Employee Retention, with ratings of Excellent, Satisfactory, Poor, or N/A; (4) Narrative Summary, requesting written responses regarding contractor strengths, weaknesses, and future hiring likelihood; and (5) Signatures for authentication. The completed form is to be submitted to Jennifer Zwahlen (Contracting Officer) at jzwa@loc.gov and Jacqueline Miller (Contract Specialist) at jacmiller@loc.gov. This template establishes the standardized evaluation criteria that past clients of prospective offerors must complete to support their proposal submissions.
