GENERAL NOTES

1. The project involves the removal and replacement of a damaged sanitary sewer pipe buried under a flood protection levee in the City of Olean. The project will require excavation of the flood protection levee to reach the pipe and backfill placement to restore the site to preconstruction conditions after replacement of the sanitary pipe. The flood protection levee under this project is located on the east side of Olean Creek.

2. The provided Technical Specifications should be used in conjunction with the Contract Plans. The pay item numbers shown on the Contract Plans for specified material and construction activities are in accordance with the Technical Specifications.

3. Subsurface explorations have been completed for this project at the locations indicated on Sheet 3 of the Contract Plans. Boring logs, groundwater monitoring well installation logs, and other subsurface information shall be made available for the inspection of bidders.

4. Unless otherwise noted or approved by the Engineer, construction work is to be accomplished in the sequence as shown on the Contract Plans and as described in the Technical Specifications.

5. The Contractor shall supply a materials list of chemicals to be used or stored on the job site. This shall include material safety data sheets and documentation of worker training.

6. Access for rescue and fire fighting equipment and personnel shall be maintained at all times. The rescue and fire fighting operations shall have right-of-way over all Contractor's operations at all times.

7. The Contractor shall refer to the Technical Specifications for time constraints and construction scheduling.

8. All elevations shown on the Contract Plans are based on NAVD 88.

9. The Contractor shall examine and verify, in the field, all conditions and dimensions. The Contractor shall take all field measurements to ensure proper fit of the finished work, and the contractor shall assume full responsibility for their accuracy. If field conditions and dimensions differ from those shown on the Contract Plans, the Contractor shall use the field conditions and dimensions and make the appropriate changes to those shown on the Contract Plans, as approved by the Engineer. When shop drawings based on field measurements are submitted for approval, the field measurements made shall be indicated on the shop drawings submitted for reference of the reviewer.

10. The Contractor's attention is directed to the fact that, due to the nature of reconstruction projects, the exact extent of reconstruction work cannot always be accurately determined prior to the commencement of work. The Contract Plans and Technical Specifications have been prepared based on field inspection, historical construction drawings, and other information available at the time. Actual field conditions may require modifications to construction details and work quantities. The Contractor shall perform the work in accordance with field conditions.

11. The Contractor will be held responsible for all damage to existing utilities and structures caused by its operations which are not included as part of the intended work. All damage to the existing structures which are not part of the intended work shall be repaired by the Contractor without cost to the Owner, and to the satisfaction of the Engineer and Owner.

12. The Contractor shall conduct its operations to the satisfaction of the Owner to prevent or reduce to a minimum any damage to any waterway from pollution by debris, sediment, or other foreign material, or from the manipulation of equipment and/or materials in or near Olean Creek. The Contractor shall not return directly to a stream, or to a ditch immediately flowing into a stream, any water which has been used for wash purposes or other similar operations which could cause this water to become polluted with sand, silt, cement, oil, or other impurities.

13. Visibly turbid discharges from dewatering operations or excavation activities shall not be allowed to enter any waterway. The Contractor shall be responsible for all measures necessary to eliminate the turbidity prior to discharge.

14. Erosion and sedimentation control measures are described on the Contract Plans and in the Technical Specifications (Section 31 25 00 - Erosion and Sedimentation Control). The Contractor shall modify the controls as necessary to accommodate its operations, and obtain approval by the Engineer and Owner prior to implementing any changes.

15. The Contractor shall contact Dig Safely New York in advance of any digging to avoid interruption of service provided by underground utilities. The phone number is 811.

16. Should utilities be encountered during construction which interfere with the work and for which provisions are not provided on the Contract Plans, the Contractor shall immediately notify the Engineer and Owner of their existence and extent of conflict with the work. The Contractor shall make arrangements with the owning agency to modify the site in order to allow the work to progress. Any adjustments to contract quantities as a result of such utility conflicts will be paid for at contract unit prices.

17. Location of utilities, public and/or private, indicated as existing and/or to be constructed as shown on the Contract Plans, are approximate only. Their exact location shall be determined in the field. Additional utility lines, whether abandoned or in service, may exist and it shall be the Contractor's responsibility to conduct its operations and take the necessary precautions to prevent interference with or damage to these or other facilities during the course of construction.

18. In the event the Contractor damages an existing utility service, causing an interruption in said service, it shall immediately commence work to restore service and may not cease its work operation until service is restored. Any damage caused by the Contractor's operations shall be repaired by the Contractor at no cost to the Owner.

19. The Contractor is to be aware that the water level in Olean Creek varies depending on evaporation, rainfall within the watershed, and downstream discharge. The Owner has indicated that the anticipated rise in water level in the creek is delayed several days following a significant precipitation event.

20. Work for this project is to be conducted during late summer/early fall when the water level historically in the creek has been at its lowest levels.

FLOOD EMERGENCY OPERATION PLAN NOTES

1. The Contractor shall monitor the forecasted and actual precipitation within the Olean Creek watershed and the potential effects on the water level in Olean Creek at the construction site. Early preventative action can be critical should a rain event be predicted. Coordination and consultation with the Owner is required during critical periods of construction. Prior to the start of construction, the NYSDEC and USACE shall determine and provide to the Contractor the "action" stage level of the creek that will initiate the flood emergency operation plan.

2. There are two USGS gaging stations located downstream of the project site. The first gaging station is USGS Gage 03010810 "Olean Creek at Olean NY" - this gaging station is located within Olean Creek and is shown to be deactivated. The second gaging station is USGS Gage 03010820 "Allegheny River at Olean NY" - this active gaging station is located along the Allegheny River at the City of Olean wastewater treatment plant.

3. The Contractor will be required to install a temporary staff gage along the bank of Olean Creek to monitor the water level in the creek, Item No. 022100-4.1, prior to the commencement of earthwork. The gage should be placed so that it is visible without obstructions throughout the construction schedule and so that it remains visible at increasing water levels. The gage should have graduated marks at each tenth of a foot of height and it should have additional distinctive marks at each whole foot. The "action" stage level determined for the construction site will be measured with respect to this temporary staff gage. The Contractor shall observe the stage level of the creek at the beginning and end of every day and shall report its observations to the Engineer and Owner.

4. Prior to the commencement of earthwork, the Contractor will be required to coordinate with the Engineer and Owner the criteria for backfill placement and compaction in the event of the "action" stage level of the creek. The criteria should be in general accordance with the Technical Specifications (Earthwork - Section 312323).

5. In the event the floodwaters are predicted to reach an "action" stage level within a 48-hour time span as identified by the NYSDEC and USACE, the Contractor must initiate the closure of the flood protection levee excavation as follows:

a. Stop all work, temporarily cover/cap-off exposed sanitary piping to a sealed condition, and remove any temporary excavation dewatering measures.

b. Backfill the excavation with stockpiled suitable fill material located near the excavation as indicated on Sheet 4 of the Contract Plans. The backfill shall be placed and compacted in general accordance with Note 4 of the Flood Emergency Operation Plan Notes.

d. Provide a 14-foot top width flood protection levee crest and match the existing embankment slopes on both the upstream and downstream sides.

e. Provide erosion and sedimentation control measures on the downstream slopes to prevent soil erosion during the "action" event.

f. When flood waters recede below the "action" stage level, the Contractor can resume work, at their own risk, by re-excavating the work area to the condition it was prior to the "action" event.

6. Contractor to notify the Engineer and Owner immediately upon enactment of flood protection levee excavation closure. A project contact list will be composed upon award of contract so that all involved parties remain informed.

7. The costs associated with the Flood Emergency Operation Plan will be negotiated in the event of the "action" stage level of the creek.

EXISTING CONDITIONS CROSS SECTION LEGEND NOTES

1. The August 1949 Plan shows that the 1949 Sanitary Manhole was to be removed. However, the December 1951 Plan does not indicate that the 1949 Sanitary Manhole was removed. There is a possibility that the sanitary manhole still remains and is buried under the flood protection levee. The August 1949 Plan and the December 1951 Plan show that the 15-inch diameter, 835 foot long, sanitary pipe that fed into the 1949 Sanitary Manhole (extends into the page to the north) was abandoned and plugged at both ends of the pipe. The August 1949 Plan shows that two corrugated metal square seep rings, approximately 5.5 feet in length and height, were to be installed around the 15-inch diameter corrugated metal pipe, at the 1949 Sanitary Manhole and approximately 15 feet west of the 1949 Sanitary Manhole. It is not clear on the December 1951 Plan if the seep rings were installed. Locations of these items will be field verified during construction.

2. The June 1952 Plan shows that the relief trench was constructed at the toe of the flood protection levee for a length of approximately 170 feet and was backfilled with graded sand and gravel. The relief trench was installed with an approximate width of 2 feet and extended approximately 0.5 feet into the pervious soil (i.e., sandy silt) for a total depth between 3 and 6 feet. Location and dimension to be field verified during construction.

SURVEYING AND RECORD PLANS

PART 1 – GENERAL

1.1. SCOPE

A. This work shall consist of all necessary surveying required to construct all elements of the project as shown on the Contract Plans or as directed by the Engineer in accordance with the specifications.

B. Work shall include but not be limited to stakeout, layout, and elevations for the limit of disturbance, new sanitary piping, and remaining sanitary utility structures by qualified personnel acceptable to the Engineer and Owner and the installation of a temporary staff gage.

1.2. RELATED SECTIONS

A. Earthwork – Section 31 23 23

B. Sanitary Utility Piping and Structures – Section 33 30 00

1.3. REFERENCE STANDARDS

A. New York State Department of Transportation (NYSDOT) Standard Specifications Section 625 – Survey Operations.

1.4. SUBMITTALS

A. Submittals shall be prepared and reviewed at least two weeks prior to the start of construction.

B. Submit qualifications for the surveying firm and the qualified personnel indicating a minimum of three years of experience with construction layout and record plan development.

C. Submit plan for construction survey control including survey equipment and location of baselines for control of work. Equipment shall conform to the latest NYSDOT Standard Specifications Section 625-2.

D. Submit plan for installation of temporary staff gage including equipment and location of gage.

PART 2 – MATERIALS

None

PART 3 – EXECUTION

3.1. TEMPORARY STAFF GAGE

A. Prior to the commencement of earthwork, the Contractor shall install a temporary staff gage along the bank of Olean Creek to monitor the water level in the creek. The gage should be placed so that it is visible without obstructions throughout the construction schedule and so that it remains visible at increase water levels. The gage should have graduated marks at each tenth of a foot of height and it should have additional distinctive marks at each whole foot. The “action” stage level determined for the construction site will be measured with respect to this temporary staff gage and will be provided to the Contractor prior to the start of construction. The Contractor shall observe the stage level of the creek at the beginning and end of every day and shall report its observations to the Engineer and Owner.

3.2. GENERAL

A. Work shall conform to the latest NYSDOT Standard Specifications Section 625-3.

B. Survey measurements are required to be accurate to within 0.12 inches (0.01 feet) in horizontal location and within 0.12 inches (0.01 feet) in elevation.

C. Prior to commencement of earthwork, the Contractor shall establish all necessary baselines, and horizontal and vertical control points, necessary to accurately complete the construction.

D. Existing grades shall be verified within the construction limits shown on the Contract

Plans, with information provided to the Engineer for review and issuance of proposed grades.

E. Approximate limits of the relief trench, as shown on the Contract Plans, shall be surveyed prior to sloped excavation to help verify during excavation if the relief trench is present. If the relief trench is encountered during sloped excavation, the Contractor shall survey the actual limits of the relief trench and reconstruct the relief trench in the existing location and to the existing dimensions.

F. As-built points, sufficient to define surfaces for measurements and payment, shall be taken at all surfaces, including: the ground surface following the completion of topsoil removal, following the completion of the sloped excavation, following the completion of the trench excavation, following completion of the relief trench excavation, and following placement of all soil types. As-built grades shall be taken at a maximum interval of 25 feet and at breakpoints to create record plans of the work completed.

G. The Contractor shall provide grades of the topsoil removal, sloped excavation, trench excavation, and relief trench excavation to the Engineer for review prior to placement of backfill materials.

H. As-built points shall also be made to define the geometry of new sanitary utility piping and all remaining sanitary utility structures to create record plans of the work completed. This includes all dimensions, pipe slopes, and invert elevations to determine if the piping and structures are within the design tolerances.

Olean Flood Protection Levee Page 02 21 00 - 3

I. All as-built grades shall be clearly marked on record plans in an approved format, acceptable to the Engineer and Owner. The record plans shall be submitted to the Engineer and Owner at the completion of the project.

PART 4 – MEASUREMENT AND PAYMENT

4.1. METHOD OF MEASUREMENT

A. Payment for surveying and record plans will be made on a lump sum basis.
